RESOLUTION NO. GP-0014-12-13 SUBJECT: APPROVAL OF SBYSP SUMMER PROGRAM AT HOBOKEN HIGH SCHOOL RESOLVED, that the Board of Education, upon recommendation of the Superintendent, approves the SBYSP Summer Program at Hoboken High School. " Camp Redwings 2013" is a summer program which is required by SBYSP contract between the Hoboken Board of Education and NJ State Division of Children and Family Services. The entire program will run for six weeks beginning on July 1, 2013. The program will provide students with a variety of learning support, healthy youth development, recreational activities and a student orientation component. The program will take place at Hoboken High School and include a variety of approved field trips pending renewal of the grant. Salaries from #20-436-200-100-13-1027-000. RESOLUTION NO. GP-0016-12-13 SUBJECT: APPROVAL OF MOCK CAR DISPLAY AT HOBOKEN HIGH SCHOOL RESOLVED, that the Board of Education, upon recommendation of the Superintendent, approves Hoboken High School to display a crashed vehicle outside of Hoboken High School from May 28, 2013 until May 30, 2013. The purpose of this display is to demonstrate to our students the dangers of drinking and driving. The Hoboken Police Department will have police caution tape around the display for safety precautions. In conjunction with this Ms. Maria Estevez (Mothers Against Drunk Driving) will speak to 12th graders in a one hour assembly on May 29, 2013.

GP-0017-12-13 SUBJECT: ADDITION TO PREVIOUSLY APPROVED JOB TITLE RESOLVED, that the Board of Education, upon recommendation of the Superintendent, approves the following job title change for the 2013-2014 school year: Old Title: Supervisor, Student Support Services New Title: Supervisor, Student Support Services and Junior High Programs which will include the oversight of Junior High teachers, students and programs RESOLUTION NO. GP-0018-12-13 SUBJECT: APPROVAL OF THOMAS G. CONNORS ELEMENTARY SCHOOL SUMMER PROGRAM 2013 RESOLVED, that the Board of Education, upon recommendation of the Superintendent approves Connors Summer Program starting July 8, 2013 through August 2, 2013. Tentative schedule is from 8:30 a.m. - 12:30 p.m. The academic program will be open to all rising 3rd through 6th grade student, including special education in Connors Elementary School. To confirm that Hoboken Public School District has a 2013-2014 School Choice seat available at the third grade level which we are willing to offer to said parent's child, Jersey City resident JP, knowing that we will not receive any Choice funding for that student for the 2013-2014 school year due to family circumstances. GP-0021-12-13 SUBJECT: APPROVAL OF PART-TIME AIDES FOR THE 2013-2014 SCHOOL YEAR RESOLVED, that the Board of Education, upon recommendation of the Superintendent, approves the employment of aides, on a part-time basis for no more than 5 hours and 45 minutes per day excluding lunch period for which the high school is 40 minutes and the elementary school is 43 minutes for the 2013-2014 school year.

RESOLUTION NO. GP-0023-12-13 SUBJECT: APPROVAL OF WORKSHOPS RESOLVED, that the Board of Education, upon recommendation of the Superintendent, approves the following workshops: WHEREAS, N.J.A.C. 6A:23B-1.1 et. seq. requires that each board of education adopt a formal policy and procedures relating to travel and expense reimbursement for its employees and board members; and WHEREAS, the School District of the Hoboken Board of Education (the “Board”) adopted a Travel Expense Reimbursement Policy that addresses the reimbursement of travel-related expenses by Board members and employees of the District; and WHEREAS, the Board has considered all other relevant guidelines and circulars associated with the adoption of its Travel Expense Reimbursement Policy; and WHEREAS, the Board has determined that the travel listed in this Resolution is educationally necessary and fiscally prudent; and WHEREAS, the Board has concluded that the travel and expense reimbursement listed in this Resolution are directly related to and within the scope of the employee’s current responsibilities; and WHEREAS, the Board has determined that the travel and expense reimbursements listed in this Resolution are for travel that promotes the delivery of instruction or furthers the efficient operation of the school district; and WHEREAS, the Board has concluded that the travel and expense reimbursement listed in this Resolution are in compliance with State travel reimbursement guidelines as established by the Department of Treasury and incorporated herein; THEREFORE, BE IT RESOLVED, pursuant to N.J.A.C. 6A:23B and Board Policy and upon the recommendation of the Superintendent, that the School District of the Hoboken Board of Education, in the County of Hudson, New Jersey, approves travel requests as listed.

RESOLUTION NO. FI-0025-12-13 SUBJECT: APPROVAL OF MEMBERSHIP IN THE NEW JERSEY STATE

INTERSCHOLASTIC ATHLETIC ASSOCIATION RESOLVED, that the Hoboken Board of Education approves membership in the New Jersey State Interscholastic Athletic Association (NJSIAA). The annual dues for 2013-2014 are $2,150.00.

HOBOKEN BOARD OF EDUCATION MEMBERSHIP IN NJSIAA

RESOLVED, that the Hoboken Board of Education, in the County of Hudson, New Jersey, as provided for in Chapter 172 Laws 1979 (N.J.S.A. 18A:11-3 et seq. and Policy 2431) herewith enrolls Hoboken High School as a member of the New Jersey State Interscholastic Athletic Association to participate in the approved interschool athletic program sponsored by the NJSIAA; and BE IT FURTHER RESOLVED, that this resolution will continue to be in effect until or unless rescinded by the Board of Education and shall be included among those policies adopted by the Board. Pursuant to N.J.S.A. 18A:11-3, in adopting this resolution the Board of Education adopts as its own policy and agrees to be governed by the constitution By-Laws and Rules and Regulations of the NJSIAA. RESOLUTION NO. FI-0026-12-13 SUBJECT: APPROVAL OF TRAVEL RESOLUTION FOR THE 2013-2014 SCHOOL YEAR WHEREAS, the Hoboken Board of Education is required pursuant to N.J.S.A. 18A:11-12 to adopt policy and approve travel expenditures by district employees and board members using local, State, or Federal funds to ensure that travel is educationally necessary and fiscally prudent, and WHEREAS, the Hoboken Board of Education has determined that the training and informational programs submitted for board approval are directly related to and within the scope of district employees’ and board members’ duties, and WHEREAS, the Board of Education has determined that the training and informational programs submitted for board approval are directly related to and within the scope of the traveler’s current responsibilities and the board’s professional development plan, and WHEREAS, the Board of Education has determined that participation in training and informational programs requires school district travel expenditures and that this travel is critical to the instructional needs of the district and/or furthers the efficient operation of the district, and WHEREAS, the Board of Education has determined that the school district travel expenditures to these programs are in compliance with State travel guidelines of $.31 per mile reimbursement as established by the Department of the Treasury and within the guidelines established by the federal Office of Management and Budget; except as superseded by conflicting provision of Title 18A of the New Jersey Statutes, and

WHEREAS, the Board of Education has determined that participation in training and informational programs are in compliance with the district policy on travel, THEREFORE BE IT RESOLVED that the Hoboken Board of Education hereby approves the attendance of the listed district employees and school board members at these regularly occurring, no cost, training and informational programs per Policy 9250 Regular Business Travel Authorization and Approval

Regular business travel such as NJDOE meetings and association events, is authorized by the board not to exceed $1,500 per employee, pursuant to N.J.A.C. 6A:23A-7.3(b). Approval by the Superintendent or designee is required, including justification for the travel. Regular business travel is authorized for regularly scheduled in-state professional development activities for which the registration fee does not exceed $150 per employee or board member. WHEREAS, the Board of Education hereby approves the attendance of the listed number of school board members and district employees at any New Jersey School Boards Association and Hudson County School Boards Association training and informational programs, and the costs of attendance including all registration fees, and statutorily authorized travel expenditures, provided that such expenditures are within the annual maximum travel expenditure amount listed above; and, BE IT FURTHER RESOLVED, that the Board of Education authorizes in advance, as required by statute, attendance at regularly occurring and state mandated training programs and informational events as per the listing below:

FI-0028-12-13 SUBJECT: APPROVAL OF COLLECTION AND MAINTENANCE OF PUPIL RECORDS RESOLVED, that the Hoboken Board of Education authorizes certified school personnel to collect and maintain the following mandated pupil records as per N.J.A.C. 6A:32-7.3 and Policy 5125: a. The student’s name, address, telephone number, date of birth, name of parent(s), gender, citizenship,

RESOLUTION NO. FI-0030-12-13 SUBJECT: APPROVAL OF PROCUREMENT OF GOODS AND SERVICES THROUGH

COOPERATIVE PRICING AGREEMENT RESOLVED that the Board of Education approves the following resolution regarding Procurement of Good and Services through Cooperative Pricing Agreement: WHEREAS, N.J.S.A. 40A:11-11(5) authorizes contracting units to enter into Cooperative Pricing Agreements; and WHEREAS, the Board, on May 14, 2013 duly adopted resolution to enter into a Cooperative Pricing Agreement through Educational Data Services, Inc. with Educational Services Commission of Morris County as Lead Agency; and WHEREAS, the Hoboken School District has the need, on a timely basis, to procure goods and services utilizing Cooperative Pricing Agreements, and WHEREAS, the Hoboken Board of Education desire to authorize its purchasing agent to make any and all purchases necessary to meet the needs of the school District throughout the 2013-2014 school year. NOW THEREFORE, BE IT RESOLVED, that the Hoboken Board of Education does hereby authorize the District purchasing agent to make purchases of goods and services entered into this Cooperative Pricing Agreement through Educational Data Services, Inc., with ESC of Morris County as Lead Agency utilizing various vendors as per the attached lists. RESOLUTION NO. FI-0031-12-13 SUBJECT: APPROVAL OF PARTICIPATION IN JOINT TRANSPORTATION

AGREEMENT WITH SUSSEX COUNTY REGIONAL COOPERATIVE RESOLVED, that the Hoboken Board of Education upon the recommendation of the School Business Administrator participate in a Joint Transportation Agreement with the Sussex County Regional Cooperative. RESOLUTION NO. FI-0032-12-13 SUBJECT: WAIVER OF COVERAGE FOR THE 2013-2014 SCHOOL YEAR

WAIVER OF COVERAGE STATE HEALTH BENEFITS PROGRAM

WHEREAS, employees are now permitted to waive their SHBP medical and prescription coverage – provided they have other health care coverage, and

WHEREAS, a State Health Benefits Program Coverage Waiver/Reinstatement Form and Active Employee Health Benefits Application must be submitted through the Human Resources Office to the SHBP in order to waive SHBP medical and prescription coverage, and WHEREAS, to reinstate coverage under the SHBP, an employee must once again complete a State Health Benefits Program Coverage Waiver/Reinstatement Form and Active Employee Health Benefits Application, and WHEREAS, the employee must notify the SHBP within 30 days of the loss of the other coverage and provide proof of loss of that coverage. And WHEREAS, reinstatement will be effective immediately following the loss of the employee’s other health plan coverage, NOW THEREFORE BE IT RESOLVED that the Hoboken Board of Education offers the opt out plan to all active eligible employees, and BE IT FURTHER RESOLVED, those active eligible employees who are eligible for other health care coverage, and waive coverage with the Hoboken Board of Education, will receive an incentive payment of either 25% of the amount saved by the employer or $5,000, whichever is less, and BE IT FURTHER RESOLVED, the incentive payment will be made twice a year on the closest pay date to December 15 and June 15 each year. The decision of the Hoboken Board of Education to allow its employees to waive coverage, and the amount of incentive to be paid, cannot be subject to the collective bargaining process. RESOLUTION NO.
