What is palliative care?Palliative care is an approach to care which aims to relieve suffering and improve the quality of living and dying. Pharmacy• In the Calgary Zone, hospice

patients qualify for the AlbertaBlue Cross Palliative CareDrug Program which providespremium-free drug coveragewith no waiting period.

• The patient is responsible for30 per cent of the cost ofdrugs to a maximum costof $25 per prescription.

The lifetime maximum amountthe patient pays out of pocketis $1,000.

• Items that are not covered include:infusion equipment/supplies,diabetic supplies (excludinginsulin), etc.

Oxygen• Oxygen and oxygen set up are

provided to hospice patientsby O2 vendor agencies.

• Patients are responsible forthe cost of oxygen and oxygensupplies while in hospice.

If the patient was associated withan O2 vendor prior to admission,every effort is made to continuewith that vendor. The hospicewill notify the appropriate O2vendor agency in preparation foradmission prior to transfer.

• Some or all of the costs associatedwith O2 may be covered by AlbertaAids to Daily Living (AADL). Pleaserefer to “O2 funding process foradults” on the AADL website.

What are the costs associated with hospice care?

Other Services• Hospice does not provide

physiotherapy, occupationaltherapy or recreational therapy.

• Patients/families are responsible forcosts and possible adverse effectsof complementary care such asalternative medicines, acupuncture,chiropractic care, massage,physiotherapy, occupationaltherapy etc. (Always check withhospice staff before engaging anyof the above).

• Ambulance transportation costsmay be the responsibility of thepatient if the patient is not beingtransported from or to an acutecare hospital. Please check withyour insurance provider for details.

• Please note, additional costsmay be involved with parking,phone rentals, guest rooms,guest meals, etc. These costswill vary depending on thehospice in question.

TransfersHospice care is highly specialized. In some circumstances patients will stabilize and are then no longer appropriate for the hospice setting. In such situations the patient will be reassessed for transfer to a more suitable location or program where he/she can access physiotherapy, occupational therapy and/or enjoy greater social interaction. These transfers usually are to home or a care facility.

There is no daily accommodation fee charged to the individual or families for hospice care.

Alberta Health Services (AHS) Calgary Zone provides hospice care through six contracted service providers and one AHS Hospice throughout the greater Calgary area.

When remaining at home is no longer possible, or a hospital admission is no longer required, your care provider might suggest hospice care as the most suitable alternative.

Once the decision has been made that hospice would best meet your or your loved ones needs, you will be asked to identify at least three of your preferred hospices from the list on the right. We will do our best to arrange for you to go to one of these preferred hospices.

When a bed becomes available in any of the three hospices indicated, you will be notified and arrangements will be made for transfer as soon as transport can be arranged.

How to access hospice in the Calgary Zone

Please note that once you are on the hospice wait list, transfer may occur very quickly.

PLEASE NOTE:

• If you are in a hospitalyou will be unable to refusea hospice transfer basedon where the hospiceis located.

• When hospitals are overfull, you may be required tomove to a hospice that isnot on your preference list.We do this in order to meetthe health care needs ofall Calgarians.

• There is also no opportunityto transfer from one hospiceto another.

Please speak to the Palliative Care consultant if you have any specific questions or concerns.

• Pharmacy services are provided to individuals in hospice by contracted community pharmacies.

• Individuals who are registered with the Alberta Health Care Insurance Plan are eligible for the Alberta Blue Cross Palliative Coverage Program. This program provides premium-free prescription medication coverage with no waiting period.

• With the Palliative Coverage Program, individuals are responsible for 30% of the cost of drugs to a maximum of $25 per prescription. The lifetime maximum that the individual pays is $1,000.

• Items that are not covered by the Palliative Coverage Program include: infusion equipment/supplies, over the counter medications, diabetic supplies (excluding insulin), and pharmacy dispensing fees.

Hospice care is highly specialized. In some circumstances, an individual’s care needs change and hospice is no longer the most appropriate care setting. In such situations the individual will be reassessed for transfer to a more suitable location or program where he/she can access the right care for their needs. These transfers are usually to home or a care facility (e.g. Long Term Care or Supported Living)

• Oxygen and oxygen equipment are provided to individuals in hospice by home oxygen companies.

• If you or your loved one have previously been using home oxygen, every effort will be made to continue with your same vendor. The hospice will notify the home oxygen company of your move.

• Individuals are responsible for the cost of their oxygen and oxygen supplies while in hospice. Some or all of these costs may be covered by Alberta Aids to Daily Living (AADL). Please refer to “O2 funding process for adults” on the AADL website, or speak to your health care provider for more details.

Smoking • Hospice staff and volunteers cannot accompany individuals to smoke. Accessibility

to designated smoking areas varies by site; some hospices do not allow smoking anywhere on their premises.

When a space becomes available at one of your selected hospices, you will be notified and arrangements will be made for transfer as soon as possible.

Once admitted to a hospice, we do not transfer from one hospice to another.

Alberta Health Services (AHS) Calgary Zone provides hospice care through several contracted organizations at various locations throughout the greater Calgary area.

Once the decision has been made that hospice would best meet your or your loved one’s needs, you will be asked to identify at least three hospices from the list provided.

Identifying more than three hospice sites may help to minimize your wait time.

Please be aware that your clinical requirements may mean that specific hospices are better choices to meet your care needs.

Your Palliative Care provider will assist you in the hospice placement process.
