• Provideandmaintainahealthyandsafeworkplace.• ParticipateinopencommunicationregardingissuesonOccupationalHealthand
Safety.EmployeesandContractorsareexpectedto:
• Workandencourageotherstoworkinahealthyandsafemanner.• Reportanyunsafeconditionsthatcometoyourattention immediatelytoyour
StaffAustraliarepresentative.• Notifyyourconsultantofanychangeindutiesfrominitialjobspecificationgiven
byStaffAustraliaIMMEDIATELY.Intheeventofaninjuryatwork:
• Participate in a Return toWork programwith guidance and consultationwithyourStaffAustraliaReturntoWorkrepresentative,yourdoctorandconsultingdoctors.
RehabilitationPolicy
Staff Australia commits, with its contractors and employees, to the responsibility ofprotectingtheirhealthandsafety.Intheeventofaninjuryoccurringwewillmakeeveryefforttoprovidethetreatment,careandrehabilitationnecessaryfortheindividualtoreturnintheirformerpositionandduties.Where required, this process shall be via a graduated program (Return to work plan) oralternativeduties,whichwillenablethemtobecomeindependent.StaffAustraliahas implementedproceduresandrelationshipsofthehighestquality,whichcomply with, or exceed the requirements of current Occupational Health and SafetyLegislation.
• Bepositiveandhonourcommitmenttowardsyourrehabilitationandreturntoworkprogramwhenrequested.
Anyreturntoworkprogramisdesignedwithyourbestinterestsandhealthasourpriority.Suchprogramsaredesignedwithinputfromyourself,yourdoctorsandyourStaffAustraliarepresentative.Our Rehabilitation policy is located in the induction room of your localStaffAustraliaoffice.

WhatToDoIfYouAreInjuredIfyouareinjuredwhileonassignmentyoumustfollowthesesteps:
• NotifyyourStaffAustraliarepresentativeandsitesupervisorimmediately,evenifnomedicalattentionisrequired.
• Ifyouvisitadoctor,obtainaWorkCoverCertificate.We have organised some local doctors, ask your Staff Australia representative fordetailsifaninjuryoccurs.
• PROMPTLYcompleteall compensationandcompany forms,whichyouwill receivefromStaffAustralia.
• If Staff Australia insurance managers organise medical examinations, you arerequestedtoattend.
GeneralSafetyandGoodHousekeepingDuring your induction, Staff Australia will inform you of your responsibilities in terms ofOccupational Health and Safety. We will also spend time looking at likely injuries in theworkplaceandhowtoavoidthem.Pleasegiveyourfullattentionduringourinductionprogramme.Yoursafetyisournumberonepriority,pleasemakeityours.SafeWorkingPractices
• Observeallsafetysignsandwalkways.Neverstepoverorwalkonconveyors.• Never use any equipment or machinery for which you are not qualified AND
experienced.• Payattentiontoandinformyoursupervisorifyouobservedangeroussituations,i.e.
Tangledelectricalleads,ripped/tornfloorcovers,etc.• Keepallflooringandshelvingaroundmachineryclear.Familiariseyourselfwithand
usecorrectstorageareas.• Keepyourworkareacleanandfreefromrubbishandspills.• Donot skylarkorhorseplay in theworkplace. (Notonly is thisdangerousbutwill
alsodecreaseyourchancesofbeingrequestedformoreworkatthesite.)• Useofradio’s,headsetsandmobilephonesisstrictlyforbidden.• Donotwearjewelleryorloose-fittingclothingand,ifyouhavelonghair,tieitback.
MachineryandEquipment
• Donotleavepowertoolsorequipmentrunningwhennotinuse.• NeveroperateequipmentormachineryunlessyouarequalifiedANDtrained.• Familiarise yourself with correct operating procedures, especially Emergency Stop

ChemicalandSubstanceHandlingBefore handling or working in spaces containing hazardous chemicals or substances, youmust be fully inducted at the client’s premises and understand the correct operationalprocedures.Ifyouareunsure,don’tbeafraidtoaskquestions.Injuriescanoccurbypoisoning,suffocationorchemicalburns.Onsite:
• All containers must be identified with the content name. Many chemicals arehazardousandhavegovernmentlawsfortheiruse.
• Protect the body; i.e. wear gloves, protective clothing and nose, eye and mouthprotectionwhereapplicable.
• Practicegoodhygienebywashingthoroughlypriortoeatingetc.• Familiarise yourself with locations of wash areas, eye flushes, first-aid equipment
andexhaustfanswitches.• ALWAYSKNOWWHATYOUAREHANDLING• You should be aware of the side effects from any prescribedmedication you are
taking. Somedrugsmaycausedrowsiness,whichmayplaceyouatriskwithintheworkplace.Letyoursupervisorknowifyouaretakingmedication,whichmayaffectyourabilitytocompletethetask.
PPE–PersonalProtectiveEquipmentPPE is necessary to protect us from various Occupational Hazards. Although it doesn’teliminate thehazard completely, itwill still reduce the risk of damage to eyes, ears, skin,bodyandboththerespiratoryanddigestivesystems.
• Helmetsandhatsprotectyourheadfromfallingobjects.• Earmuffsorplugsprotectyourearsfromnoisehazards.• Protectiveeyewearsuchasglassesandgoggles–protectyoureyesfromchemicals
ordebris.• Glovesprotectyourhandsfromchemicals,machineryandsharpobjects.• Respiratorsormasksprotectyoufrombreathingindust,fumesandvapoursetc.• Protective clothing suchasaprons,hi vis vests,overalls, steel capped safetyboots
protect your body from falling objects, chemical splashes and poor visibility ofothers.
DO NOT operate machinery or tools if you are taking medication which may make you drowsy.
YOU MUST WEAR STEEL CAPPED SAFETY BOOTS ON SITE WHILST WORKING FOR STAFF AUSTRALIA.
You must also wear all other appropriate PPE when instructed to and where signs indicate that you need PPE.

EnvironmentalRisksSunlightandSkinCancerExposuretosunlightovermanyyearsmaydamageyourskinandcauseskincancer.
• Protect yourself with ample clothing (including sunglasses and a hat) and userecommendedsunscreeneverytwohours.
• Youandyourdoctorshouldregularlycheckyourskinforcancergrowths.• Workandrestintheshadeasmuchaspossible.
HeatHeatexposuremaybe caused fromdirect sunlightorplant rooms forequipment, suchasfurnaces.
• Ifyouarenotphysicallyfitandhealthy,youwillrequiremedicalconsenttoworkinhotenvironments.
• Drinkplentyofwatertoreplacelostfluidfromsweat.• Avoidalcoholatleast24hoursbeforeworkinginhotenvironments.
Cold(e.g.FreezerorCoolRooms)
• Knowthelocationofallexits,especiallyemergencyexits.• Alwayswearsuitableclothing,includinghatandgloves.• Alwaysmakesureasupervisorisawareyouareenteringafreezerroom,incase
youcollapse.
Someofourclientsitesarenonesmokingsites.Whensent toworkat theseplacesDONOTSMOKEONSITE.Onothersites,smokeinspecifiedareasonly.Notakingmatchesorlightersintoproductionareas.

Bullying,OccupationalViolenceandHarassmentPolicyBullying– is repeatedunreasonablebehaviourdirected towards anemployeeor groupofemployeesthatcreatesrisktohealthandsafety.Occupational Violence – is an incident where an employee or group of employees arephysicallyattackedorthreatenedwhichcreatesrisktohealthandsafety.Harassment(SexualHarassmentorPhysicalHarassment)-Harassment is the discrimination against an individual or group based on age, religion,gender,politicalpreferenceorsexualpreference.THERE ARE STRICT GOVERNMENT LAWS AND PENALTIES AGAINST INDIVIDUALS WHOPRACTICETHESETYPESOFBEHAVIOUR.StaffAustraliaandourclientswillnottoleratebullying,occupationalviolenceorharassmentwithin the workplace. Investigation and counselling will be pursued where an allegedincidenthasoccurred.Theoutcomemayresultindismissal.REMEMBER: Everyone has different tolerance levels and ideas about what could beoffensive language and behaviour. So, please be sensitive to the situation and yourworksite.Behavioursuchaswhistling,gesturingormakingsuggestivecommentswillnotbetolerated.

AlcoholandDrugsPolicySTAFFAUSTRALIAPTYLTD is committed toprovidinga safe,andas far isaspracticable,ahazard free environment for all workers, visitors, customers and the public. STAFFAUSTRALIA PTY LTD also believes that all personnel have a responsibility to their fellowworkersandtothemselvestomaintainthissafeandhealthyworkingenvironment.The inappropriate use of alcohol and other drugs can lead to the impairment of anindividual’s health and work performance and can cause or contribute to accidents andinjuries.Tominimise the risk and harm associatedwith alcohol and other drugs in theworkplace,STAFFAUSTRALIAPTYLTDwill:• TakeappropriateactiontoensurethataworkerorcontractoronSTAFFAUSTRALIAPTY
LTDpropertyor STAFFAUSTRALIAPTY LTDworksite, or on STAFFAUSTRALIAPTY LTDbusinessduringthescheduledhoursofwork,isnotaffectedbyalcoholorotherdrugs,eitherillicitorprescribed.
• StaffAustraliamayarrangealcoholandotherdrugtestingtobeconductedbyaqualifiedpersonornominatedclinicorbystatutoryauthorities.
- At pre-placement or at random depending on the requirements of theposition/site-Wherealcoholordruguseissuspected.-Followingaseriousaccidentorinjury
• Implement alcohol and other drug screening to form part of routine and pre-employmentmedicalexaminationsforcertainjobroles

• EncourageworkerswhomayhaveadrugoralcoholproblemtoseekprofessionalcareandcounsellingandSTAFFAUSTRALIAPTYLTDwillco-operateinsuchefforts.
• As appropriate, provide information and training on the effects of alcohol and otherdrugs.
• Takedisciplinaryactionwherealcoholandordruguseresultsinpoorworkperformanceandposesriskstohealthandsafetyoranyoneonaworksite

InjuryManagementPolicy
Rehabilitation/ReturntoWorkDefinitionRehabilitation may be defined as “the restoration of a person suffering occupational illhealthorinjurytothefullestphysical,mental,social,vocationalandeconomicusefulnessofwhichheorsheiscapable”.Objectives1. Toprovidesupportandfacilitiestoinjuredworkerstoensureareturntoworkthatis
as appropriate as possible with consideration for the worker’s condition and thesafetyoftheworkersandothers.
2. To ensure that the duties selected to facilitate a graded return to optimal functionprovide a meaningful and satisfying participation in STAFF AUSTRALIA PTY LTDoperations.
3. To co-ordinate and liaise with the providers of medical, Para-medical andrehabilitationservicestomonitorprogresstowardresumptionofnormalduties.
Commitment1. Preventinginjuryandillnessbyprovidingasafeandhealthyworkenvironment.This
commitmentiscoveredunderourWorkplaceHealth&SafetyPolicy.2. Ensuringthattheworkplacerehabilitationprocessiscommencedassoonaspossible
after an injury/work related illness consistent with advice of the treating medicalpractitioner,andasperlegislativerequirements.
3. Ensuring that return to work as soon as possible by an injured worker is a normalpracticeandexpectation.
4. Consulting with workers to ensure that our rehabilitation programs operateeffectively.
5. Ensuring thatworkers participating in a rehabilitation program are fairly treated byother field staffworkersand internal StaffAustraliaworkers, andareprovidedwithequalresourcesandfacilities.
6. Providinganopportunityforan injuredworkertoparticipate inearlyreturntoworkprograms performing those duties within Staff Australia Pty Ltd field of operationswhicharesuitabletothatworker’scurrentmedicalcondition.ItshouldbenotedthatSTAFFAUSTRALIAPTYLTDcannotundertaketocreatespecialemploymentcategoriesoroperationstofacilitaterehabilitation.
7. Continually striving toprovidemoreeffective andefficient rehabilitation services toinjured workers by keeping abreast of current developments in industry, and bymonitoringandbenchmarkingperformance.
RehabiliteeResponsibilitiesinclude:• communicating regularly with their Staff Australia Workcover/Return to Work
Coordinatoraboutallissuesrelatingtoyourinjury• Takinganactivepartinyourrehabilitation.

CasualEmploymentContract
1. IunderstandthatbysigningthisagreementIamenteringintoaCasualEmploymentContractwithStaffAustraliaPtyLtd. I alsounderstand thatmyengagementwithStaffAustraliaPtyLtd is on a casual assignment by assignment basis. I acknowledge that, although Imay beplaced with various clients to Staff Australia Pty Ltd, these placements will not create anemployment relationshipbetweenmeand anyof those clients. I undertake that Iwill notmake any claim against any of Staff Australia’s clients which is inconsistent with theseacknowledgements.
2. Imayacceptorrejectanyassignmentoffered.3. IunderstandthatStaffAustraliaPtyLtdhasnocontroloverthelengthofmyassignment.4. IagreetonotifyStaffAustraliaasearlyaspracticablebeforethecommencementofmyshift
ifIamunabletoattendworkduringanyperiodorassignment.5. IagreetoconsultwithStaffAustraliapriortoacceptinganyofferofemployment(whether
thepositionistemporary,contractorpermanent)fromanyclientofStaffAustralia.6. Before I can represent Staff Australia on any assignment requiring a license issues by
statutory authority e.g. Car, truck, forklift etc, these qualifications must be sighted andphotocopiedbyStaffAustraliaasproofthatIamtheholderoftheappropriatelicense.Ifatanytimemy license iscancelled,suspended,restrictedor limited inanyway, ImustnotifyStaffAustraliaimmediately.Iunderstoodthatspotchecksoflicenseswillbeconductedonaregularbasis.Whenmylicenseisrenewed,Iacknowledgethatproofofrenewalwillagainberequired.
7. Iacknowledgethatthiscasualemploymentagreementdoesnotinvolveanyrepresentationor expectation of continuing work or regular and systematic engagement. This casualemploymentagreementalsodoesnot involveanyrepresentationorexpectation that Iwillbecome an ongoing employee or, if an ongoing position becomes available, that I will begivenpriorityorprecedenceoveranyotheremployeeorapplicant.
8. AsaCasualemployeeyouareentitledto:• ahigherhourlypayratethanequivalent full-timeorpart-timeemployeesperformingthesame
dutieswithintheunderpinninginstrumentthatStaffAustraliasuppliestoitscustomer.Thisisduetoapplicationofacasualloading
• 2daysunpaidcarer'sleaveand2daysunpaidcompassionateleaveperoccasion• unpaidcommunityserviceleave.
9. ACasualloadingispaidabovetheordinaryhourlyrate.Mostmodernawardsprovideforthepaymentofaloadingtocasualemployees.The‘standard’casualloadingis25%.Theloadinggenerally is not considered a ‘penalty rate’ because the benefits for which a casual iscompensated. This extra loading compensates you for annual leave, sick leave,personal/carers leave, public holidays and other entitlements applicable to a Permanentposition.• ThecompensationoftheCasualloading(25%)ismadeupofandbrokendownintothefollowing:
Annualleave–11.9%PublicHolidays–7.15%Sickleave/Personalcarersleave-5.95%
10. OurhourlyratesofpayincludetheCasualloading.Anypaymentswemaketoyounowcanbeoffsetagainstanyclaimsofunderpaymentmadebyyouatalaterdate.
11. If my name appears on a roster at any time, I acknowledge that it will be purely foradministrative purposes and in no way represents regular, systematic or ongoingengagementoremployment.
12. Iunderstandthatwhenaclientassignmentends,ImaybeofferedfurtherassignmentsbutneithertheclientnorStaffAustraliaPtyLtdisobligatedtodoso.
13. IacknowledgethattheclientshallhavetheauthoritytoinstructmetoimmediatelyceaseanassignmentwhereIamresponsibleforanydishonesty,seriousmisconductorseriousbreachofdutyorrefusetocomplywithanyreasonableinstructionsgivenbytheclient.

14. IagreetocompleteaTaxdeclarationFormsuppliedbytheAustraliaTaxationOfficeforStaffAustraliaeachfinancialyearifIhavenotalreadydonethis.
15. I agree to indemnify Staff Australia Pty Ltd for any loss, damage or liability whatsoeverincurred through any act, neglect or default of mine, my agents (or employees). To theextentthatanemployeerelationshiporthevicariousliabilityofStaffAustraliaPtyLtdexistsormaybedeemedtoexistorbeimpliedbylaw,IfullyindemnifyStaffAustraliaagainstanyliabilityorclaim,whichmaytherebyarise.
16. If any provision of this agreement is declared by any competent authority to be void,voidable, illegal or unenforceable, the parties may amend that provision to achieve theintention of the parties, or at the discretion of both parties it may be severed from thisagreement and the remaining provisions shall continue with full force and effect. Theseterms shall be governed by Australia Federal Law, or where appropriate, the laws of theStateinwhichIamtoperformtheassignment.
17. OccupationalHealthandSafety/WorkcoverRequirements
• Iwill only accept assignmentswhere I have thenecessary skills toperform the assignment andwhere I have no pre-existing illness or injury, which would prevent me from completing theassignmentsafely.Also,it isacknowledgedthatStaffAustraliaPtyLtdhasreliedontheaccuracyand correctness of all information givenbyme to Staff Australia Pty Ltdwhen referringme forassignments.
• I accept that I am under the care, control and supervision of Staff Australia’s client during theperiodofanyassignmentand Iacknowledge the rightof thatclient todirectmyworkactivitiesnotwithstandingthatfactthatStaffAustraliaalwaysremainsmyemployer.
• I agree to occupational Health and Safety policies of Staff Australia Pty Ltd as outlined in StaffAustralia’sOH&SinductionandthesafetyrulesandpoliciesofStaffAustralia’sclients.
• IagreethatImustwearsteelcappedfootweartoallassignments.WhenIamrequiredtoprovideequipment,IwillensurethatanysuchequipmentissuitablefortheworkandIshallmaintainitingoodworkingcondition.
• I understand that throughoutmy employmentwith Staff Australia Pty Ltd Imay require to useknivessuchasStanleyknivesforopeningboxesorcartonsandtoassistwithaccessingproducts.Iunderstandthat instructiongiventomeinrelationtothesafeuseofknivesandwill followittopreventinjurytomyselfandothers.
• StaffAustraliaPtyLtdisresponsibleforstatutoryWorkCoverInsuranceand, intheeventofanyinjuryoccurringduringanassignment, Iwillnotifymysite supervisorandStaffAustraliaPtyLtdwithdetailsofthatinjuryimmediately.
18. PayrollProcesses
• AtimesheetsignedbybothmyselfandanauthorizedrepresentativeofthecompanytowhichI
amassignedmustbeproducedbeforeremunerationcanbemade.Somecompaniesmayprovideclockcardswhere Iwillberequiredtoclockonandoffasdirectedbytheclient.Notethat it isagainstthelawtoclockonoroffanyclockcardotherthanyourown.ThisisanactasFRAUD.Instantdismissalandlegalactionwillresultiffoundguilty.
• Iunderstandthatfuturepaymentsmaybeadjustedifactualworkinghoursorotherdetailsdifferfrom information provided on authorised time sheets received by Staff Australia and I furtheragreetoallowStaffAustraliatodeductanyoverpaymentsfromfuturepayments.
• Payment to me by Staff Australia will be by electronic fund transfer to the bank accountnominatedbymeonaweeklybasis.
• I will be paid in accordancewith the award, or enterprise agreement or AustralianWorkplaceAgreementthatappliestothesiteIamworkingonandunderstandthatthiswillvaryfromjobtojob. Pay rates applying will be advised with each job. Rates, Overtime, special allowances andconditionswillbepaidaccordingly.
• Government legislation provides workers with Superannuation paid for by Staff Australia. Iunderstandthatmysuperannuationcontributionswillbepaid inaccordancewiththe legislationinto the complying fundofmy choice. If Imakeno choice, then itwill bepaid intoAMPSuperLeader.
• OnanyassignmentStaffAustraliawillpaymeforaminimumoffourhoursunlessIdonotstartworkthatdayorleavethesiteofmyownaccordpriortothecessationofthework.

19. I acknowledge as an employee of Staff Australia andwhilst on assignment with any StaffAustraliaclientthattheconfidentialityandsensitivityofinformationseenbymeneedstobeguaranteedandwillnotbedisclosed,unlessdisclosureisdulyauthorised,duringoraftertheagreedassignment.
20. IagreetonotifyStaffAustraliaasearlyaspracticablebeforethecommencementofmyshiftifIamunabletoattendworkduringanyperiodorassignment.
21. IagreetoconsultwithStaffAustraliapriortoacceptinganyofferofemployment(whetherthepositionistemporary,contractorpermanent)fromanyclientofStaffAustralia.
22. IagreetocompleteaTaxdeclarationFormsuppliedbytheAustraliaTaxationOfficeforStaffAustraliaeachfinancialyearifIhavenotalreadydonethis.

General
Ideclarethatallemployment-relatedinformationprovidedbymetoStaffAustralia istrueandcorrect.I have read, understand& accept the Terms and Conditions of ‘The Staff Australia CasualEmployment Contract’ and the Staff Australia Policies, without the requirement of anInterpretingService.Furthertoabove IdeclarethatStaffAustraliahasoffered interpretingservicespriortothereadingandsign-offofthisagreement.I accept Staff Australia’s and/or its client’s policies in relation tomedical requirements,safetyregulations,andanyotherconditionsapplicable.I understand that this agreementwill only become valid and effective once successfullyplaced at my first assignment with Staff Australia. Prior to placement this agreementremainsnullandvoid.I understand that further to clause above this agreement is only valid if signed by anauthorisedStaffAustraliarepresentative.Iunderstandthat IwillbeprovidedwithacopyofthisagreementoncesignedbyaStaffAustraliarepresentative.ThecopyprovidedcanbeprovidedbyStaffAustraliaeitherintheformofhardcopyorsoftcopy.
