Dear Mr. Carkido:
RE: RED FOX WWTPPERMIT NO. 3PH00037PORTAGE COUNTYSHALERSVILLE TOWNSHIP
On December 7, 2011 an inspection of the above referenced facility was conducted. Thefacility was represented by Mr. Don West, Class IV Superintendent and you. The purpose of theinspection was to: (1) evaluate the performance of the wastewater treatment system along withthe facility's compliance status with respect to the terms and conditions of the above-referencedNational Pollutant Discharge Elimination System (NPDES) permit and (2) determine requiredadditional actions to be undertaken as part of the permit renewal process.
During the inspection, the following items were noted/discussed:
1. The plant design of the wastewater treatment system is 160,000 gpd.2. In accordance with Ohio Administrative Code (OAC) 3745-7-04, the sewage
treatment facility is classified as a Class II facility. Once the facility's permit isrenewed, the permittee shall ensure that the treatment works operator of record isphysically present at the facility five times per week for a minimum of twenty hoursper week.
3. The trash trap is pumped out every 7-10 days.4. Detail plans (permit-to-install [PTI] application number 802314) for influent screening
equipment to replace the existing influent trash trap was approved on April 1, 2011.5. The blowers were running and the plant was receiving good aeration.6. The sludge digester was also being aerated.7. The aeration portion of the treatment system was comprised of an East and West
tank.8. The contents of the aeration tanks were orange-ish brown in color. A small amount of
foam was present on the surface of both tanks. You indicated that the foam is notalways present in the tanks. The cause of the orange-ish color is caused by theaddition of ferric chloride.
9. Both skimmer return lines were working properly and returning clear water.10. Both sludge return lines were working properly and were returning medium brown
water.11. The weirs and the sidewalls in the settling tank were free of scum/solids build-up.12. The treatment system has two IDI tertiary sand filters. Both were operational.13. The treatment system is also equipped with an Ultra Violet disinfection unit. The
system is functional. However, it is not required to be operated this time of year.

14. The temperature reading in the automatic sampler was 5.0 degrees centigrade. Thetemperature should read 4.0 degrees centigrade. Mr. West indicated that he wouldcalibrate the unit.
15. The discharge was clear.16. Please note the following includes some of the changes to the facility NPDES permit:
A. A permanent marker at the outfall is required to be posted per therequirement of Part II, letter I of the facility's NPDES permit
B. A schedule of compliance will be placed in the permit renewal which requiresthe facility to meet E. Coli limits in the final table for outfall monitoring station3PH00037001.
C. A schedule of compliance will be placed in the permit renewal which requiresthe facility to meet mercury limits in the final effluent table for outfallmonitoring station 3PH00037Q1.
D. Color, severity and odor, severity have been removed from station3PH00037001.
E. Nitrite plus nitrate, total monitoring has been added to station 3PH00037001.F. Total dissolved solids monitoring has been added to station 3PH000370.G. Copper, total recoverable has been given a daily maximum effluent
concentration limit of 41 ug/l in both the interim table and final table formonitoring station 3PH00037001.
H. Sampling station 3PH000371 has been removed.17. Per your recent phone conversation with Mr. Bill Zawiski, Water Quality Manager, of
this Office, it was recommended that Portage County Water Resources Hire aconsultant to assist the County in submitting mercury variance requests for all of theCounty's waste water treatment facilities which contain mercury monitoring.
18. Once the facility's renewal permit is drafted it will be public noticed. During publicnotice, you will have a 30 day period to evaluate your permit and comment in writingany concerns you may have.
This office has recently reviewed your self-monitoring reports covering the period November 1,2008 through September 30, 2011 for the referenced facility. Our review indicates violations ofthe terms and conditions of your NPDES permit. The specific instances of noncompliance areas follows:
