WHAT IS HOME HEALTH?

The goals of Home Health Care are to promote and maintain health, while providing comprehensive medical care for patients when it is difficult and taxing to leave their home without assistance. We work to prevent re-hospitalization and thus reduce the cost of health care.

Care is provided in the home throughintermittent skilled care visits to homebound patients. Our team supports families so that they can fully participate in the care of the patient in the comfort of the patient’s home. Our expertly skilled medical social workers assist and educate patients and families on available community resources personalized to meet their needs.
