The school shall retain a signed copy of the fully executed Enrollment Agreement as a part of the student’s permanent record. - Enrollment Agreements shall provide consumer information including, but not limited to: an explanation of all criteria requirements for retention, progress towards credential, and completion by the student; the institution’s tuition, cancellation and refund policies; and a statement of purpose and amount of any fees assessed. o More specifically, the Enrollment Agreement shall include, at a minimum: an accurate description of the program, or course offering, and all program requirements such as prerequisite courses, program objectives, and length of program; a clear description of all costs, including a schedule of tuition, fees, and all other charges and expenses necessary for completion of the course of study such as training and incidentals; t he institution’s financial aid policies and practices; policy on refunds; reference to the disclosure information required by the Act and such other material facts concerning the institution and the program or course of instruction as are likely to affect the decision of the student to enroll; and the Board’s Internet website, and the address and phone number of the Board for students to report complaints. o Although the school may use an Enrollment Agreement that sets forth the total cost of the program, no school may have a tuition policy or Enrollment Agreement that: a. requires that a student register for more than a single semester, quarter, term, or other such period of enrollment as a condition of the enrollment, or b. charges a student for multiple periods of enrollment prior to completion of the single semester, quarter, term or other period of enrollment. - Institutional Enrollment Agreements are prohibited from including any clauses that limit or waive students’ rights to initiate or participate in class action lawsuits against said institution. o No institution shall enter into any Enrollment Agreement in which the student waives the right to assert against the school or any assignee any claim or defense he or she may have against the school arising under the agreement. Course Description This is a six week course in which the student will study the principles and procedures to be used by the nursing assistant in long term care, home health, and hospital settings. The focus is on basic bedside nursing care, teaching 21 basic nursing skills, residents’ rights, ethics, abuse and neglect, infection control, Alzheimer’s training and CPR. The successful student will have a basic knowledge of human

anatomy as well as how to apply this in the clinical setting. Upon successful completion of the program the student will receive a certificate of completion and will have met the requirements to register to take the IDPH (Illinois Department of Public Health) state competency exam. The state approved Certified Nursing Assistant Training Program: The program is intended to meet the

Illinois state requirements for Nursing Assistant certification and to prepare the student to take the Illinois Nurse Aid Competency Exam. This program also meets the requirements of the Nursing programs and other related healthcare fields. Skills include all elements of personal care, body mechanics, safety measures, resident rights, infection

control, communication, and observation. Also includes the care of a person with Alzheimer’s disease or related dementias. The course offers learning experiences in the classroom/lab setting and the long term care clinical setting. The IDPH program model will be utilized as a guide for this course. The student will also receive a signed certificate indicating successful completion of the program.

Philosophy: The expected goal of the Carepaks Health Services Basic Nursing Assistant Program is that the student will be a valued part of the health care team, meeting the needs of the health care consumer with a qualified and informed manner. The learning experience is a step toward continual growth and development of the total person and is

intended to stimulate desire and need for lifelong learning. The teaching process is a shared responsibility of the instructor and the student. The role of the instructor is to provide meaningful learning experiences in a suitable learning environment. The role of the student is to assume responsibility for his/her own learning and to recognize and accept that the level

of achievement is contingent upon individual ability and motivation.

Philosophy of Patient/Resident Care: The patients/residents will have their medical and nursing needs met to the best of the health team’s ability with regards to medical asepsis, patient/residents rights, and safety. The main emphasis is placed

on the quality of life regardless of race, sex, color, or creed. The dignity and worth of each patient/resident is to be preserved in regards to promotion of independence to the best of the patients/residents ability, comfort, and contentment with themselves, their surroundings, and the people with whom they spend their days.

PREREQUISITE & OTHER REQUIREMENTS FOR ADMISSION TO PROGRAM :

Prerequisite: Must be 16 years of age or older, if less than 18 must have parent signature - Provide copy of state ID and Social Security Number - Complete physical and 2-step or T-spot TB skin test prior to first day of clinical (physical must

state:”No Restrictions”)

- English comprehension test (for those students that have English as a second language it is highly recommended that the student has a college level of English comprehension to optimize successful completion of the program)

- Acceptable fingerprint background check. Students must see instructor before or during

orientation if student has ever been arreste d for anything! The instructor will have a list of

unacceptable convictions that will prevent students from being able to work in healthcare or related fields. The ‘Health Care Worker Background Check Act’ requires an acceptable criminal background check before being added to nurse Aide Registry in Illinois. The host clinical site also requires a clean background check before allowing students to perform clinical in their facility.

- A student missing a scheduled appointment for the Fingerprint Criminal Background check is

responsible for immediately informing the instructor so that a rescheduled appointment may be

made as soon as possible. Time must be allowed for the Background check to come back without

any infractions before start of clinical. Students who do not complete this requirement are not

allowed to attend clinical.

C.P.R. Certification: in compliance with IDPH guidelines and Carepaks requirements for CPR certification, each student will attend a scheduled CPR class regardless of current CPR certification.

Each student must pass a written exam and performance evaluation. Failure to attend a scheduled CPR class will constitute an absence from the theory portion of the class. The student MUST make up the CPR class by the end of the course. If a student is unable to attend the scheduled CPR class during the course it will be the students responsibility to successfully complete a privately offered CPR class and

submit proof to the instructor by the last day of class. This option must be approved by the CNA instructor.

Supplies Used or Required for Class/Clinical Textbook with corresponding workbook will be issued to each student at orientation. The fee for all

textbook materials is included in the program fees.. (Textbook materials will be ordered and updated according to need and availability) The student must dress with regards to medical asepsis, safety, and use all guidelines pertaining to safety, infection control, and resident/patient rights in the clinical setting.

Students are required to purchase a scrub uniform consisting of brown top, white scrub pants and white shoes , gait belt, watch with second hand, and note pad for clinical.

PROGRAM / COURSE OBJECTIVES:

Learning Objectives Upon completion of the course, the student will be able to: - Identify the role of the Certified Nursing Assistant as a member of the health care team - Recognize the legal and ethical responsibilities of the Certified Nursing Assistant. - Apply principles of infection control in the healthcare setting.

- Demonstrate the ability to perform nursing assistant skills in a safe manner according to IDPH established criteria. - Demonstrate the ability to meet patient/resident’s basic needs with empathy and caring.

Course Requirements - Theory: The theory portion of Carepaks program is 94 hours total. The student must pass the theory portion with a score of 80% or above to satisfy the requirements of IDPH and the program. - Lab: Successful completion of certain lab skills are necessary prior to performing on the clinical floor.

- Clinical: A minimum of 40 hours are required by IDPH to satisfy this portion of the program. The clinical portion of Carepaks program is 42 hours total. The clinical experience utilizes the classroom

concepts and the lab skills. These skills will be performed under the supervision of the clinical instructor. The clinical portion is Pass/Fail and is graded separately from the theory portion. Any missed time from on-site clinical must be made up hour for hour with the clinical instructor at a designated clinical site at a later date. This may result in additional fees.

A 2-step or T-spot TB test and Physical exam are required to be done and on file prior to the start of clinical. The student must complete the minimum required hours of this program to satisfy the IDPH requirement to take the State Exam. If, due to unforeseen circumstances, the student is not able to complete the

required hours of this program in the six week time allotment, the student has, according to IDPH Administrative rules, 120 days from the first day of class to make up the required time and sit for the State Exam. This 120 days will also include a 30 day lag time from the registration date, to the date of actual test. Postmark dates and testing dates, along with more information may be found by going to the

Theory Class Requirements Each student enrolled in the CNA program is accountable for their individual behavior and is expected

to respect each other and the instructor by acting in a mature manner that reflects honesty, integrity,

courtesy, and responsibility. Any behavior on the student’s part that makes learning difficult for

themselves or other classmates will not be allowed. Such behaviors include but are not limited to the

following:

- Talking while the instructor is presenting educational material

- Sleeping in class

- Using cell phones during class

- Engaging in inappropriate or argumentative behavior

- Excessive tardiness or absenteeism

- Falsification of any document required by IDPH

- Attending any class while under the influence of any substance.

- Acts of lying or theft

- A student exhibiting behavior that hinders the learning process may be asked to leave the

classroom and the situation will be brought to the attention of the CNA program coordinator

- Pets are not allowed on CarePaks premises or clinical premises at any time for any reason. Do

not bring pets and leave them in the car.

- Students are not allowed to bring children with them to class

- No solicitation allowed during class or clinical time.

- Instructors will take reasonable measures to facilitate student success in the Certified Nursing Assistant course. Each student will be evaluated on classroom and lab performance weekly, and after each clinical session. The student is to abide by the Professional Conduct Policy, all

objectives, all requirements, and all stated policies specific to the clinical facility. When delivering cares the student must act with regard to safety, medical asepsis, and resident/patient rights.

- Whenever a student’s performance has been deemed unsatisfactory or unacceptable, the

instructor will promptly discuss the identified deficiencies with the student. The warning about

poor performance will be documented in the students evaluation form and the Performance Counseling process may be initiated.

- If the student’s performance warrants, the instructor will initiate the Performance Counseling process. The instructor will discuss the infractions that have occurred with the student, a

Performance Counseling form will be completed, and specific information will be given to the student to improve and correct behavior. The student may make comments to the form and must sign the form stating that it has been read.

Clinical Requirements - The clinical portion of the Basic Nurse Assistant course will be evaluated on a Pass/Fail basis

- Each student will be required to completely perform and pass the twenty-one (21)

Skills mandated by the Illinois Department of public Health (IDPH). The list of skills and guidelines are detailed further in the text materials.

- At each clinical session, the student will be evaluated on the ability to perform competently in

the clinical setting with regards to Safety, Medical Asepsis, and Patient/Residents Rights, and

the ability to apply theory knowledge.

- The student must abide by the objectives for the clinical setting and follow Carepaks Program

Policies and professional Conduct Policies at clinical sites.

- Whenever a student’s performance in the lab or clinical setting is deemed unsatisfactory or

unacceptable, the instructor will promptly discuss the deficiencies with the student and initiate

the Performance Counseling process.

- Failure to PASS this portion of the course will result in a failing grade for the entire course

regardless of the theory grade.

Professional Conduct Policy for Classroom and Clinical Socialization of students to appropriate professional conduct is a significant component of Carepaks CNA program. Students in the program are expected to be reliable and competent, exercise sound judgment, act with a high degree of personal integrity, represent Carepaks in a respectful manner, and

observe the rules and regulations of the clinical sites to which they are assigned. Students also have the responsibility to protect the welfare and safety of the residents/patients for whom services are being provided. In accordance with these expectations, when assigned to clinical sites, students in the program must

adhere to the following Policy for Professional Conduct; the policy includes but is not limited to the following: 1) Providing competent and reliable services and care to the Residents/Patients using sound judgment and discretion. Students are expected to:

a. be on time b. refrains from excessive absenteeism c. remain in assigned work areas, leaving only with permission d. not leaving patients unattended

2) Demonstrating respect and courtesy to patients and their families 3) Demonstrating respect and courtesy to instructors, supervisors, and all other members of the healthcare provider team at the site and in the classroom setting 4) Cooperating with faculty, staff and peers without insubordination

5) Providing care and/or services on a non-discriminatory basis

6) Performing only procedures and/or services authorized by instructors in accordance with accepted professional standards and scope of practice

7) Observing all rules and regulations of all clinical sites 8) Complying with the ethical/professional standards of the profession and the program including: a. proper use of equipment and other property b. not removing property of classroom or clinical facilities

c. not distributing or possessing or under the influence of illegal drugs or controlled substances d. not possessing or being under the influence of alcohol e. never taking prescription pads or writing prescriptions e. never falsifying documents regarding student performance or the care of patients

f. complying with drug testing (if it is suspected that a student may be under the influence, of drugs, Carepaks reserves the right to submit any student to a drug test if it is deemed necessary, and the cost incurred will be the responsibility of the student) 9) Maintaining confidentiality of patient information in accordance with recognized professional and

institutional rules without unauthorized release and /or misuse of patient information or institutional data 10) Maintaining proper dress, appearance, hygiene and decorum in accordance with the standards set by the program, clinical sites, and profession.

The following are examples of unprofessional behavior and misconduct at clinical sites and the classroom, which will result in disciplinary action, up to, and including dismissal from the Program; 1. Incompetence, neglect of duty, or poor performance of assigned duties 2. Excessive absenteeism

3. Failure to be at assigned clinical location or leaving an assigned clinical location without authorization 4. Theft or misuse of property 5. Verbal and /or physical altercations on site

6. Insubordination 7. Breach of Patient confidentiality 8. Harassment of a patient, instructor, staff, peers, or others while at the clinical site or in the classroom 9. Any other conduct or behavior detrimental to the program or clinical site

A Performance Counseling process will be initiated upon any violation. The penalty for violating the Professional Conduct Policy will be discipline up to and including dismissal from the Program. If the disciplinary action is dismissal from the Program, the student may appeal that disciplinary action

to the Carepaks Professional Review Board within 10 days of the discipline imposed. This appeal must be submitted in writing. The student will NOT be allowed to participate in the Program during the appeal process. The appeal will result in a conference before the Carepaks Professional Review Board within 10 days of the date the

appeal is filed. Both student and faculty involved in the disciplinary action may make an oral presentation to the Board in addition to written comments.

Confidentiality Policy The student must understand and agree to comply with the matters set forth by the HIPPA law and agree

that any breech thereof will result in disciplinary action up to and including termination from the program. The student has the responsibility to keep all information pertaining to Patients/residents that is learned as a Carepaks Student in the strictest confidence. It is agreed not to discuss/release any information obtained within the agency regarding any Carepaks

Health Service client, their medical record, or any patient/client’s condition with any individual not

directly associated with the clinical care or Carepaks Health Services nor with Carepaks students who are not directly associated with that patient/client. In addition, upon completing this program, The

student must agree to immediately return to the instructor all such confidential information, including, but not limited to:

Financial information

Patient/resident files or records

Test/exam materials

It is the policy of Carepaks Health Services that the internal and external business of this organization,

particularly confidential information, patient information and trade secrets, represent CarePaks assets and each student has a continuing obligation to protect.

Understanding Health Information Privacy

The HIPAA Privacy Rule provides federal protections for individually identifiable health

information held by covered entities and their business associates and gives patients an array of rights with respect to that information. At the same time, the Privacy Rule is

balanced so that it permits the disclosure of health information needed for patient care

and other important purposes.

The Security Rule specifies a series of administrative, physical, and technical

safeguards for covered entities and their business associates to use to assure the confidentiality, integrity, and availability of electronic protected health information.

Chemically Impaired CNA Student Policy/Procedures The Personnel at Carepaks Health Services have an ethical and professional responsibility to provide a safe and effective educational environment for students and patients who are receiving nursing care from students . The students are expected to participate in any learning experience, including classroom,

laboratory, clinical settings, and any other Carepaks sponsored functions free of any chemical impairment including withdrawal symptoms that impair the students functioning. Based on the assumption that addiction is an illness that can be successfully treated, Carepaks is committed to assisting students with recovery from substance abuse. In developing the policy and procedures for

intervening with a student who displays symptoms of substance abuse, Carepaks has adopted the following basic assumptions of the American Association of Colleges of Nursing Policy and Guidelines for Prevention and management of Substance Abuse In the Nursing Education Community (AACN 1992).

Carepaks Health Services, in developing this Policy, takes into consideration, and adheres to the following legal, ethical, professional, financial, and academic issues including:

Confidentiality of records and privacy (42USC Secs 290ee-3 and 290dd-3)

Anti-discrimination laws and agencies (American with Disabilities Act ADA (PL 101-336 ) 42

USC Sec.12101 et seq.

Drug Free Workplace Act of 1988 (41 USC Sec 701 et seq) that requires a drug free workplace

for recipients of federal funds

Safety laws (Occupational safety and Health Act, 29 USC sec. 651 et seq)

Carepaks Health Services adheres to the belief that: Substance abuse compromises both the educational process and patient safety and must be

addressed by the program faculty and administration.

Academic programs in nursing have a commitment to and a unique role in identifying abuse,

intervention, referral for treatment, and monitoring of recovering individuals

Addicted individuals need to recognize the consequences of their substance abuse

Addiction is a treatable illness, and rehabilitative and therapeutic approaches are effective in

facilitating recovery

Individuals with addictive illnesses should receive an opportunity for treatment in lieu of, or

before disciplinary action (dismissal)

Definitions Carepaks Health Services define the chemically impaired student as a person who, while in

the academic setting, or clinical setting, is under the influence of, or exhibiting withdrawal symptoms of,

or has abused, either separately or in combination, mind-altering substances including alcohol, over-the-

counter medications, prescribed medications, illegal drugs, inhalants, or synthetic designer drugs.

Chemical abuse is a disease process with physical, social, emotional, and spiritual aspects that can have either episodic or chronic presentations. Chemical abuse is a condition that can be treated successfully and that individuals can return to a productive level of functioning.

A student known to be chemically impaired places a legal and ethical burden on Carepaks faculty and Administration. Legal ramifications of a chemically impaired student are: injury to patients and potential malpractice, implicating the student, faculty, clinical agency, and educational institution. Carepaks has a responsibility to intervene when patient safety and clinical performance are

compromised by a student who is chemically impaired. When chemical impairment is suspected, Carepaks Procedure for Intervention with the Chemically Impaired CNA Student will be implemented.

The policy on the chemically impaired CNA student incorporates the following components: Prevention and education to Carepaks CNA students, faculty, and staff regarding substance

abuse and addiction.

Procedure for drug testing

Identification of suspected chemically impaired student

Intervention whereby the student is confronted with his/her reported behaviors and is asked to

seek evaluation of a possible substance abuse problem

Evaluation and referral for treatment

Provisions for re-entry to Carepaks upon successful rehabilitation

Disciplinary procedure for a chemically impaired student who is unable or unwilling to be

rehabilitated.

Procedure for drug testing

Drug and breathalyzer testing may be conducted by a qualified laboratory. Cost associated with drug and breathalyzer testing is the responsibility of the student. The procedure for urine collection, as determined by the collection site, will involve a witness to the voiding of the urine sample, securable urine containers, and chain of custody procedures. The urine sample will be screened for drugs as identified.

Available evidence, including health history and witnessed behaviors will be used to determine the presence or absence of drug abuse. Carepaks will ensure the confidentiality of results by making the information available only to the student and appropriate Carepaks staff.

Procedures for Administration/Faculty Intervention with the Chemically Impaired CNA Student Carepaks prohibits the illegal possession, use, or distribution of drugs and/or alcohol by students on campus property, client’s homes, or any affiliated activity, including off-campus learning facilities such as clinical sites. Violators will be prosecuted in accordance with applicable laws and ordinances and will

be subject to disciplinary action by Carepaks.

Identification Identification of impairment possibly due to substance use will be based on a pattern of observable,

objective, quantifiable behaviors (e.g. alcohol on breath, slurred speech, motor incapacities, absenteeism, severe tremors, or other known symptoms identified that suggest impairment of an individual’s ability to meet standards of performance, competency, and safety in clinical sites, classroom, or client homes.

Indicators suggesting chemical impairment of a student’s ability to meet standards of performance, competency, and safety are to be gathered and documented. If impairment is observed, the student must submit to random drug testing as described in the section on Drug Testing Procedure.

Refusal of a student to submit to testing may result in disciplinary action, including dismissal from the CNA program. Carepaks Personnel who suspect a student of impairment are to notify the Program Director and the Nursing Director of the clinical site of their concern.

Faculty/Administration is advised to keep detailed, anecdotal notes that describe the events involving the student. If the student demonstrates impaired behaviors that compromise patient safety and or academic performance, the student is to be removed from the setting to discuss the behaviors observed and allow

the student to provide a verbal explanation. If symptoms or other indicators of impairment are observed, the student will be informed of Carepaks responsibility to dismiss a student who is physically or mentally unable to meet the standards of performance, competency, and safety. The student is to be relieved of further clinical/lab/classroom or

client responsibilities for the day. The student will be informed that she/he is suspected to be in violation of Carepaks Chemically Impaired policies. It will be the student’s responsibility to arrange safe transportation to the testing facility for drug testing at that same time. Refusal of a student to submit to immediate testing or any other requirement of this Chemically Impaired policy may result in disciplinary

action including dismissal. The Medical Professional Panel Drug Screens includes common street drugs, prescription drugs, and those drugs that health care workers have access to and abuse. Drugs monitored may include:

Evaluation The student will be required to undergo a comprehensive evaluation of chemical abuse status by an approved professional.

A copy of the full report along with the name and contact information of the professional evaluator will be submitted by the student for review by Carepaks Administration and faculty. Refusal to submit to a comprehensive chemical abuse evaluation may result in dismissal.

If the Professional assessment and testing is positive for substance abuse, the student may be denied the opportunity to complete the required classroom and clinical rotations for graduating the current CNA program.

Re-entry The student will be expected to provide written documentation of the course of specialized treatment together with the treatment center, Physician, Substance Abuse Counselor or Mental Health Counselor’s written assessment of the student’s ability to perform the academic/clinical requirements of the CNA

program for students.

Noncompliance/Relapse

If additional or continuing substance impairment occurs subsequent to the implementation of these procedures, Carepaks faculty and administrative personnel will recommend that the student be administratively dismissed from the CNA program.

The student will be subject to random drug testing while enrolled in the CNA program. A positive drug test/breathalyzer, or refusal to submit to testing may result in dismissal from the CNA program as

outlined in the Chemically Impaired Employee/CNA Student Policy/Procedures

CONSUMER INFORMATION

Disclosure Information

Carepaks Health Services is currently accredited by the Illinois Board of Higher Education and approved through the Illinois Department of Public Health’s Education and Training Section for the Basic Nursing Assistant Training Program (# 0863).

This is a Certificate Program with no transferability to other institutions. This is a prerequisite for most Nursing Programs and depends upon successful completion of this program and passing of the Illinois State exam for Certification.

A new 6 week CNA course is offered every 7 weeks throughout the year. The Illinois Department of public Health allows up to 20 students per course.

Carepaks Health Services is a private company and is not involved in the placement of students in the job market or institutions of higher learning such as colleges.

The number of students who were admitted in this program for the reporting period From July 1st,2016 to July 1st 2017 were 86

There were 7 total courses offered during that same reporting period.

18 students withdrew from the program during the reporting period

68 students completed the program with a passing grade.

68 students took the State examination for Certification with 57 students passing the State certification examination in that same reporting period.

20 students were placed in their field of study.

37 students continued on in a Nursing program or related field of study after satisfying the prerequisite of a CNA certification for that field of study.

Some healthcare employers will hire CNA Certification pending students, prior to the student actually taking and passing the State examination. The average starting wage

for CNA Pending is $9.00 per hour. This amount goes up after the student passes the State exam.

Carepaks currently does not have any financial aid available but recognizes the need for some to

have the total cost for the program broken down into manageable payments.

Refund/Cancellation Policy - Refund calculations are based on date of withdrawal/termination from program. Initial deposit is

only refundable before the day of orientation. If a student wishes to withdraw after the 1st day

of class, refunds will be made after all non-refundable expenses listed have been deducted. No

refund of any kind will be issued after the first day of class. If the student is terminated from the

program by Carepaks Health Services Inc. for unsatisfactory progress, or misconduct, NO

REFUND will be issued.

- Any monies due the applicant or student will be refunded within 30 days of date of cancellation,

termination, or determination of withdrawal. If a student received a loan for tuition, a refund

will be made to the lender to reduce the student’s loan debt. If the amount of refund exceeds the

unpaid balance of the loan, the balance of funds will then be applied in the following order:

- 1. To pay authorized charges at Carepaks Health Services

- 2. Returned to student

- In cases of prolonged illness, accident, or death in the family, or other unavoidable

circumstances that make it impractical to complete the program, Carepaks will make a

settlement that is reasonable and fair to both parties.

- Carepaks will make refund determinations for all tuition and fees paid up to cancellation date, in

accordance with the above mentioned schedule, after all expenses have been deducted.

STUDENT’S RIGHT TO CANCEL

The student has the right to cancel the initial enrollment agreement anytime up until the end of

the 1st day of class after the student has been admitted. If the right to cancel is not given to any

prospective student at the time the agreement is signed, then the student has the right to cancel the agreement at any time and receive a refund on all monies paid to date within (14) days of cancellation. Cancellation should be submitted to the authorized official of the school in writing.

Grievance Procedure Policy All student grievances, issues, differences of opinion are to be resolved directly with the instructor. Should a student feel the need to discuss a grievance with the Program Coordinator, the student must make an appointment to discuss the grievance. The appointment will be a joint meeting with the

student, instructor and program coordinator. If the student wishes to make an appeal regarding any disciplinary decision, this appeal must be written and submitted to the Carepaks Professional Review Board within 10 days of initial disciplinary action. If a student should feel that their grievance was not adequately or appropriately addressed, they may

contact the Illinois Board of Higher Education for further assistance.

2. This agreement is a legally binding instrument and is only binding when the agreement is accepted, signed, and dated by the authorized official of the school or the admissions officer at the school’s

principal place of business. Read all pages of this contract before signing.

3. You are entitled to an exact copy of the agreement and any disclosure pages you sign.

4. This agreement and the school catalog constitute the entire agreement between the student and the school.

5. Any changes in this agreement must be made in writing and shall not be binding on either the

student or the school unless such changes have been approved in writing by the authorized official of the school and by the student or the student’s parent or guardian. All terms and conditions of the agreement are not subject to amendment or modification by oral agreement.

6. The school does not guarantee the transferability of credits to another school, college, or university. Credits or coursework are not likely to transfer; any decision on the comparability, appropriateness and applicability of credit and whether credit should be accepted is the decision of the receiving institution.
