VITALITY ACTIVE LIFE INSURANCEThis proposal o�ers life insurance benefits for the customers. Under the arrangement, insurance cover will be provided to you, i.e. the customers who will subscribe to this service. The benefit is a lump sum amount which will be paid in case of death due to accident or sickness as per the terms and conditions. It is a low cost and simple-to-understand insurance cover without the requirement of any medical exam/medical questions, etc. Simplicity of the plan allows its comprehension for you as a customer.

VITALITY ACTIVE REWARDS The Vitality programme takes customers through these three stages: • Know your Health • Improve your Health • Enjoy the Rewards

You as a customer will engage with Vitality and earn Vitality points. Vitality points, in turn, determine a customer’s Vitality status which can be Bronze, Silver, Gold or Platinum.

Each customer receives a weekly challenge for physical activity, upon achievement of which you are able to earn and redeem a weekly ‘active reward’. These rewards take the shape of a 50% discount on a movie ticket booked through Easy Tickets, or a PKR 500 discount on a healthy meal ordered from Foodpanda or PKR 500 as a mobile top up to any network in Pakistan. New rewards are being added on an ongoing basis and will be advised in due course of time.

The physical activity targets can be tracked through a wearable device or your smartphone. A large number of devices are compatible with the Vitality programme, making it a one-of-its-kind mobile-led approach. In addition to o�ering death and disability coverage, Vitality Program o�ers the customers an opportunity to improve their health by motivating them to stay fit and active. The Vitality rewards program helps in bringing positive change to the customers’ lifestyles.

ELIGIBILITY

You must fall within the age bracket of 18 – 59 years at the time of

enrollment and have renewed your policy up to your 65th birthday.

The maximum age limit for the plan is your 65th birthday.

EXCLUSIONS

Claims will not be admissible in the following cases:

• Occurrence of death due to HIV

• Active participation in war/war-like operations
