Putting the Physical Activity Plan Together

• Begin putting together your personal physical activity plan for approval and sign-off

Safety and Risk Management

• The risk-management process is an essential part of a personal physical activity plan– Not about eliminating risk or injury– Identify the risk inherent in any physical activity – Identify strategies to deal with the identified risk

Planning for Risk Management

• Complete 2 of the Safety and Risk-Management Planner sheets for physical activities that you have chosen for your physical activity plan that have the highest risk factor rating.

Finalizing the Physical Activity Plan

• The physical activity plan should include– Physical activities that contribute to

cardiorespiratory endurance plus one or more of the other health-related fitness components

– Risk-management plan – Physical Activity Safety Checklists– Parent Declaration and Consent Form– Student Declaration Form

Implementing the Physical Activity Plan

• Begin implementing your physical activity plan and enjoying the physical activities you have chosen

Recording Physical Activity Participation

• Details of the physical activity participation• Assessment of the amount of time spent• Daily reflections of satisfaction

Completing the Physical Activity Plan

• Parents will need to sign a form – indicating that the information provided is an

accurate record of the students’ participation in the OUT-of-class physical activity practicum
