Instruction to submit the Application Form for issuing

Indo-Bangladesh Passport. Applicants will be submitted the following documents (Xerox copy) with

their applications to fresh and renew for getting the Indo-Bangladesh Passport are given

below as follows:-

a) For mans (General) and womens (unmarried).

1. Voter card

2. Ration card

3. Residential certificate from Pradhan, Panchayet Samity or Chairman,

Municipality

4. Birth Certificate or Affidavit from Notary for date of birth with address

5. # Qualification certificate must be submitted who are qualified and

# Affidavit must be submitted from Notary for qualification who are

Home educated or illiterate person.

6. # Birth certificate must be submitted whose date of birth on and from

7. 01.01.1989.

8. Applicants will be shown above document’s original paper at the time

of submission of Indo-Bangladesh Application Form.

b) For mans & womens (Central Govt.& State Govt. employee / employees of Statutory Bodies and Public Sector Undertaking )

1. # Above a) 1 to 7 Sl. Nos documents

2. # Annexure “B” & “I” (these are in the instructions of Passport

Application Form)

# Above two (2) instruction are to be followed

c) For children age upto 18 years

1 Birth Certificate from Sub-Registrar, Birth & Death of Panchayet /

Municipality

2. Qualification certificate(i.e.Transfer Certificate or School Leaving

Certificate)

3. Annexure “H” (this is in the instructions of Passport Application Form )

4 Ration cards (own with parent)

5 Residential certificate from Pradhan, Panchayet Samity or Chairman,

Municipality

6 Voter cards (parent)

7 Applicant will be shown above original papers at the time of submission of

Indo-Bangladesh Application Form.

d) For women (after marriage)

1. # Above a) 1 to 7 Sl. Nos documents

2. # Xerox copy of Marriage Registration Certificate or # Joint Nationalized

Bank Passbook or # Annexure “D” this is in the instructions of Passport

Application Form.

3. # Above two (2) instruction are to be followed

e) **At the time of Hearing to be shown and produced the following

any one original Government documents before March, 1971 to the 1st class Magistrate, Nadia Collectorate.

1. Land deed (own name or father name or grand father name)

2. Migration Certicate (same as)

3. Citizenship certificate (same as)

4. Pension papers (same as)

5. or any other Government document before March, 1971.
