    y Monitor urinary function, prevent or relieve bladderdistention.

    y Provide continuous bladder drainage.

    y Obtain sterile urine specimeny Measure residual urine.

    y Provide means for irrigating the bladder with fluids ormedications.

    y It is an introduction of a rubber or plastic catheterthrough the urethra into the bladder.

    y It provides continuous flow of urine in patients unable

    to control micturation or in those with obstruction tourine flow.

    y All catheterization should be performed under thephysicians order.

    y Catheterization is done underAseptic Technique

    throughout the procedure.y If possible, Male nurse will do the catheterization to

    MALE patients.

    y The urine bag should be hanged at all times

    y It should be changed every 2 weeks.

    y Assessment of patient.

    y During catheterization, if any resistance is felt!

    DO NOT USE FORCE AGAINST THE REISITANCE

    REMOVE THE CATHETER IMMEDIATELY AND

    NOTIFY THE PHYSICIAN.

    y EQUIPMENTS:

    y Catheterization set:

    -sterile gloves

    -foley catheter

    -lubricant

    -cotton balls-forcep

    -antiseptic solutions (betadine)

    -sterile container/cup

    -drapes pads, bath blanket/sheet for draping.-syringe with 5 ccwater

    -standing lamp/ light source

    PROCEDURE

    (MALE)

    y Assess patient for the following:y Time of urination

    y Level ofAwareness and knowledge of purpose ofcatheterization.

    y Mobility and physical limitationy Patients age

    y Pathological condition that may impair passage ofcatheter

    y Verify physicians order & identify patient.

    y Explain the procedures to the patient

    y Provide the patient with opportunity to performpersonal hygiene (penile)

    y Wash hands.

    y Position patient in supine position with only thegenitalia exposed.

    y Drape the patient.

    y Open catheterization tray.

    y

    Put on all sterile gloves, and sterile materials.

    y With non dominant hand, hold the penis at a 90 o

    angle to the body.

    y If the patient is not circumcised, pull backthe foreskin to visualize the meatus.

    y Clean the urinary meatus with one downward stroke or

    use circular motion from meatus to base of penis. Drythe meatus.

    y Lubricate the catheter and gently insert into theurethra (approximately 8 inches) until urine begins to

    drain.y Insert an additional inch (2.5) and inflate the balloon.

    y Check the placement by gently pulling on catheter.

    y Connect the drainage bag and tape catheter to the

    abdomen. (taped to the thigh to straighten theangulation of penoscrotal junction)

    y Wash hands.

    y Record the time of completion of the procedure andamount and color of urine and time.

    y Notify physician for any abnormalities.

    y Assess the patient for the following:y Time of urination

    y Level of awareness and knowledge of purpose ofcatheterization.

    y Mobility and physical limitation.y Patients age

    y Pathological condition that may impair passage ofcatheter.

    y Position the patient in a supine position with knees

    bent, hips flexed, feet resting on bed.y Direct visualization of genital area.

    y Position moisture proof pad under the patientsbuttocks

    y Separate Labia minora so urethral meatus is visualizedone hand to maintain separation of the labia untilcatheterization is finished.

    y Cleanse around the urethral meatus with a povidoneiodine solution.

    y Introduce a well lubricated catheter 5-7 cm (2-3inches) into urethral meatus using strict aseptictechnique.

    y Secure the catheter, inflate the balloon according to

    manufacturers direction.y Allow urine to flow through the catheter before

    collecting a specimen

    y Place the patient in a comfortable position.

    y Record the time of completion of the procedure andamount & color of urine and time.

    y Notify physician for any abnormalities.
