Grant funds are available to assist with medication costs as payer of last resort. Recovery Support Care coordinators support a team approach to develop a personalized care plan. Peer recovery support services are available throughout the region at no cost. Provider Resources Substance use disorder treatment professionals have access to resources that support their practice, including a health information exchange platform and local trainings. Collaboration Partners throughout the region work together to improve care for those affected by substance abuse via abstinance-based, cultural, and/or medication-assisted treatment (MAT) pathways. Any adult residing in Forest, Iron, Oneida, Price, or Vilas counties or the Forest County Potawatomi, Lac du Flambeau, or Sokaogon Chippewa tribal communities with an opioid and/or methamphetamine use disorder is eligible to participate in the HOPE Consortium. HOPE Consortium partner agencies have provided regional treatment and recovery support services to hundreds of clients since 2015 with a broad impact on families and communities across the service area.

All pathways to recovery, whether abstinence-based, cultural, or medication-assisted, are respected.

HOPE Consortium participants are eligible for: HOPE Consortium partner agencies have access to:

• Care coordination services at no cost• Priority treatment access at partner agencies• Access to grant funds as payor of last resort for medication

and/or uring drug testing

• Local and regional training and technical assistance• Evidence-based patient, family and provider resources,

general and tailored to specific needs• A web-based health information exchange platform
