Parkinson’s Disease Foundation (PDF) for fiscal years 2012 and 2013. The essence of PDF’s work over this period may be encapsulated into the fol-
lowing theme: People Advancing Science/ Science Informing People. In thesephrases rests the heart of PDF’s strategy: linking our dreams for tomorrow with thenecessary tasks of today. Let us explain.
People Advancing Science. In this dimension of our work, PDF harnesses theenergies of committed doctors, gifted scientists and generous donors to push thescience forward in the quest for new Parkinson’s treatments and — ultimately — acure for the disease. This commitment has been at the core of PDF’s researchstrategy since the organization was created more than a half-century ago.
This research — investigator-initiated and rigorously reviewed by peers andpeople with Parkinson’s — takes many forms. One could be the individual scien-tist in her laboratory, studying potential disease-modifying treatments for Parkin-son’s. A second could be the work of a diverse panel of experts exploring thedebilitating impulse control disorders that affect so many people with Parkinson’s.A third could be the long-term collaborations of a university-based research teamexploring the role of genetics in Parkinson’s disease.
In addition to the communities of doctors, scientists and donors who have tra-ditionally led this work, PDF includes a fourth community: lay advocates in the sci-entific process, trained and deployed by PDF though in-person trainings and anonline course. Through this program, known as Parkinson’s Advocates in Re-search (PAIR), people touched by Parkinson’s can join our network of more than230 Research Advocates from 42 states who are working to bring about bettertreatments at a faster pace. These individuals are taking on a variety of chal-lenges, including recruiting individuals for drug trials, providing advice to industryand federal regulatory agencies, and educating support groups.

Science Informing People. As much a philosophical precept as it is a pro-gram strategy, placing information and education on the firm basis of science is acore principle at PDF. Whatever the medium — our publications and newsletters,our popular website, our online educational seminars, or our online programs ofprofessional education for nurses and physical therapists — PDF serves as an en-trepot of relevant, timely and authoritative knowledge and information on Parkin-son’s. Our goal is simple: to identify the questions that are uppermost onpeople’s minds and then to give them answers they can rely on, based on thebest science available.
Many of you who read this report will be the people who already supportPDF’s work financially. It is your generosity that powers our work — on the onehand, advancing, through research of the highest quality, our understanding ofParkinson’s disease, and on the other, through science-based education, improv-ing the lives of people who are touched by the disease. David Sulzer, Ph.D.: Finding Potential Targets for Preventing or Treating Parkinson’s
Daniel Segal, Ph.D.: Learning from Other Diseases to Find New Treatments
Although the vast majority of cases of Parkin-son’s disease (PD) have no known genetic cause,among those that do run in families, the most com-mon mutations occur in the LRRK2 gene (leucine-rich repeat kinase-2). Now, researchers led by AnaMaria Cuervo, M.D., Ph.D., at Albert Einstein Col-lege of Medicine and David Sulzer, Ph.D., at PDF’sResearch Center at Columbia University MedicalCenter, funded in part by PDF, have shown for thefirst time, why the faulty LRRK2 protein is harmful,and how it can block the normal cell recyclingprocess. This research was published in the March3, 2013, online edition of Nature Neuroscience andbuilds upon earlier work also funded by PDF.
Normal cells, including those in the brain, use arecycling and waste disposal system to stay healthy.The process — known as chaperone-mediated au-tophagy — begins when molecules called chaper-ones patrol for damaged proteins and then
transport them to a recycling center (the lysosome).After arriving there, the damaged proteins travel tothe interior region of the lysosome, where they arebroken down into components that can be reused.
Drs. Cuervo and Sulzer showed that chaper-one-mediated autophagy was capable of recyclinghealthy LRRK2 proteins, but it was not as successfulin breaking down those LRRK2 proteins that hadthe mutation found in people with Parkinson’s dis-ease. In addition, these faulty LRRK2 proteins thenprevented other proteins, such as alpha-synuclein(a protein that causes PD), from getting brokendown and recycled.
The studies of Drs. Cuervo and Sulzer over theyears have spotlighted the need for developingtherapies that can help restart or speed up cellularrecycling — therapies that could potentially benefitpeople whose PD has no known genetic cause aswell as those who have LRRK2 mutations.
In Parkinson’s disease, the protein that clumpstogether and causes the disease is called alpha-synuclein. If scientists could stop alpha-synucleinfrom clumping in the brain, this could potentiallyprovide a new treatment for the disease.
With funding from PDF, Daniel Segal, Ph.D., of Tel Aviv University, has developed a new mole-cule, based on quinone, a compound that has beenshown to inhibit certain cellular pathways, that maybe able to do this. His molecule is small enough tofit into the brain, and is similar to another moleculeshown to stop clumping in the protein that causesAlzheimer’s disease.
Dr. Segal found that in a petri dish, this mole-cule was able to effectively prevent alpha-synucleinfrom clumping together, and that higher doses weremore effective than lower doses. Now he is testingthe molecule in animal models. His early tests in
fruit flies found that treatment with this molecule re-sulted in improved motility (measured by climbingbehavior) in PD fly models containing excess alpha-synuclein in their brains. He now will extend his test-ing to PD mice models.
If he finds this molecule to be successful, thiscould mean a promising new therapy for Parkinson’sdisease. Ofcourse, clinicaltesting would haveto be done in hu-mans to ensurethat the moleculeis safe and worksthe same way inhumans as it doesin flies and mice.

Genes, in ways still unknown, contribute sig-nificantly to the risk of Parkinson’s disease. Un-derstanding the genetics of those who live withParkinson’s disease may hold the key to finding acure. To further our understanding of genetics,PDF established in 2012 the Lucien Côté Early In-vestigator Award in Clinical Genetics, which sup-ports postdoctoral researchers and junior facultyin the New York City metropolitan area who arestudying the genetics of Parkinson’s.
One of the first recipients of this award wasSheng-Han Kuo, M.D., of Columbia UniversityMedical Center, who is using induced pluripotentstem cells (iPS cells) to study how changes, ormutations, to certain genes may cause PD. iPScells are skin cells from people with Parkinson’sdisease that have been chemically induced todevelop into dopamine neurons, which are thecells that are lost in the brains of people with PD.
Dr. Kuo is studying dopamine neurons de-rived from skin cells that have a specific changeto a gene called GBA (the full name is glucocere-brosidase). He wants to determine how changesto this gene affect normal function in the brain,especially how they affect the clumping of alpha-synuclein (a protein that causes PD). Past studiesin rats have shown that changes to the GBA genedo cause clumping of alpha-synuclein in thebrain. Now the goal is to see if this clumpingalso occurs in human brain cells containing theGBA mutation, and the mechanisms by whichthis happens. This knowledge may potentiallylead to the development of new PD treatments.

Since 2000, PDF has provided over $69 million to more than 400 scientists as part of a comprehensive and multifaceted approach that supports both research and training.
The PDF Conference Award program was launched in 2012 to supportgatherings of experts in the field to address emerging clinical or basic sci-ence questions about Parkinson’s. Two of the initial recipients were Anto-nio Strafella, M.D., Ph.D., of Toronto Western Hospital and Mark Stacy,M.D., of Duke University. Together, they used the PDF award in conjunc-tion with other funding to gather clinical and preclinical experts from vari-ous research backgrounds to develop a strategic plan for studying thedebilitating impulse control disorders (ICDs) that affect so many peoplewho have Parkinson’s.
ICDs can be a side effect of dopamine agonist therapy and includebinge eating, compulsive gambling or shopping and increased sexual be-havior. The conference, “Impulse Control Disorders in Parkinson’s Dis-ease,” was held in October 2012 and featured experts from fields includingneurology, psychiatry, neurobiology, pharmacology, addiction, neurologicimaging and epidemiology.
This multidisciplinary meeting addressed the spectrum of symptomsassociated with ICDs, and generated new ideas for clinical, experimentaland translational approaches to better understand ICDs. It also facilitatedinteraction and collaboration among experts from diverse fields, and de-veloped a network of interested scientists and clinicians to further studythe disorder. The proceedings of the conference will be published inMovement Disorders.

Could we be returning to the tradition ofseeing our doctors at home? This is the conceptof telehealth. But how does it compare to con-ventional in-person medical care?
Investigators led by E. Ray Dorsey, M.D.,M.B.A., at Johns Hopkins Medicine, and includingPDF Summer Fellow Benjamin George, recentlyfound that people with Parkinson’s may benefitfrom a consultation with a specialist via onlinevideo conferencing from home, providing an al-ternative to traveling to an in-person appoint-ment. These findings appeared in a studypublished on March 11, 2013, in JAMA Neurol-ogy. This study is one of the first to investigatewhether telehealth appointments are feasible andbeneficial to people with Parkinson’s disease.
Researchers measured the participants’ qual-ity of life and PD symptoms, and found the sameresults for individuals, regardless of whether theysaw a doctor in-person or via video conference.In addition, compared with in-person visits, each"virtual" visit saved participants, on average, 100miles of travel and three hours of time. Althoughthe number of participants in this study wassmall, the results provide encouragement thatvideo conferencing with a specialist can providesimilar benefits to people with PD as in-personvisits, while reducing the time and expense ofseeing the doctor.
However, before virtual visits become a reality,researchers must conduct studies with larger num-bers of participants, and with participants of morevaried socioeconomic backgrounds. In addition,issues surrounding technology and insurance re-imbursement and rules in the United States thatrequire doctors and the person being treated tobe in the same state, will need to be resolved.

PDF believes that better treatments can be found at a faster pacewhen people with Parkinson’s are primary partners in research alongsidescientists, industry and government. As the number of non-English speaking in-dividuals increases in the United States, theneed for resources for these communities con-tinues to grow. In fact, by 2050, the Hispanicshare of the elderly population is expected toalmost triple to 17 percent from six percent in2005. Since age remains the biggest risk factorfor Parkinson’s, the prevalence of the disease inthis community is expected to increase.
PDF is committed to ensuring that all peo-ple — regardless of who they are, where theylive, what language they speak or how muchmoney they have — receive needed Parkinson’sinformation and resources. In 2012, PDF ex-panded our HelpLine to include interpreterswho are available to speak with callers in Span-ish and more than 200 other languages. OurHelpLine is staffed by a team of informationspecialists who can answer questions aboutParkinson’s treatment and care, identify legaland financial resources and provide referrals todoctors and support groups where available.
PDF also expanded our line of educationalmaterials available in Spanish to include factsheets on topics such as deep brain stimulation(DBS) as a treatment of Parkinson’s, care partnerconcerns, the role of hospice in Parkinson’scare, managing Parkinson’s medications and un-derstanding pain in Parkinson’s. In 2012 and2013, more than 5,000 individuals requested in-formation in Spanish.

Since 2007, PDF's HelpLine has responded to inquiries from almost 50,000 individualsaround the world as part of its commitment to ensuring that everyone has access to Parkinson's information and resources.

While no treatment can prevent or reverse Parkinson's, research sug-gests that physical therapy can help improve strength, fitness level, walk-ing speed and balance, leading to reduced disability and greaterindependence. Unfortunately, there is a lack of trained physical thera-pists to care for the growing Parkinson’s community.
To address this need, PDF launched in April 2012 a free online course,“Parkinson’s Disease: A Practical Approach to Evaluation and Treatmentfor the Physical Therapist.” The course was designed by expert physicaltherapists in the field of Parkinson’s to help other physical therapists pro-vide better care to people with the disease. PDF led the development ofthis course in collaboration with the American Parkinson Disease Associa-tion (APDA), the American Physical Therapy Association (APTA) and theVisiting Nurse Service of New York (VNSNY). The course includes ten ed-ucational sessions and two interactive panel discussions. Since its launch,more than 6,000 physical therapists have participated in this program.
This program was made possible with the support of Kenneth M. andSusan Granat Weil in memory of John and Joan Haine; the Edmond J.Safra Philanthropic Foundation and the American Physical Therapy Asso-ciation. It also received kind support from Rusk Rehabilitation at NYULangone Medical Center. Additional promotion was provided throughour media partners, which include the Houston Area Parkinson Society,National Parkinson Foundation, Northwest Parkinson's Foundation andParkinson Association of the Carolinas.

Page Morton Black: 1916-2013
In 2013, the Parkinson's Disease Foundation (PDF) lost a great, much-loved, deeply admired ambassador for all those who live with Parkin-son’s disease — our Chairman Emeritus Page Morton Black. It was
William Black, Mrs. Black’s late husband and founder of the Chock fullo’Nuts coffee and restaurant chain, who created PDF in 1957. Mrs.Black served as Secretary of PDF until assuming the role of Chairmanafter her husband’s death in 1983. She resigned from the Board in2012, serving as Chairman Emeritus until her death. In 2005, PDF
presented Mrs. Black with its Lifetime Achievement Award.
