1. Onlineapplicationform:

WordslikeLate,Alias,Shri,Smt,Kumari,Dr.,Major,Captain,M/setcshouldnotbementionedinthenamefieldsofApplicantName,Nameoncard,Fathername&RepresentativeAssesseeName.OnlyonesinglecharacterinitialisallowedinmiddlenamefieldinPANapplicationforIndividualapplicants.Applicantisrequiredtoverifytheapplicationdetailsbeforesubmittingtheonline

application.

2. Photo:

'Individual'applicantsincludingminorapplicantsshouldaffixarecentcolourphotograph(size3.5cmx2.5cm)inthespaceprovidedintheacknowledgmentreceipt.However,photoofminorwillnotbeprintedonPANCard.Kindlypastethephotographinthespaceprovidedonacknowledgementreceipt.Pleasedonotstapleorclipthephotographtotheacknowledgementreceipt.Pleaseensurethatthephotographhastheclearvisibilityofapplicant'sface.

3. Thumbimpression/signature:

Signature/LeftThumbImpressionshouldbeputwithinthe

boxprovidedintheacknowledgment.Signatureshouldnotbeputacrossoronthephotograph.Individualapplicants

Acknowledgementreceiptshallbesignedbytheapplicantinthespaceprovidedforsignature.Incaseofaminor/deceased/idiot/lunatic/mentallyretardedacknowledgmentreceiptshallbesignedbytheRepresentativeAssessee.

NonIndividualapplicants

IncaseofHinduUndividedFamily(HUF),acknowledgmentreceiptshallbesignedbytheKartaoftheHUF.IncaseofCompany/Firm/AssociationofPerson(s)/BodyofIndividuals/AssociationofPerson(s)Trust/ArtificialJudicialPersonorLocalAuthority,theauthorisedsignatory

shallsigntheacknowledgmentreceipt.

ThumbImpressionshouldbeattestedbyaMagistrateoraNotaryPublicoraGazettedOfficer,underofficialsealandstamp.

ProofofIdentity,AddressandDateofbirthshouldbeinthenameoftheapplicantasmentionedinitemno.
