For many patients and their loved ones, having a choice about where they spend their final days is very important. Our Myton at Home service enables patients to remain in the comfort of their own home and be supported in their final weeks or days of life.

WHAT DOES OUR SERVICE OFFER?

We offer a high quality support service delivered by experienced staff who are specialists in end of life care. This service operates seven days a week, providing in your home:• Personal care (attending to a patients’ personal needs such as washing,

dressing and toileting)• Support for the whole family, providing emotional and psychological

support as well as practical advice • Night sit support for patients at end of life, usually 3 per week

WHO CAN ACCESS OUR SERVICE?

The service is available to anyone living with a terminal illness which no longer responds to curative treatment, is in the last weeks of life, and is registered with a Rugby GP. We support people with cancer and non-cancer illnesses.

HOW TO ACCESS OUR SERVICE

Referrals can be made via your District Nurse. Once you have been referred, our Senior Nurse will visit or contact you to discuss your nursing needs and will ensure that the care is tailored to meet these needs in the most appropriate way. Our Myton at Home service will complement the existing medical and nursing care that you are receiving.

WHO WE WORK WITH

We work hand in hand with District Nurses, Macmillan Nurses, other local hospices, care providers and nursing homes to ensure you have continuity of care and support.

HOW DOES THE SERVICE OPERATE?

Myton at Home is made up of a small team of nurses / nursing assistants. As the team is small, we can assure you that our nurses will become familiar with you. All of our team are specialists in end of life care.

HOW MUCH DOES THE SERVICE COST?

Our Myton at Home service is provided free of charge by The Myton Hospices and we are a registered charity. Over 80% of our income is generated through voluntary funding.

HOW OFTEN WILL WE VISIT AND FOR HOW LONG?

We will visit up to twice a day, once in the morning and again in the afternoon. We will be with you for around one hour but our visits are not at specific times, with the exception of night sits. Nursing care can be provided 10pm - 7am.

WILL YOU NEED TO PROVIDE ANYTHING?

Should you need any specific equipment for your home then this will be discussed and arranged with your District or Community Nurse. We will also assess your needs and will be able to supply items such as slide sheets which enable our nurses to move patients safely.

WHAT HAPPENS IF I CAN NO LONGER COPE AT HOME?

Sometimes being at home at end of life or caring for someone at home at end of life can be more difficult than expected. If staying at home is no longer possible or appropriate, the Myton at Home team are skilled in being able to support individuals and families to know and understand the choices around options for care. The Myton at Home team in conjunction with the wider team is able to make referrals to other care services including The Myton Hospice’s Inpatient unit.
