How to pre-register for your COVID-19 vaccine with PrepMod - English

Step 1 Go to https://www.comassvax.org/, click your language preference in the upper right hand

corner

Step 2 Click Pre-Registration Sign Up. Fill in information. Please select the most appropriate occupation.

If you have two jobs, please select the occupation that is eligible first

If you do not work, or there is not an option for your occupation, please select other. Summit

County PH will send an additional survey to assess what eligibility category

It is best to use your own unique email address if you have one. However, any email that is listed

on this form should be an email that you have access to, and is checked frequently. Email will be

our primary method of contacting you.

Please select Summit as the county if you want to be considered on the Summit County waiting

list

Step 3 Below your information, sign or type your name and enter the date

You can toggle between signing and typing depending on your preference

Step 4 Click Save and Continue. You are now registered!

How to confirm your appointment if you have been randomly selected- PrepMod - English

Step 1 Open the registration email from PHCovid19Response@summitcountyco.gov.

Click on the green button with the appointment date.

Step 2 Select an appointment from the list of available time slots. Then click Save and Continue.

Step 3 Fill in personal information. Click Save and Continue.

Step 4 Choose your insurance type. Click Save and Continue.

Step 5 Fill in health questions. Indicate whether you will be receiving your first or second dose. Click

Save and Continue.

Step 6 Sign or type your name. If you are registering on behalf of someone else, indicate this below your

signature. Click Save and Continue.

Step 7 Review your information. Click Save and Continue.

Step 8 Receive an email confirmation with your appointment details.

Step 9 Save the date and location for your appointment, found in the confirmation email.
