    HISTORY

    At about 7 am on the morning of 09/04/1992 Mr Fumbatha

    was assaulted with knobkierries and tjsamboks.

    As a result of the assault he sustained the following

    i njuries:

    1) A head injury

    2) Injuries to his right arm

    3) Injuries to his left forearm

    4) Injuries to his back

  • Following the assault he was taken by ambulance

    to the Natalspruit Hospital. At hospital X-rays

    were taken of both forearms and a wound on his

    head was stitched.

    Following the assault he was unable to work for

    approximately one week and he continued getting

    headaches for approximately two weeks.

    His symptoms gradually subsided, however he continues

    to have problems with his right shoulder.

    PRESENT COMPLAINTS

    He experiences pain in his right arm when he lifts

    heavy objects.

    EMPLOYMENT

    Prior to the assault he was a drivers assistant.

    Following the assault he was off work for approx

    imately one week and then returned to his previous

    job.

    He carried on doing this until January 1994 when

    the firm closed down. He says that the assault

    has affected his ability to work in that he c a n’t

    lift heavy objects and experiences pain in his

    arm at times.

  • His formal education consists of having passed

    standard six.

    SOCIAL AMENETIES

    Lifting heavy objects causes him pain in his right

    a r m .

    GENERAL HEALTH PRIOR TO THE ASSAULT

    His health was good. He was on no medications and

    specifically did not have any problems with his

    right shoulder.

    EXAMINATION FINDINGS

    He appeared generally well.

    He had a mark approximately 5 cm by 1 cm wide over

    the right back a r e a . He had several other smaller

    marks over his back.

    RELATED TO HIS RIGHT SHOULDER

    There were crepitations present on full passive

    abduction of his right shoulder. There was

    tenderness over his supraspinatous tendon.

    He experienced pain on active abduction from about

    60° onwards.

    DIAGNOSIS

    As a result of the assault Mr Fumbatha sustained

    multiple injuries including a head injury with a

    laceration to his scalp. The bulk of the injuries

    have healed, however he still has visible marks

    on his back and he had features of a rotatnr cuff

    inj u r y .

    PROGNOSIS AND FUTURE

    More than two years after the assault Mr Fumbatha

    has features of a rotator cuff injury. This should

    be treated with a course of anti-inflammatories

    and an intensive course of physiotherapy.

    This will cost approximately R 1,100.00 at todays

    p r i c e s .

    Following this his symptoms should be largely

    settled, however the possibility exists that he

    will require further treatment because of the

    chronic nature of his injury.

    It would be reasonable to allow for a further two

    such treatment programs.

    In the longer term operative intervention may

    become necessary. This would take the form of a

    surgical decompression of his shoulder.

  • This would involve a period of hospitalisation of

    approximately three days, a cost of approximately

    R 6,000.00 and a period of incapacity of approx

    imately six weeks.

    CONCLUSION

    As a result of his assault on 09/04/1992 Mr Fumbatha

    sustained multiple injuries. He retains scars and

    marks which are likely to remain as permanent

    disfigurements.

    He has a shoulder which has remained symptomatic

    for approximately two and a half years and currently

    requires medical treatment and because of the

    chronicity of his symptoms may require treatment

    over a prolonged period of time.

    His ability to work has been adversely affected

    by his shoulder injury as have his social ameneties.

    I find no reason to suspect that his life expectancy

    has been altered by the orthopaedic injuries
