• In Thailand, in 2012, cancer death rate per 100,000 for men was 127.8 and for women was 82.6.

CRC in Thailand

• Hospital-based cancer registry 2010 of the National Cancer Institute of Thailand, colorectal cancer accounted for 21.5% of all male cancers and 10.4% of all female cancers.

• Limitation in assess to colorectal cancer screening, majority of CRC cases are advance stage.

• Data from Srinagarind Hospital; CRC patients were Stage IV (35.85%) and stage III was the second most common (12.56%) presented in colon cancer patients at the first diagnosis .

• Rate of receive chemotherapy as schedule 71.7% vs 100%

Incidence of emergency call center

• Most problems are about the side effect of chemotherapy

• 1st analysis • About 10 months : Only 2 cases ( Gr3) that call center

manage by contact attending doctor and transfer patients to ER

• Case 1 :bleeding per colostomy – Root: bevacizumab side effect – Response: Contact ER and consult surgeon

EVALUATION OF QUALITY OF LIFE AND COST OF CARE IN METASTATIC COLORECTAL CANCER

PATIENTS RECEIVING AMBULATORY CHEMOTHERAPY

• An observational cohort 75 patients from Dec 2015 to Nov 2016.

• Nurse coordinators followed up with the patients by phone. The Functional Assessment of Cancer Therapy-General and Colorectal (FACT-G and FACT-C) scale and cost of treatment questionnaire were collected at time of enrolment, 2 months and end of treatment

Results

• 30 patients treated with AC and 45 patients treated with inpatient.

• 65 returned the questionnaire( response rate 86%).

• Intention to treat analysis revealed significantly improved in social well being (p<0.001) in AC group.

• This analysis demonstrated trend of QOL improvement for the FACT-G (p= 0.059).

In 2017, HCRM has received national awards as Thailand Public Service Award: TPSA.

Conclusion

• HCRM changing as thought by “think different in care ”

• HCRM helps Colorectal cancer patients to live normal lives by administer treatment at patients' home and results to significantly improved in quality of life especially in social well being. Moreover, ambulatory chemotherapy (HCRM) reduced cost of chemotherapy treatment.
