PARENTAL DECLARATION

I, Mr. and my wife Mrs. here declare that the child with bio-data stated above is our child and we wish to name and dedicate him/her to the Lord in accordance with our Christian faith and the Word of the Lord. We hereby attach a copy of the child’s birth certificate and our marriage certificate as proof of parentage of our child.

The following process should be followed by parent of new child to ensure smooth naming and dedication.

1. Parents must be registered Tithe paying members of RCCG - International Christian Centre to benefit from child naming and dedication.

2. A form is given to the parents by the Hebrew Midwives Ministry 2 months before the expected date of delivery3. A 3-way naming & dedication meeting with parents (both mother and father), Lighthouse Care Group shepherd and a member

of the pastoral team of the church.a. During the meeting, discussions will be based on the significance of dedicating a child and the responsibility of the parents

who are choosing to present their child to be dedicated to Christ.b. Nature of the Christian naming and child dedication ceremony and the thanksgiving in church at a subsequent date (i.e. any

of the thanksgiving service).4. Child’s naming and dedication will be done at the same time at the parent’s house or designated place. Child naming and

dedication takes approximately 1 hour5. The child is brought to church for presentation and thanksgiving on the next thanksgiving service at the family’s convenience.
