Please read the following information regarding how to complete the attached form. This information will assist you in the details you are required to provide, rating a specific risk and assigning tasks to responsible parties.
IdentificationIdentify the risk categories at your event and the specific risks attributed to each. I.e. amusement equipment and injury caused using amusement equipment, emergency evacuation procedure and risk of fire or people and disorderly conduct.
RatingsConsequences, likelihood and risk ratings are listed and described below relevant to the rating section in the attached form.
Consequences (C)Insignificant: no noticeable effect on project management objectives.Minor: Some difficulties experienced which are easily managed.Moderate: Moderate effects on project objectives requiring management efforts to rectify.Major: Major effects on project objectives, requiring significant effort to rectify.Catastrophic: Unacceptable effect on project objectives.
Likelihood (L)Almost certain: Expected to occur in most circumstances.Likely: Will probably occur in most circumstances.Possible: Might occur at some time.Unlikely: Could occur at some time.Rare: May only occur in exceptional circumstances.
Risk rating (R)This table refers to the risk rating combining the consequences and likelihood ratings above.Consequences ->Likelihood

PreventionConsider the actions that can be taken to reduce the likelihood and consequences of the risk. When completing the prevention section of the attached form, please include: proposed action, resources used, person responsible and time frames.
Responsible partiesEnsure that those persons identified as responsible are notified about their assigned task or tasks. Distribute the risk management plan prior to the event being held so all involved are aware of the risks and measures taken to prevent them.
