EXECUTIVE MESSAGE 5
A MESSAGE FROM OUR BOARD CHAIR AND OUR PRESIDENT & CEO
The measure of accountability for any hospital
is both simple and complex at the same time.
It is found in the answer to one question: did
this hospital give its patients exemplary clinical
care, and did they do it with caring and
respect? If a hospital can answer yes, then it
has successfully met its responsibility to its
patients and its community.
But how do you know what the answer to that
question truly is? At Humber River Hospital we
have many internal strategies and benchmarks
for ensuring the care we provide to patients is
of the highest possible quality. Each one
measures our performance against the latest
standards for hospital care or operating
efficiency. To bring even more rigour to our
quality review initiatives, we regularly validate
this work by subjecting it to unbiased
examination from external healthcare experts.
This past year we undertook several such
examinations. In one, our Laboratory Services
Team received a full Four Year Accreditation
from their Ontario Laboratory Accreditation
(OLA) assessment.
Surveyors from the OLA were at Humber to
review the performance of both the Finch and
Church site laboratory sections against over
five hundred quality standards. With scores of
over 98% and 97% respectively, both labs
demonstrated the competency and integrity of
our Laboratory Services Program.
In another, four Accreditation Canada
surveyors spent a week reviewing our
organization’s policies and practices across all
levels: for safety; for quality of care; for fiscal
responsibility. They interviewed staff,
physicians, patients, families and community
partners. And they judged Humber River
Hospital worthy of Accreditation with Exemplary Standing – their highest possible
Award.
To be awarded Exemplary Standing by Accreditation
Canada is something all Canadian hospitals strive for – to
receive it speaks volumes about the dedication and skill
of our staff, physicians and volunteers, and about the
quality of care they provide every day for our patients and
their families.
Results such as these Accreditation decisions say that
patients can be confident they will receive the excellent
clinical care they need from our hospital. It says that this is
a group steadfast in their mission to provide that care with
compassion and with caring. It says that this is a hospital
team you can believe in.

ACCREDITATION CANADA6
ACCREDITED WITH
“EXEMPLARY STANDING”
with a 100% compliance score for all Required
Organizational Practices. This result places
Humber firmly among the top performers in the
country.
The decision letter from Accreditation Canada
reads in part: “This achievement demonstrates
your organization’s determination and
commitment to ongoing quality improvement.
We applaud your leadership, staff, physicians
and accreditation team members for their efforts
and dedication to the provision of safe, quality
health services.”
And at our survey debriefing the lead surveyor,
Andrew Williams, spoke of the Humber River
Hospital team’s “passion and commitment to
quality and to doing what is right for patients.”
On March 26, 2014, Accreditation
Canada announced its final decision
resulting from their latest Accreditation
survey at Humber River Hospital:
Accreditation with Exemplary Standing. This is the highest possible
Award that can be obtained under the
Accreditation survey process for
Canadian hospitals.
An independent, not-for-profit
organization, Accreditation Canada is
the national body charged with
evaluating healthcare organizations
across Canada, including hospitals,
against more than 2,000
comprehensive standards for quality
and safety. Our hospital scored 98.3%,
98.3% COMPLIANCE RATE WITH
MORE THAN 2,000 ACCREDITATION
STANDARDS; AND A 100%
COMPLIANCE RATING WITH ALL
ACCREDITATION CANADA REQUIRED
ORGANIZATIONAL PRACTICES
OLA ACCREDITATION 7
In addition to their contributions to our Accreditation
Canada success, our Laboratory Services Team
received a full Four Year Accreditation from their
Ontario Laboratory Accreditation (OLA) assessment.
Surveyors from the OLA were at Humber River to
review the performance of both the Finch and
Church site laboratory sections against over five
hundred quality standards.
Among these were quality goals, management
commitment, communication within and outside the
laboratory, client satisfaction and sound reporting
practices. With scores of 98% and 97% respectively,
both labs demonstrated the competency and
integrity of our Laboratory Services Program.
The surveyors made many positive comments at
their debriefing. In particular they noted how
impressed they were by how “the good internal
working relationships and team cooperation showed
throughout their work.” The surveyors also pointed to
the great work done on quality management: how
the staff was “‘strict’ with each other in a good way to
ensure quality performance.”

SUCCESS: DEMONSTRATING
COMPETENCY AND
INTEGRITY
The approval of our hospital’s
redevelopment plans presented a
unique opportunity for Humber
River Hospital to push the
envelope during the planning,
design and construction phases
– to bring to life the hospital
facility our community needs and
deserves. This past year saw the
results of that work really take
shape as our new building rose
to full height, with the exterior
cladding mostly finished.
From the start of the planning
process for the new Humber
River, the work needed to be
ground-breaking. The planning
and design had to result in a
hospital that will still be relevant
30 years from now, capable of
adapting to new work flows and
future technologies.
So the core design question
became ‘does this element
support one or more of our
project vision elements (Lean,
Green and Digital) to help us
create a patient-centred care
environment within a workplace of
distinction? And as work
progressed this year, one of the
answers to that question began
to become clear - connectivity.
“Our work on the new hospital this past year was focused on supporting the future delivery of high-quality, efficient, safe patient care. Most of all, it was about putting the patient at the centre of their care in a real and meaningful way.”
Barb Collins, COO and Redevelopment Lead
OUR NEW HOSPITAL
OUR NEW HOSPITAL8
OUR NEW HOSPITAL 9
For example, electronically
connecting patients to all
aspects of their care became
a priority. Monitoring and
treatment equipment
connected to you or your
smart bed will automatically
record information about you
and alert the care team of
abnormal results or activity.
You may use your smartphone
to connect to the hospital.
Once in your inpatient room,
the bedside terminal allows
you to see your health record,
with test results, images and
notes about your progress. It
also allows access to email,
food menu, health education
videos, room climate and
window shading controls and
online entertainment – all this
and more.
In our new hospital, families
will also be connected to the
care process like never before.
The videoconference feature
lets patients communicate with
them any time day or night,
without disturbing others,
because most inpatient rooms
are single-bedded rooms. And
should they choose, there’s
space built into each inpatient
room for a family member or
friend to spend the night
comfortably.
Our work on the new hospital
this past year was focused on
supporting the future delivery
of high-quality, efficient, safe
patient care. Most of all, it was
about putting the patient at the
centre of their care in a real
and meaningful way. It is about
reinventing patient care.
Building a patient and family
friendly hospital is priority one for
Humber River Hospital’s planning
team. Being community friendly is
also very important to us, and our
approach to environmental
sustainability and campus design
shows this in a very meaningful
way.
The new hospital will be a green
building, incorporating the latest
in environmental sustainability
concepts. The construction is on
track to meet the Tier 1
requirements of Toronto’s Green
Standard, and we are confidently
looking to achieve LEED® Gold
certification.
An emphasis on natural lighting
use, reflective and green roofing,
and other environmental
strategies will reduce energy use
by more than 40% in our new
building over the ASHRAE model.
PCL has been working with
recycled and locally sourced
building materials, and 75% of
construction waste has been
diverted from landfill.
The installation of low-flow
bathroom fixtures and irrigation
cisterns will reduce the use of
water by 35% compared to our
current facilities.
In addition, patients will benefit
from improved air quality with
the use of 100% fresh air and
low Volatile Organic
Compound emission finishes
throughout the building.
Staff will be encouraged to use
alternative transportation
through the hospital’s
partnership with Smart
Commute Toronto, with bicycle
parking and showers, as well
as vehicle charging stations.
Thanks to our green approach,
the new Humber River Hospital
will help reduce CO2 by
roughly 20,000 tons annually.
That’s the same as taking
about 4,000 cars off the road.
The new hospital is surrounded
by a welcoming,
community-friendly campus. It
features native,
drought-resistant plants,
healing gardens and walking
paths, public art and a
community piazza, all part of
our goal to be not just a great
hospital, but a great neighbour
as well.

A PATIENT’S STORY:IN THEIR OWN WORDS
A PATIENT’S STORY 11
My 79 year old father had major surgery and was discharged yesterday. I was at my father's side from the pre-op procedure, to the recovery room, to ICU and finally to the third floor inpatient unit.
First of all I want to thank my father's surgeon Dr. Alexander Iskander for his excellent work during office visits and the actual surgery.
I would also like to thank the ICU doctor who was very thorough when speaking to me. I believe his name was Dr. Cooligan. I also want to thank the nurses in recovery who were respectful and did their job very well. Special thanks to Raj in ICU who was wonderful.
Finally a very big thank you to the nurses on the 3rd floor, especially
Rinechia, Stella, Josie, Lina and Lizzy. I was impressed with everyone that we came across on the 3rd floor.
Once again thank you to all for their hard work.

TRANSITION PLANNING:SAFE. SEAMLESS. SUCCESSFUL.
October 18, 2015 will be opening day for Humber River
Hospital!
To ensure a smooth transition to our new building,
Humber River has retained the services of a Canadian
firm famous for successfully moving hospitals: Health
Care Relocations (HCR). Among the high profile hospital
moves HCR has coordinated are the Niagara Health
System, Walter Reed Hospital in Washington, Bridgepoint
and the UCLA Medical Centre in Los Angeles.
Across the organization, our Transition Planning Teams
are hard at work with HCR, preparing for moving day,
where timing is not only crucial but critical, and careful
preparation is at the core of a successful, safe move.
Our moving plan will be in place months in advance, and
there's a period of four to five months before the official
opening to ready the hospital for the actual move. The
hospital will be operational before the first patient enters
its doors, and the entire transition has to be seamless.

ASPEN GROVE ART GLASS14
THE ASPEN GROVE SYMBOLIZES THE SHARED BELIEF IN A UNITY CONNECTING THE
RICH DIVERSITY OF HUMAN CULTURES, SOCIETIES AND RELIGIONS.
EACH TREE STANDS AS AN INDIVIDUAL, YET IS CONNECTED TO THE WHOLE.
ASPEN GROVE
The new Humber River Hospital is designed to
complement the local streetscape and architecture,
and to fit into the neighborhood in a real and
meaningful way. The most visible of these design
elements, located along both the north and south
exposures of the new building, is our Graphic Art
Glass architectural feature: Aspen Grove.
Throughout time Aspen Groves have been embraced
by many cultures as symbols of unity and of strength
through diversity. This art glass installation celebrates
our connection with our patients, our community and
each other: each diverse member a part of the larger
entity, joined together for the benefit of all.
The art glass will also be featured prominently as a
design element within our new hospital’s
Spiritual Centre.
A PATIENT’S STORY 15
First, I must tell you about the nursing care for my mother while on the 6th floor. My mother was cared for by the best nursing care I have ever seen. I must tell you your staff is the best.
Marisa, the nurse on the 6th floor, is one of the most caring individuals I have ever met.
This lady is not only a great nurse, but a great person. She is outstanding. And I have to say there is a great lady working in your food area, Saleema, she is such a great person. Very good at what she does, but also very caring, such an individual you would be hard pressed to find.
I was a supervisor for Toronto police, I am now retired, however, if I had these people on my shift, it would truly be a treasure.

APOTEX PLEDGES$10 MILLION TO SUPPORTTHE NEW HUMBER RIVERHOSPITAL
A HEALTHY INVESTMENT16
“Apotex, as an organization, and Jack Kay personally, have been dedicated to this community and our hospital for over 20 years. This latest gift is yet another tangible expression of their commitment to making community investments that have meaningful results.”
Dr. Rueben Devlin, HRH President & CEO
In August, Apotex Inc.
announced a major gift in
support of the Emergency
Department at the NEW
Humber River Hospital.
Apotex President & COO Jack
Kay joined our President &
CEO, Rueben Devlin, Health
Minister Deb Matthews and
friends of Apotex and the
hospital at the construction
site of the new hospital to
make the announcement.
As Mr. Kay noted, “with our
headquarters in Ontario and
over 3,000 employees in the
hospital catchment area, this
hospital is very important to
Apotex. We have had a long
association with HRH and
many of our employees have
benefitted from the care
provided at the hospital.
This is why we are proud to
support Humber River Hospital
with a significant gift of $10
million in support of the
Hospital Foundation’s $225
million capital campaign.”
The new hospital’s Emergency
Department will be named in
recognition of Apotex’s
generosity. It is expected to
see over 114,000 visits in the
first year of operation.
A PATIENT’S STORY 17
My 9 year old daughter chipped a bone in her finger and we have been to Emergency and fracture clinic a number of times over the last 2 weeks.
I am completely impressed with the care we received from both the Emergency and fracture clinic.
I apologize I did not get the names of your staff to thank them personally. However,
thank you to the 2 ladies at triage for joking around with my daughter and making her smile; to the registration desk for adding a little fun to the process; and to the nurse and doctor in Emergency for your very fast and friendly care.
To the fracture clinic, we had interaction with 4 individuals that absolutely went above and beyond. The young
lady at reception, the nurse as well as Val and the other gentlemen in the cast area. One young man even offered to buy me a coffee.
A job very well done! Thank you.

A PATIENT’S STORY18
I was a patient in your hospital with Dr. Hagen. I must say the quality of care I received was exceptional. I had already heard that Dr. Hagen was a great surgeon, and Dr. Glazer runs a great program.
But what stands out was the high quality of the nursing care I received. The three nurses who looked after me were Jennifer, Gylanne and Derrick.
Jennifer is a gentle sensitive caring woman who obviously cares deeply for her patients. Gylanne showed me a no-nonsense attitude combined with a real desire to listen and help. And Derrick was amazing... he shows a real dedication to his work.
All these nurses displayed a strong caring attitude that does them credit.
Thanks to them I am well on the way to recovery and am reaping the benefits of this life changing surgery.
Please pass on to them my sincere thanks and my thanks to you for listening.

YORKDALE + HUMBER RIVER:SHOWCASING THE FUTURE
CIBC MAKES A HEALTHYINVESTMENT IN THE NEWHUMBER RIVER HOSPITAL
A HEALTHY INVESTMENT 19
“Big thanks to every member of the Yorkdale team for their help with this terrific day of community engagement. We were thrilled to partner with such an outstanding organization, and to work with such great people.”
Barb Collins, COO and Redevelopment Lead
Yorkdale Shopping Centre chose Humber
River Hospital as their partner for
Canada’s first Giving Tuesday on
December 3rd – a day designed to
encourage charitable giving and
volunteering during the holiday season.
Yorkdale provided space for a display
featuring our Da Vinci robotic surgery
simulator. Shoppers were invited to stop
by and try the simulator for themselves.
Shoppers were able to make donations
on their mobile device, and our friends at
Yorkdale donated $1 for every re-tweet
with the hashtag #YorkdaleGivesBack.!
Representatives from CIBC gathered at
the construction site of our new hospital
last year to celebrate CIBC’s generous
pledge of $500,000 in support of our
Foundation’s Capital Campaign.
“With a community investment focus on
Kids, Cures and Community, we are
proud to support the Humber River
Hospital Foundation,” Senior Vice
President Larry Tomei said. “We’re
committed to working with organizations
that support those living with cancer;
research into early detection and
advanced treatment methods; and
ultimately towards finding a cure.”
The new hospital’s Cancer Centre waiting
room will be named in recogintion of
CIBC’s investment in our community.
OUR VOLUNTEERS
OUR VOLUNTEERS:AN IMPORTANT ROLE
From our very first days Humber River volunteers and the
Volunteer Association have been a vital part of our hospital.
Through every minute they’ve shown tremendous
compassion and caring for each patient who walked
through our doors.
Last year, those minutes totaled more than 67,000 hours
across more than 50 areas of the hospital. They also
launched Volunteer Student Alliance programs at both the
University of Toronto and York University. And joined with
Plan Group, one of our partners on the new hospital, to
create the first corporate volunteer program at Humber.
Last summer twenty-nine Humber volunteers received
Ontario Volunteer Service Awards, presented by the
Province in celebration of their commitment and outstanding
contributions to volunteerism.
Everyone at the Hospital, from the Board of Directors down,
sincerely appreciates their ongoing support. Our volunteers
play an important role in making us a very special place.

A PATIENT’S STORY 21
I just wanted to show my appreciation and thanks for all the wonderful care I received before, during and after my surgery in Humber River Hospital.
My surgeon was Dr. Heller; he is a wonderful man and an exceptional surgeon. The nursing staff on the 5th floor was very professional, quick, knowledgeable and top notch in all. I wish I could remember all their names!
Ronald is an excellent therapist as well as the physiotherapy assistants. I could not have made the progress I did without the expert help of my therapist Christine who was so patient, gentle and had a smile at all times, no matter how tedious I was!
Could you please relay my thanks and appreciation to all involved? Everyone works so hard they
deserve to know this was a job excellently done! I will always remember this experience with admiration for everyone in the hospital and my eternal thanks.

STUDENT VOLUNTEERS:RAISING AWARENESS
STUDENT VOLUNTEERS22
“Student volunteers play an important role in our plan to engage youth and have them ‘grow up’ with Humber River Hospital. The students are a passionate and energetic group who are committed to promoting Humber River on campus and in the community”
Louisa Ceci, Manager, Volunteer Services.
Last year, students from York
University and the University of
Toronto each launched a
Student Alliance (HRHSA) club
affiliated with the Humber River
Hospital Volunteer Association.
Their goal is to strengthen the
relationship between students
at the universities and our
hospital through their
partnership with the Volunteer
Association. Each HRHSA
strives to enrich the lives of the
student body while raising
awareness and funds for
Humber River Hospital through
on-campus social and
academic events.
The student clubs participate
in orientation week, organize
campus activities, set up
displays and run information
tables to promote awareness
about Humber River and the
club. In November the U of T
club hosted a volleyball
tournament, and the table at
York University is directly in
front of a large wall, which has
a large banner highlighting our
new hospital.
In their first year of operation
the two clubs raised close to
$2000 through events such as
fashion shows, samosa and
bake sales, and pub nights.
Recruitment has already
grown to over 300 members.
CORPORATE VOLUNTEERS:HELPING AT-RISK YOUTH
CORPORATE VOLUNTEERS 23
As a redevelopment project
partner, Plan Group is
coordinating the installation of the
data communications and
electrical systems needed to
support our ICAT infrastructure.
As a donor, Plan Group has
committed $250,000 to the
Hospital Foundation.
But CEO Bill Kurtin wanted to do
more than donate: “we didn’t just
want to hand over a cheque,” he
explains. So Bill asked Pina
Lostritto, his Executive Assistant,
to act as a liaison between Plan
Group and Humber River
Hospital. “Since Plan Group has
an interest in helping at-risk
youth,” Pina says, “we decided
we wanted to volunteer with the
Child and Adolescent Mental
Health Unit.”
So, three nights a week, three
members from a group of 14 Plan
Group volunteers socialize and
play with the kids. For those
hours, the kids aren’t thinking
about their treatment or being in
hospital. They’re just focused on
having fun with the Plan Group
volunteers. This can range from
playing cards or ping pong, doing
art work, or talking quietly in a
corner. Sometimes, the
volunteers and teens engage in a
two hour sing along, showcasing
the unique talents of these kids
“We call ourselves the elite
playing squad,” says Pina.
Doris Galas, Clinical Manager in
the Child and Adolescent Mental
Health Program, says that “the
experience has been absolutely
amazing. To have people taking
the time to come and spend
quality time with the kids, and
making their time here more
meaningful, is very valuable and it
means a lot.”
The Plan Group volunteers even
surprised the Unit with presents
at their Christmas party. “Things
like new movies, a large supply of
popcorn, a karaoke machine, and
arts and crafts supplies,” says
Doris. “The Plan Group volunteers
help make the lived experience
on the unit easier for the kids.”
“The experience has been absolutely amazing. To have people taking the time to come and spend quality time with the kids, and making their time here more meaningful, is very valuable and it means a lot.”
Doris Galas, Clinical Manager, Mental Health Program
A PATIENT’S STORY:IN THEIR OWN WORDS
A PATIENT’S STORY24
I was a day surgery patient at Humber River Hospital for an Arthroscopy procedure on my knee.
I have to say I have walked away with a great deal of praise and respect for the staff as well as appreciation and thanks for the care I received.
I was tremendously comforted by the warmth of the staff and well, they were just
always smiling and ensuring I was looked after.
Dr. Cayen came out to see me in the holding room, explain again what he would be doing in surgery and on being wheeled into the OR, speaking with the anaesthetist and nurses, I felt in very good hands.
Waking up in the recovery room was another excellent
experience. I made sure I remembered the name of that Nurse... it was Debbie Moore. She was so cheerful, warm and caring; it was like having my own mother looking after me.
A very big thank you to HRH.

I want to say thank you for the great care my daughter has been receiving on Paediatrics.
The Nurses there have been so kind, thorough and Professional! It is just wonderful especially because when you are worried about your child, this makes things so much easier.
I'd also like to thank our Pediatrician Dr. G. Miller, whom we adore, respect, and admire so much for his enormous knowledge and great treatment of Emily!
Also I'd like to thank Dr. Hagen and his Resident & Clerk for seeing Emily so quickly, and giving us quick answers to her treatment.
What could have been a much more difficult time here due to Emily's illness, was made much easier by HRH's staff, treatment, and over all atmosphere!
Thank you so much.
