    Incident: An event or chain of events which cause, or could have caused

    injury, illness and/or damage to assets, the environment or third parties.

    Accident: An Uncontrolled occurrence in the operation of a site which

    leads to severe or catastrophic consequences to people, assets, the

    environment and/or company reputation.

    Near miss: An undesired event which had the potential for causing an

    accident or under slightly different circumstances may have caused injury to

    person, damage to plant, equipment, material or environment.

    Unsafe Act: An act that has a potential to cause an accident or incident.

    Unsafe Condition: A condition that has potential to cause an accident or

    incident.

    A loose cable is laid on the stairs.

    A person got tripped but fortunately didnt loosehis balance.

    The cable gets a cut by something as it is in the

    passage and it causes a spark but nothing

    happened.

    A person trips over the cable, he fell from stairsand got some scratches.

    The damaged cable produced an spark which

    caused a fire and the cable burnt along with some

    carpet or other stuff.

    Tube light fitting detached from roof and hanging.

    Unsafe Act

    A team of scaffolding group working on the scaffolding observed

    throwing the tools from the scaffolding to the ground.

    Near Miss

    Tube light fitting detached from the roof and fell on ground, luckily

    nobody injured.

    The HSE Concerns (Unsafe Condition/Act) reporting is a

    preventive approach and therefore reflects the overallpositive safety culture of an organization.

    Near miss reporting are free learning opportunities and their

    reporting should be encouraged as it helps reducing the
