Doug will open a checking account for the remainder and learn to balance his
checkbook weekly by graduation.
2. Community Integration and Participation
2.1 Independent Living Goal:
While participating in the Young Adult Program through the school district, Doug will
live at home with his family.
Following completion of his secondary education, Doug will continue to live at home
until he saves enough funds to support living independently.
Doug will move in to an independent living situation in an apartment with a roommate
within three years of graduating high school.
2.2 Transportation/Mobility Goal:
Doug will learn to use public transportation, such as the city bus, to transport himself
independently between desired locations without supports.
Doug will practice using public transportation by taking the bus to and from each of his
vocational placements while in high school.
Doug will learn to look up the bus schedule online and make it to the proper bus on time
by the end of his Young Adult Program.
By his senior year of high school, Doug will take the bus to and from school as well asto his vocational placements daily.
2.3 Social Relationship Goal:
Doug will demonstrate awareness of social moderation, where he will be social when
appropriate with his friends, but he will refrain from talking and socializing whileworking by graduation.
2.4 Recreation/Leisure Goal:
In 11th and 12th grades, Doug will participate in a team sport through his high school.
While in the Young Adult Program, Doug will participate in team sports or clubs
through his local park district.
Doug will learn to use the internet to maintain relationships with his friends and to find
clubs and organizations within the community to participate in by the end of high
school.
Following completion of his secondary education, Doug will continue to participate in

3. Personal Competence
3.1 Health/Safety Goals:
While in high school, Doug will remain active by participating in a team sports andexercising in gym class.
Following graduation, Doug will exercise twice a week at the local gym with a personal
trainer to go down to and maintain a healthy weight for his age.
Following graduation, Doug will take an Introduction to Cooking class at the local
community college to learn how to safely prepare food independently.
3.2 Self-determination Goal:
In 11th and 12th grade, Doug will lead his own IEP meetings in order to share his wants,
needs, and interests with his IEP team.
Following his vocational placements, Doug will search for a job and discuss his needs
with his employers.
III. STUDENTS COURSE OF STUDY
11th Grade:
- Vocational placement at a retail location for four periods a day- General Education Physical Education Class
- Community Based Instruction (1 periods a day)
- Computer safety/Internet usage class- 1:1 instruction to continue learning skills such as dollar-up, learning work-
related functional words, and other independent living skills, self-determination,
etc.
12th Grade:
- Vocational placement at a retail location for four periods a day
- Basic Home Economics Class (to learn to balance a checkbook/home budgeting)
- Community Based Instruction (1 periods a day)- General Education Physical Education Class
- 1:1 instruction to continue learning skills such as dollar-up, learning work-
related functional words, and other independent living skills, self-determination,etc.
Young Adult Program:
- Vocational site part-time with employment support services- Job-specific training: interview skills, functional words, etc.
- Independent living skills instruction
