Attached are two original copies of Change Order No.4 for the subject project. This change order provides changes to the contract for the following:

• Provide compensation for changes in striping to properly accommodate the new Shell road alignment (City of Georgetown), it has been necessary to change/modify the lane configuration in the vicinity of this intersection(Sta. 155+00 to 127+38).

• Compensation for special excavation at sink hole as recommended on ACI consulting report item . no. 2(Sta.85+00).

• Compensation for additional drive ways and drainage pipes with SET not included in original plans (sta.20+00, 20+20 and 1086+88), and gate relocation (sta.20+20)to adjust entrance due to change in grade of existing driveway.

• Compensation for the 6" HMAC to allow construction of tie in points at sta. 15+50 to 18+65 and sta. 1072+50 to Sta. 1074+20. Removal of concrete driveways and construction of concrete driveways.

Please execute both original copies of this agreement, and return to my attention for further processing.
