Home Care

Aged Care can be confusing for both the customer and their family.

There are a lot of options out there and some people are not sure where to start. The information

in this booklet will provide you with the knowledge you need to make choices about your home

care or your loved one’s care.

We will navigate you through what home care is, the services that can be provided, how do

you access funding, and the steps to get the process underway. We have a friendly Customer Care

What is Home Care?Home care is a range of care and services that help you, or the person you care for, to continue

living at home for as long as possible. Home care can be subsidised by the Commonwealth

Government, depending on your eligibility. Home care services can include:

What are the DifferentTypes of Home CareAvailable in WA?Depending on your needs and preferences, you may

be eligible for government-subsidised services.

These fall into two categories:

Home and Community Care (HACC)HACC is for those who only need a low level of

support, such as cleaning, shopping, participation in

social activities. HACC is subsidised by the State

Government.

To find out if you are eligible for HACC services,

contact us on 1300 26 26 26 and we will help you.

Alternatively, you can talk to My Aged Care on

1800 200 422. A Regional Assessment Team will

then carry out an assessment and decide whether

you are eligible for HACC services.

Packaged CarePackaged care is for those who are likely to need

more home care services as time goes by, as needs

increase.

There are four levels of package from simple home

support to more regular clinical services, such as

nursing and even dementia care. Packaged care is

subsidised by the Commonwealth Government.

Packages are funded by the government on an

approval basis. Package amounts for the different

levels can be seen to the right. There are extra funds

available for dementia clients called a dementia

supplement, your service provider can guide you on

whether you are eligible for this.

You have control over the types of care and services

your receive in your home, how and when they are

delivered and who delivers them. You receive regular

statements so that you can see how much funding is

available for your services and how the money is

being spent.

If you choose Amana Living as your home care

provider, you will have your own Service Coordinator.

A dedicated Amana Living Service Coordinator can:

Your Coordinator will walk alongside you as much as

you like. Some clients like to leave the organisation of

their services to their Coordinator after agreeing on

what’s required. Others want to have a greater level of

control. The choice is yours.

Who is Eligible?Home and Community Care (HACC)To find out if you are eligible for Home and Community Care (HACC) services, contact us on

1300 26 26 26 and we will help you. Alternatively, you can talk to the Regional Assessment Services

on 1300 785 415.

Home Care PackagesEligibility for Packaged Care is determined by an Aged Care Assessment Team (ACAT). We can help

you at every step of the process, so call us at any time for assistance. Assessment works in the

following way:

• Your GP can refer you to an ACAT or, if you are in hospital, the nursing or medical team there willtake care of your referral.

• The ACAT representative will call and make a time to meet with you at home or in hospital, wherethey will carry out a comprehensive assessment.

• After this, the ACAT will inform you of the level of care chosen for you as a result of theassessment.

• A letter will then be mailed to you confirming that your package of care, and giving you a referralcode.

• Once you have received this letter, be sure to call Amana Living to discuss the services you wouldlike to receive. We operate across the Perth metro area and in Kalgoorlie-Boulder.

What to Consider with Choosing a Home Care Provider

1. StaffNavigating aged care can be difficult, so ask the service provider if you will get a dedicated Care

Coordinator who will spend time with you to understand your needs.

2. CostThere are a few vital questions to ask to help you understand what you’re being charged for,

including:

3. Service Range and RestrictionsA provider who has a wide range of services, without any restrictions on what you can buy and from

who, will offer greater value longer term. There’s greater flexibility and means the support they

provide can respond to your changing needs. Consider whether you will want to use the same

provider if you ever have to go to a nursing home. If so, choose a provider that also has nursing home

facilities.

4. PurposeFinally, what’s the passion or purpose of the organisation and does it align with your own beliefs

and goals for the future?

• What is the hourly rate for services?

• Are services charged in 15, 30 or 60 minute blocks?

• Is there an additional cost for weekend or evenings?

• Is there a charge for extras beyond the hourly rate, for example travel?

• What is the cost for specialised services?

• Are there entry or exit fees?

• Will I be charged an administration fee and what does it cover?

• Will I get a monthly statement? Providers should give you a reconciliation of what has been spent and how much of your budget is left.

• Can I top up my own package?

• How long has the provider been in business?

• What training and qualifications do your staff have?

How to get started

Value Interim PackageHave you recently been assessed by the ACAT team

for home care but are still waiting on your

confirmation letter?

Unfortunately there can be a delay of a few months

between an ACAT assessment and the Commonwealth

Government issuing a home care package.

Amana Living has an excellent offer to help you in the

interim - for $199 you can buy 5 hours of home care.

This will include a visit from a Coordinator to explain

the services available and 5 hours of care from a choice

of these services: Domestic Assistance, Personal Care,

Respite and Social Support.

These services are offered between 7am-5pm, Monday

to Friday. You can purchase as many blocks as you like

or if you want to purchase additional single hours they

cost $57.60 per hour.

Who is Amana Living?Amana Living is one of WA’s largest not-for-profit providers of care, accommodation and support

for older people. Amana Living has been operating for over 55 years, and was previously known

as Anglican Homes.

The Amana Living business encompasses:

• 13 nursing homes providing accommodation and support to 752 people, including 2 specialist nursing homes dedicated to providing care and support to people living with dementia

• 17 retirement villages

• 6 day clubs in Osborne Park, Kinross, Alexander Heights, Quinns Rock, Wanneroo and Bull Creek, providing activities and support for older people. This service also provides much-needed respite for carers

• 2 Transition Care Centres, providing people transitioning from hospital back to their home or residential care

• Delivery of care to 1,125 people living in their own home

• 1,634 staff dedicated to providing high quality care and services

• 174 volunteers delivered 10,924 volunteered hours in 2016, over a broad range of enrichment activities including arts and crafts, social support, outings, Men’s Sheds, pastoral care, and worship support.
