Message from the President

Bright Ideas for a New Year - Hello IntegrityM team -

Welcome to the inaugural edition of the Insider Magazine! 2016 is well underway and IntegrityM is busy delivering, enhancing, and expanding. We are very proud that IntegrityM is now a woman-owned small business and I personally want to thank the entire IntegrityM family for contributing to our continued success. We see this designation as another means to assist in achieving our overall growth strategy as set forth in our 2015 Strategic Planning sessions. As we move full steam ahead towards our strategic goals, it’s also important to look back and see where we’ve come from and what we’ve accomplished.

All who have accomplished great things have had a great aim, have fixed their gaze on a goal which was high, one which sometimes seemed impossible.

We are excited to announce that effective immediately IntegrityM is officially a Women-Owned Small Business, with ownership by the company's two senior officers, Nisha Shajahan (President & CEO) and Cheryl Gordon (Chief Financial Officer). As a Women-Owned Small Business (WOSB), continuing under Nisha and Cheryl's leadership, IntegrityM now has even greater opportunities for business growth in both federal and state government agencies. Along with the WOSB status, IntegrityM has also now successfully achieved its own incorporation as Integrity Management Services, INC., and therefore is no longer under the umbrella of Strategic Management. These achievements would not be possible without the leadership of Nisha and Cheryl. Having served with the Strategic Management companies for fourteen years, Nisha Shajahan led the very first Program Integrity contract that our company ever endeavored back in 2006. From there, she went on to spearhead the growth of our program integrity line of business and the eventual establishment of Integrity Management Services in 2009. Likewise, Cheryl Gordon has been with our company for over 19 years, and at the forefront of every major business initiative undertaken. This includes helping our company to branch out from exclusively commercial work to diving into government contracting in 2005, to supporting the start and build up of our program integrity business initiatives in 2006-2009, and ultimately establishing Integrity Management Services in 2009. Since then, both Nisha and Cheryl have been instrumental in leading our company’s growth and getting us to this remarkable achievement that we celebrate today. Congratulations and thanks to Nisha Shajahan, our President and CEO, and Cheryl Gordon, our CFO, for their utmost dedication to the growth of IntegrityM from its very inception, and for their relentless focus on the success of our company, our employees and staff, and our clients. And please join us in celebrating our new status as Integrity Management Services, INC – a Women Owned Small Business!!!

The Big Short Thumbs up for the movie, The Big Short, directed by Adam McKay and based on the book, The Big Short: Inside the Doomsday Machine by Michael Lewis. It provides a detailed (and entertaining, from our vantage point in 2016), description of the build-up of the bubble generated in the housing market that led to the financial crisis of 2007-2008. Although the financial system in principle adds value to society – essentially by managing/transferring risk across individuals and firms with different abilities to bear risk – the sector in the years preceding the crisis 2007-2008 was actually generating a gigantic systemic risk to the economy at large…and the way it did is very well explained in The Big Short.

Namaste Restaurant, Alexandria, VA: Namaste has proven itself time and again… ultimately making a regular out of me, as they specialize in Indian and Nepalese food. Although I've gone for dinner, I tend to go for brunch during normal work hours. They always have the best service there, even as a solo diner. Their brunch buffet isn't the hottest, but it sure is flavorful. I particularly like their chicken tandoori, various curries, rice dishes, and carrot pudding dessert. Their naan (flat bread) is really good but can be a bit greasy for my taste. I generally spend $14 for the buffet which is a fair price, especially since it’s an all you can eat. How is it different from Aditi? It just tastes better. Boy, oh Boy (Baby Announcement!) Lindsey Yeager, one of our team investigators’, welcomed a baby boy back in October! The pictures below are Luke Scott Yeager at three months old and also the family’s most recent picture taken during the holidays! Lindsey writes, “he is the happiest, sweetest, little boy and so loved by everyone. Such a blessing!” Congratulations, Lindsey and family! Luke Scott was born Thursday 10/08/2015 at 5:33pm EST. Weight: 7lbs. 10 oz. Length: 19.25 in. Proud Parents: Lindsey and Scott

Recruitment Efforts IntegrityM continues to add to our brilliant staff by on-boarding a total of 11 new employees between March 2015 - December 2015. The HR Express is always recruiting from stop to stop! IntegrityM University As announced in the last newsletter, we unveiled the IntegrityM University Training Program. We are continually working to make this a success and will be issuing a course catalog, details of when the trainings will be available, and what courses are mandatory. As we continue to build out this program, new course offerings will be added and announced. As with any investment, we want to assess the effectiveness of the program’s courses and the first step will be to evaluate the “Reaction”. Reaction encompasses the attendees training experience, including relevance of topic area, was the training valuable, the topic covered, the material presented, the instructor, and the venue. Employee Morale Continuing our goal of dedication to our employees and striving to promote positive employee

morale efforts, IntegrityM hosted several initiatives in 2015 . Please check out the rest of the issue for all of the exciting details! Feel free to contact HR and/or Office Management directly with any employee morale event ideas. Benefits We concluded another open enrollment season. This season brought the addition of optional Voluntary Life Insurance. All current staff that did not enroll this year will have the opportunity to do so during the next open enrollment season in May 2016, which is just around the corner. All new hires will be eligible to participate in this benefit during their normal benefit eligibility period. SHANE BYRNE We are pleased to announce the promotion of Shane Byrne to One PI Client Services Lead! Shane joined the IntegrityM team in Jan 2014 providing Data Coach and subject matter knowledge support to the QSSI One PI contract. The team continues to grow, and Shane's leadership of the Data Coach and Help Desk teams will be integral as we move forward supporting the One PI contract. Please congratulate Shane on a job well done! MEGAN EGUCHI We are pleased to announce the promotion of Megan Eguchi to Data Analysis Team Lead! Megan joined the data team in Feb 2015 and has been a trailblazer supporting the ZPIC contracts, ever since. She is an excellent team player and is a phenomenal SAS programmer who has been instrumental in helping the IntegrityM team develop its’ prototype analytics detection tool. Megan will provide team leadership supporting the ZPIC Pennsylvania and New York Medicare-Medicaid (PMMDAC, NMMDAC) contracts. MARIA HABIB We are pleased to announce the promotion of Maria Habib to Sr. Operations Support Specialist! Maria has been a part of the IntegrityM team since its inception in February 2009; however, she originally started in April 2007! She has supported the organization’s operations fulfilling various roles including support of the Executive Management team, contractual compliance, general operations support, and contracts management. She has been the champion of Change Management and the company's record keeping and document management program. In her new role, Maria will help to formalize the contracts management process, continue to help build out the document management and record keeping program, and work with the various IntegrityM business units to ensure process compliance. Please join us in congratulating Maria! WHITNEY MOHR We are pleased to announce the promotion of Whitney Mohr to Project Manager! Since starting in Oct 2012, Whitney has worked on a variety of contracts. She has displayed excellence in team leadership and management of many tasks and projects. Whitney has been the recruiting arm of IntegrityM, successfully working with department heads to identify and recruit many of the excellent staff we have onboard. Sujata has fulfilled many roles with IntegrityM including compliance officer and security officer. She has written many of the company policies and procedures and has helped to implement many processes and procedures throughout the various IntegrityM business units. Sujata will oversee the IntegrityM Security Program working closely with the various departments to educate, enforce, and monitor our performance to comply with industry and internal security best practices. She will provide day to day project lead support to the IntegrityM CRC team ensuring quality products are developed, continuous improvement of application support process and procedures, and maintenance of project deliverables. New Faces Albert Appiah

Albert received a B.S. in Economics and Statistics from Garden City University College and M.S. in Biostatistics from George Mason

University. He previously worked with Albert Acquah and Associates, a Consulting firm as a Statistical Analyst where he established himself as a team leader and a researcher. Albert has experience in big data analytics, data visualization, and

Multivariate Statistical Analysis. Albert also has ten years’ experience in using SAS and R and joins IntegrityM as a Statistician. He is a big soccer fan of Manchester United. Mark Baldwin Mark is a college graduate with a B.S. in

Telecom-munications and Film from San Diego State University, with additional education and training in interrogation and interviewing from the

National Insurance Crime Bureau (NICB) SIU Advanced Academy, and is a Certified Fraud Examiner. All in all, he has over 20 years of investigations experience! Mark enjoys data mining and cold-calling individuals. When he’s not on the job, you can track Mark down gardening on the patio, golfing (sometimes hacking) on the fairways, or trap shooting (when he has the time). Currently, Mark is working as a Los Angeles Field Investigator out in sunny California.

Dan Golden After receiving a B.A. in Spanish Studies from American University's College of Arts and Sciences program in 2002, Dan is currently back in school at NOVA Community College,

going for a certificate as a Database Specialist. His previous employment also included a Medicare/Medicaid background, specifically in his last role as a DBA data analyst which focused on Opt-Out, ICD, DME, SNF, and FQHC updates… just to name a few. Even before that role, Dan also worked in numerous Business Objects, SAS, Access, Oracle, and SQL Server environments in various database analyst/programmer roles. Dan lives with a shepherd dog mix named Saké and is an avid lover of all things music and concert-related. Kimberly Hornik

Kimberly (Kim) Hornik previously directed HMS Federal’s Compliance Program, ensuring adherence to federal regulations governing privacy, security, business conduct, and

conflicts of interest. In addition, she was responsible for policy development and implementation, compliance training and communications, auditing, and issue reporting and resolution. She also previously served as HMS’ Federal’s Privacy Officer and Contract Administrator and was the chair of HMS Federal’s internal Federal Compliance Committee. Kim has comprehensive knowledge of Medicare and Medicaid rules and regulations, HIPAA regulations, and federal contract law. Kim has also served as a practicing attorney, Chief Compliance Officer for a radiology benefits management firm, and was the owner and president of a compliance consulting firm. Kimberly received her B.S. Degree in Professional Management with distinction from Nova Southeastern University and her JD Cum Laude from The Thomas M. Cooley Law School. She is a member of the State Bar of Michigan and the American Bar Association. Kim started in March and serves as IntegrityM’s Vice President of Compliance and Administration.

Stacey Kernisan Stacey received a B.A. in Interdisciplinary Studies from Virginia Tech and is currently pursuing her M.A. in Adult Education and Training from the University of Phoenix.

She joined IntegrityM as a Training Specialist after being the Training Specialist for the Mid-Atlantic Americans with Disabilities Act Center. Stacey is a true Hokie fan and enjoys spending time with her family. Stacey currently works on the CMS Trusted Third Party (TTP) contract. John McNulty

John has B.S in Business Administration, from Bryant College in Rhode Island and a M.A. in HR Training and Development from Seton Hall University in New Jersey. He was hired as

an investigator for the NEBISC contract having 25+ years in law enforcement with The New Jersey State Police (NJSP). His last eight years at the NJSP were as a Detective and Supervisor. John, his wife, and son, moved to Cape Cod in June of 2013 following his retirement. Robert Meister

Robert received a B.S. in Psychology and a M.P.H from The University of Memphis. He joins IntegrityM as a Statistician after serving in various capacities ranging

from statistician to research analyst in both academia and private companies since 2009. Robert believes college football was born from the SEC conference and is looking forward to the current season! Malar Nachi

Malar received a M.S. from University of Houston in Computer Science. She joined IntegrityM in mid 2015 after working as a data analyst for over ten years! She has been in Virginia

for seven years .Malar enjoys traveling, cooking, and yoga. She is currently is a Sr. Data Analyst supporting the ZPIC contracts.

Lindsay Williams Lindsey is from Stafford, Virginia, but has lived all over the U.S. and also in other countries. She enjoys outdoor activities and spending time with family and friends. She received a B.A. in Sociology from the University of Mary

Washington in Fredericksburg, Virginia. Lindsey is excited to join IntegrityM as the Receptionist/HR Assistant, and has years of experience working as an Administrative Support Specialist. Jimmy Yuan

Jimmy received his B.A. in Economics from University of Maryland –College Park in 2013 as well as a M.S. in Business Analytics from The George

Washington University (GWU) in 2015. He previously worked as a Graduate Teaching Assistant for the GWU MBA course, Data Analysis and Decision Making, in spring 2015. He is a self-motivated person with an eagerness to explore the data science field. Jimmy loves hiking, road-trips, and all fun activities. Donna Zeybel

Donna received her B.A. in Computer Information Systems in 2002 and then went to work for a company called Deltek. Many years later after being a stay at home mom to her three children she

got a job working at Holy Spirit School so she could be with them during the day. Donna enjoys spending time with her family and going to the movies. Donna currently is a project lead working on the CRC contract.

At IntegrityM, it is our consistent commitment to standards that achieve uniformity of the product and services we provide to satisfy our customers and requirements. Continuous quality improvement increases customer satisfaction, reduces production waste and improves IntegrityM’s performance.

We want you to develop ideas for improvement! As an employee, you are immersed in daily production/processes of developing high quality deliverables. By detailing your

work activities and performing an analysis of the data, you can assist in continual improvements which aim at increasing quality. Taking a closer look at how you spend your time, can provide recommendations to the Quality Improvement Team (QIT) ways to perform a process more efficiently and eliminate waste. We appreciate everyone’s commitment and individual contributions ensuring quality is implore throughout the organization! Additionally, the team demonstrated commitment to quality when we successfully achieved ISO 9001:2008 recertification in March last year.

Trusted Third Party is Here! CSRA recently won the TTP contract, with IntegrityM as one of their subs. Mike Keller joined the TTP team as a Subject Matter Expert on all things law enforcement and data analytics. Stacey Kernisan joined IntegrityM and the TTP team as a training specialist. We hold out hope for additional positions, most likely a statistician(s) and/or data analyst(s)! One MCO Audit Down Not so long ago, the Government Accountability Office (GAO) released a report taking to task those responsible for overseeing Medicaid MCOs. To help address this criticism, HMS Federal took the plunge into MCO auditing, and invited us along. Our auditors contributed significantly to planning for and conducting the audit of a DC-based MCO. The resulting audit report was well received by CMS and DC Medicaid. We developed an audit guide summarizing our experiences, which will be

useful for future MCO audits. If you hear of anyone looking to audit an MCO, be sure to send them our way! Review Process Change For some time, IntegrityM supported HMS Federal by reviewing audit efforts at the end of the audit process. Although HMS found the information we provided helpful, offering feedback after the team had completed the audit work reduced the feedback’s usefulness. To improve the utility of our efforts, we worked with HMS to develop an Iterative Review process. This process allows for more immediate feedback throughout the audit process. Reviews occur at agreed upon checkpoints while the audit team is conducting the audit. Our review team can then observe and comment at a time when the audit team can make changes for the betterment of the ongoing audit. Additionally, the iterative review process is intended to incorporate greater communication between our team and the HMS audit team. New HireRight Work HireRight reached out to us to create a new work product. The new report summarized

into a single document the background check requirements we identified when creating the individual 50 State summaries. HireRight believes this document will enhance its relationship with its customers. Changes to Address ICD-10 Each month, we use Medicaid data from the State of South Dakota to identify Medicaid recipients who are eligible for the Health Home program and attribute those recipients appropriately. The original attribution methodology relied on ICD-9 codes. With advent of ICD-10, Kim Upper has ably adapted the code we use to accommodate ICD-10. Investigators on the Move John McNulty has joined the NEBISC team. His first day in the Hingham, MA office was on January 11, 2016. Tim Bucher is moving from the EA and PENN BISCs to also support NEBISC. Brad Hill is supporting all three BISCs conducting RFIs.

The Security team attended the annual CSCOUT conference in Dallas, TX on February 2-4, 2016. CSCOUT stands for CMS Security Control Oversight & Update Training. The training is provided annually or twice a year by CMS to help contractors understand CMS and industry security requirements in order to plan effective strategies and mitigate security concerns. This year’s focus of the training was on incorporation of risk-based strategies to ensure potential threats and vulnerabilities to an organization’s infrastructure are regularly reviewed and assessed for potential risks. For our 2015 May Morale Initiative we focused on May 15th being National Chocolate Chip Day. We turned that into a bake off to see who/what makes the best chocolate chip cookie. On May 27th, we held the bake-off.

Katie made a regular chocolate chip cookie that was baked around a Hershey’s Kiss.

Natasha brought a double/triple cookie (please ask her for her recipe!). After much tasting the winners were: 1St Place - Natasha’s Double/Triple. 2nd place Katie’s Kiss Cookie. Along with the cookies, the team enjoyed a lunch hour Tea Party! The team enjoyed sampling a variety of teas brought in by the staff. Cathy brought in some flavors such as: Mind and Body-Lemongrass and apple, Bengal Spice-cinnamon tea, Morning Mate-caffeinated tea, and Jasmine Green tea. Sarah brought in some Almond tea, Maria

brought in Ahmed Cardamom Black tea, Nisha brought in Ceylon black tea from Sri-Lanka, and there were also a few green tea samples!

Of course, milk was also served, because no cookie eating is complete without a glass of milk!

Is your digital desktop cluttered? Put all those items somewhere for safe keeping! Your “Home Drive” which is mapped to the Z: drive is the place. It is somewhere

that only you have access to and it is backed up daily. If something happens and your laptop is out of commission, all of your files are safe and sound. How is your inbox looking? Now is a good time to take a look and see if you can organize all those emails. Creating folders for departments and people can quickly declutter any inbox. Setting up rules using the rules wizard is fast and easy. For those still using Office 2007 here is a great online ‘how to’. For those already on 2013, just click the Rules icon at the top of your Outlook and then Create a Rule. From there you can follow the 2007 online tutorial. With so many animal lovers in the office, the team wanted to lend our support to a local Animal Shelter. We chose the Vola Lawson Animal Shelter. For further information, you can see the website, here. Some items the team collected are blankets, towels, and animal food. For October, we raised money for Breast Cancer Awarness, and some of us participated in Runs/Walks. November was an extra busy month where we participated in events for Alzheimers’ Disease Awareness Month AND Donated canned goods and other goodies to give food baskets to New Hope Housing! Just in time for a great Thanksgiving meal. December had us also give food baskets to New Hope Housing and create other food drives for those in need.

We have put together some fun shortcuts for the team to use in their day to day work. ALT+PrtScn | Current Window Print Screen | Using the two keys together copies the window that is currently selected onto the virtual clipboard. From there you can CTRL+V (Paste) into a document of your choosing. Using keyboard shortcuts increase the speed you can operate a computer – especially if the keyboard shortcut reduces your need to move your hand from keyboard to mouse, back to keyboard.

CTRL+X, CTRL+C, CTRL+V Cut, Copy, and Paste

The ‘clipboard’ is a virtual data space where information can be temporarily stored. The Copy command stores a copy of a selected object in the clipboard. The Cut command is similar to the Copy, however it removes the selected object. The Paste command inserts the most recently cut or copied object.

ALT+TAB, +TAB; ALT+SHIFT+TAB, +SHIFT+TAB

Switch application

Holding [ALT] then [TAB] switches to the most recent window/application you previously had open. Continuing to hold ALT and pressing TAB will cycle through all open windows. Using ALT+SHIFT+TAB cycles reverse order. Using the Windows Key (The key between the CTRL and ALT) will provides a 3D effect for switching between open applications.

+L Lock computer Shortcut to lock your screen. CTRL+PAGEUP, CTRL+PAGEDOWN

Switch between open tabs\worksheets

In applications that contain tabs (such as Excel, Chrome, Firefox), this shortcut switches to the next/previous open tab.

+↓, +↑ Move current application

Pressing the WINDOWS key and an ARROW key will move the current window. Useful for moving windows between two monitors. Also useful for ‘snapping’ the current window to exactly ½ the screen.

CTR+Z, CTRL+Y Undo, Redo Undo command reverses your last action, Redo reverses the undo. CTRL+A Select All Select everything CTRL+P Print Open the print dialog box CTRL+ScrollWheel, Zoom CTRL the scroll wheel on your mouse zooms in and out.

Ann-Marie Akiwumi, Sewit Araia, Sarah Langford, and Maggie Chang hopped right on the business development/proposal bandwagon when support was very much needed. High fives’s all around, team on the outstanding job! Mark Baldwin received great accolades by Jarrod Duggar, the ZPIC 1 Team Lead, Fraud Investigations for ZPIC 1 TO 2. “ I want to commend Mark on his tenacity and outstanding cognizance of his investigations. I have had the privilege of having Mark on my ambulance blitz team over the last few months. Mark is very good at keeping me abreast of the progress of his investigations as they develop. Recently, Mark was assisting with a site verification with another investigator when he ran across the name of an ambulance provider he is investigating at a different location than what is reported to CMS. Mark, being the inquisitive investigator that he is, dug a little deeper and gained valuable information

regarding the ambulance provider. Mark’s investigative intuitiveness led to what could be an additional investigation and a potential for additional administrative action in the name of protecting the Medicare trust fund. Mark reported his findings to me, and collaboratively, based on the new information and information I gained during the on-site, we have determined that a doctor, a nephrologist, is possibly a part owner of an ambulance provider that primarily transports for dialysis treatment. Mark conducted a second visit to the location to gain even more information regarding the relationship between the ambulance company and the doctor.While Mark is certainly not new to conducting investigations, his tenacity and investigative skills stand out and is greatly appreciated.Congratulations to Mark, and keep up the outstanding work. Mark is an inspiration and a pleasure to work with.”

Business Development News This year has been busy for us in the Business Development (BD) team, averaging five and a half proposals a month (whew!) and securing some strong partnerships. Big Wins, New Contracts The biggest news is that we were successful in winning Trusted Third Party (TTP) with our team leaders, Computer Science Corporation (CSC). We are so excited to be a part of this new contract! This contract is highly visible to the HHS community, so look for more communication in the future! Expanded and new Partnerships Sellers Dorsey has won a spot on the South Dakota professional services contract. This allows us to continue the work we’ve been doing for the state, and also gives us a door to new business that could expand our partnership. Go Team! We finished the BD season making great new partnerships to do future business with, including talks with CSRA, Noridian, and HMS.

Bring your Child to Work Day! In April, we participated in the “Bring your Sons and Daughters to Work” Day. Many of our team joined in, and we had so much fun having our ‘little wokers’ help out for a day! As you can see from the pictures, they attended a meeting or two and worked on some fun assignments along with each one of them getting their own personalized badge made!

Starlight, Star bright…

Ray Wang was recently seeing stars in Canaan Valley, West Virginia! Ray told Insider Magazine, “the elevation is above 3,200 feet [and] the night sky was clear without light pollution. [It’s] the best place to check out our home galaxy!”

Cathy’s Green Thumb As some of you know, Cathy Furlong is into plants and gardening. Her house was in the Fairfax County Sustainable Garden Tour, in Fairfax, VA. She had an open house on Sun, June 14, 2015 from 1-5 PM. Her house and several others were on the garden tour. Paulo in Dallas Paulo Macedo presented a paper at the SAS Global Forum 2015 in Dallas April 26-29.The presentation was attended by 55 conference participants. Joyce Martin played the role of “Best Dallas Host Ever!” by taking Paulo on a tour of local landmarks before the beginning of the meeting… including the popular Longhorn Herd statue square where the attached picture was taken.

STRATEGIC PLANNING: Product Development winning teams Members of the IntegrityM Data Team competed for IntegrityM Innovation Awards during the November 2015 Strategic Planning

session. Six teams were narrowed down to three to compete for the overall 2015 Innovation Award. They presented their product

development innovation to IntegrityM Executive Management in December. The presentations were intriguing and inventive! The

competition ended with a tie!

An IntegrityM Halloween

For the month of October, we decided to hold a Cube/Office Halloween decorating contest! The rules were simple….get as creative as you want! Some folks went big and turned their cubicle into haunted cubes, or simply put up a few pictures of some pumpkins, or just one. Either way, there was a prize for decorations and the IntegrityM judging panel voted on October 28 and our very own Erin Green (nè McClain) won! Congratulations! What’s New With IT? Connecting Remotely? With winter in full effect, make sure you can connect remotely without a hitch! Did you know that we can test your VPN connection just as you would be connecting remotely from right in the IT Office? This makes trouble shooting easier when you are at the office, and you will be prepared in case you need to work from home for an extended period of time! Remember while connecting from home, if connecting to the IntegrityM network using the VPN you need to be hard wired to your home network. No exceptions. While viewing email only you can be connected to a trusted Wi-Fi network. As always, no PHI or PII should be transmitted using email! Are you using SharePoint to its full potential? The IntegrityM SharePoint site is http://integritym-home. The site can only be accessed when connected to the IntegrityM network. Did you know that there is an updated company contact list and floor plan right there on the site to help you find and reach your fellow coworkers? The site also shows a live Twitter Feed! The main page is also full of useful links and information, such as who’s just started and also our trusty dusty change management transmittals! If you aren’t already, start accessing the Master Documentation tab for any forms, and/or other documents you may need. The search function on the top right will help you find what you are looking for! Happy document hunting! Need help from the IT department? Open a ticket quickly by emailing [email protected].
