iii. Candidate who are eligible under category of Jawahar Navodaya Vidhyalaya and HSC

passed from schools under Jawahar Navodaya Vidhyalaya Schemes of any state

4. Bifurcation Mark sheet for ISCE Board students

5. Copy of NEET-UG-2017 Marksheet

6. Document showing place of birth & Date of Birth & Indian Citizenship (School leaving Certificate/

Transfer Certificate / Passport / Birth Certificate)

7. For SEBC, ST and SC Category: Cast certificate issued by Authorities of Gujarat State only

8. For SEBC Category: Non-creamy layer certificate (Parishistha ‘4’ in Gujarati/English) issued by

Authorities of Gujarat State only

9. Local Quota Certificate if admitted in Local Quota of Smt. NHL Municipal Medical College, Ahmedabad or

• Failing to bring any original documents, the candidate will have no right of admission.

10. પાર્સપોટયની નકલ [જો નાગડરકતા ડયઅુલ / મવદેર્ી હોર્ તો]

• જે ઉમેદવાર કોઈપણ અર્સલ પ્રમાણપત્રો લાવવામા ંમનષ્ફળ રહરેે્, તેવા ઉમેદવારને પ્રવેર્નો અમધકાર રહરેે્ નહીં.

• The candidates, who are of SC (Schedule Caste) category and having Free Ship Card, must report to Office of

ACPUGMEC, GMERS Medical College, Gandhinagar for reporting and submission of original documents.

• એર્સ.ર્સી. (મર્ડયલુ કાસ્ટ) વગયના અને ફ્રી મર્પ કાડય ધરાવતા હોર્ તેવા ઉમેદવારોએ ડરપોટીંગ અને અર્સલ પ્રમાણપત્રો જમા કરાવવાની પ્રડક્રર્ા માટે માત્ર અને માત્ર જીએમઈઆરએર્સ મેડીકલ કોલેજ, ગાધંીનગર ખાતેની ACPUGMECની કચેરીનો ર્સપંકય કરવો.

• For NRI Candidate: The candidates who will secure admission in Medical Course in NRI category, he/she

have to deposit Rs. 5.00 lakh by cash or by way of Demand Draft in favor of “ACPUGMEC” payable at

Gandhinagar in the branches of Axis Bank approved by ACPUGMEC. After confirming the admission in NRI

category in the admitted institute, the candidate has to pay prescribed tuition fees in US Dollar or

equivalent amount in any other foreign currency on or before 22nd August, 2017 at admitted institute.

ACPUGMEC will refund the deposit amount of Rs. 5.00 lakh after the completion of whole admission

process.

• એન.આર.આઈ. ઉમેદિાર માટે: એન.આર.આઈ. બેઠક પર મેડડકલ મવદ્યાર્ાખમા ંપ્રવેર્ મેળવેલ મવદ્યાથીઓએ રૂ. ૫.૦૦ લાખ ડીમાન્ડ ડ્રાફ્ટ “ACPUGMEC”, payable at Gandhinagar દ્વારા અથવા રોકડ રકમ દ્વારા ડીપોઝીટ રૂપે પ્રવેર્ મેળવ્ર્ા બાદ કમમટી દ્વારા મનર્ત કરેલ એક્સર્સર્સ બેંકની બ્ાચંમા ંભરવાની રહરેે્. એન.આર.આઈ. બેઠક પર પ્રવેર્ મેળવેલ મવદ્યાથીનો પ્રવેર્ જે-તે કોલેજમા ંસમુનમિત થર્ા બાદ તા. ૨૨/૦૮/૨૦૧૭ પહલેા પ્રવેર્ મેળવેલ ર્સસં્થામા ં મનર્ત કરેલ ફી અમેડરકન ડોલરમા ંઅથવા ર્સમકક્ષ મવદેર્ી ચલણમા ંભરવાની રહરેે્. અતે્રની પ્રવેર્ ર્સમમમત ખાતે ભરેલ ડીપોઝીટના રૂ. ૫.૦૦ લાખ રીફડં તરીકે ર્સમગ્ર પ્રવેર્ કાર્યવાહી પણૂય થર્ા બાદ પરત આપવામા ંઆવરે્.

• Failing to bring any original documents, the candidate will have no right of admission.

• Candidates who will get admission in Government Medical Colleges have to submit bond with amount of Rs.

5 lakh within one month of the commencement of semester. Candidates who will get admission in GMERS

medical college have to submit bond with amount of Rs. 2 lakh within one month of the commencement of

semester.

• ર્સરકારી મેડીકલ કોલેજોમા ંપ્રવેર્ મેળવેલ તમામ મવધાથીઓએ રૂ. પાચં લાખનુ ંબોન્ડ જે તે કોલેજમા ંપ્રવેર્ મેળવ્ર્ા બાદ ર્સત્ર ચાલ ુથર્ાના એક મડહનામા ંઆપવાનુ ં રહરેે્.
