1. The promoter is: FONA Dental s. r. o (Company No. 45 691 118), whose registered office is at Štefánikova 7, Bratislava 811 06, Slovak Republic.

2. The competition is open to anyone.

3. The competition is open for registration from 25.8.2016 to 10.9.2016.

4. By entering this competition, the entrant is indicating his/her agreement to be bound by these terms and conditions.

5. Only one registration and answer will be accepted per person. Multiple entries from the same person will be disqualified.

6. The closing date for entry is 10.9.2016. After this date no further entries to the competition will be permitted.

7. No responsibility can be accepted for entries not received for whatever reason.

8. The rules of the competition and how to enter are as follows: I. The entrant visits http://www.fonadental.com/fdipoznan2016/ and solves a puzzle displayed on the website. II. The entrant submits the correct answer by choosing from listed alternatives A) or B) or C), and submits his/her contact information in a registration form.

9. The promoter reserves the right to cancel or amend the competition and these terms and conditions without notice in the event of a catastrophe, war, civil or military disturbance, Act of God, or any actual or anticipated breach of any applicable law or regulation or any other event



outside of the promoter’s control. Any changes to the competition will be notified to entrants as soon as possible by the promoter.

10. The promoter is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this competition.

11. The prizes are as follows: A) Instant Prize - each entrant with the correct answer is entitled to a prize according to the

winner’s choice from the following: USB, Stylus, Yo-Yo, Pen, Cleaning cloth for smartphones or other giveaways at the FONA Dental booth at the FDI Congress in Poznan, Poland between 7-10 September 2016.

B) Daily Prize: The Daily Prize is a Lenovo YOGA tablet 3 10,1“ LTE 16 GB Slate Black Winners of the daily prizes will be chosen from all correct entries by a lottery each day of the FDI Congress at 16:00 at the Promoter's booth. The entrant is eligible to win the Daily Prize regardless of whether or not the entrant previously won the instant prize.

12. The prize is as stated and no cash or other alternatives will be offered. The prizes are not

transferable. Prizes are subject to availability, and FONA Dental s.r.o. reserves the right to substitute any prize with another of equivalent value without giving notice.

13. Winners of FDI Congress Daily Prizes will be chosen by an independent adjudicator or a panel of judges appointed by the Promoter, who will review the entries submitted by the entrants.

14. The winner of FDI Congress Daily Prizes will be notified by email and/or letter within 28 days of the closing date. If the winner cannot be contacted or does not claim the prize within 14 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner.

15. The promoter will notify the winner when and where the prize can be collected / will be delivered.

16. The competition and these terms and conditions will be governed by Slovak law and any disputes will be subject to the exclusive jurisdiction of the courts of Slovakia.

17. The winner agrees to the use of his/her name, as well as their entry/entries. Any personal data relating to the winner or any other entrants will be used solely in accordance with current Slovak data protection legislation and will not be disclosed to a third party without the entrant’s prior consent.
