High Reliability Organisations — HRO’s

High Reliability Organisations (HRO) are organisations that operate in complex, high—hazard domains for extended

periods without serious or catastrophic failures. When we analyse their organisational behaviour, we see very specific

and distinguishable traits and modes of operation which make them very different.

Preoccupation With Failures Rather Than Successes

HRO’s know very well that silence does not equal safety and that the key to success is through specific culture of re-

porting and constant ‘hunting’ for key signs of the upcoming problems. They are acutely ‘risk aware’, which forms

their specific operational culture and shapes decision making practices.

Reluctance to Simplify

HRO’s are complex by definition and they accept and embrace their complexity. They do not explain away problems,

instead they have key risk management experts, measure correct upstream performance indicators and analyse complex

data for key signals. HRO’s have deeply rooted culture of transparency and ability to constructively criticise themselves

and in turn enable organisational learning necessary for its survival, high reliability and extraordinary low incident re-

lated losses

Sensitivity to Operations

HRO’s understand that the best picture of the upcoming trouble comes from the front line and invest heavily in people,

systems and processes aimed to ensure timely detection of issues. HRO’s have unique lines of reporting, accountability

and extraordinary leadership.

Commitment to Resilience

Resilience in HRO’s means the ability to know the risks, map and test critical controls, anticipate and know trouble

spots and ensure key redundancies are in place to regain control when it is lost or is about to be lost. HRO’s hold a dif-

ferent view of people which makes them highly capable of minimising occurrences of human errors, especially those

resulting in large consequences.

Deference to Expertise

Expertise, rather than authority, takes precedence in a HRO. When conditions are high-risk and circumstances change

rapidly, on-the-ground subject matter experts are essential for urgent situational assessment and response.

In many mainstream industries today, there are perceptions that high reliability organising (HRO) is something reserved

for nuclear, aviation, military or other similar ‘ultra – high’ risk environments. There are beliefs that becoming or even

embarking on this journey requires high expenditure, extreme difficulty and great safety record.

This is simply not the case. Every organisation regardless of the industry or cultural stage can embark on the HRO jour-

ney and benefit from the lessons learned from HRO's, in many cases almost immediately, under right guidance and sup-

port. High reliability organising incorporates innovative approaches in management of safety, risks, and key operational

processes, to deliver a new edge in high business performance. Start your journey today with RiskWise Solutions.

Principles of HRO Organising, Unlearning and Reframing Existing Paradigms

Course Duration - 8 hours (Day one)

Course Content:

Learning about key HRO Principles

First steps to becoming a HRO—identification of key risk signals and creating a ‘preoccupation with failure’ mind-

set

Learning beyond incident related data, resisting oversimplifications which impede organisational progress towards

HRO state

Creating a culture of sensitivity to operations

Reframing our understanding of people and perceptions within the HRO organisational context

Course Objectives and Outcomes:

Oversight of the past and current paradigms in managing safety, risk and reliability and introduction to new, practi-

cal and critical ways of managing these critical organisational inputs and outputs to candidates as a pragmatic alter-

native for improving business performance

Analysing current states in proactive management of risk and introduction of practical warning systems and pro-

cesses to create overall organisational preoccupation with failures and early detection of upcoming events carrying

large consequences

Analysing current organisational approaches creating oversimplification and damage, candidates are taken on a

journey towards challenging the ‘way we have always done’ the business and offered a range of different strategies

in resolving over complexity but at the same time not falling into the trap of oversimplifying and loosing the key

value from systems and processes

Candidates will examine new approaches in maintaining operational sensitivity to operations and modern, science

based outlook on the role of humans and management of human error

Creating Effective and Mindful Leadership, Engineering Resilience

Through Pragmatic HRO Strategies

Course Duration - 8 hours (Day two)

Course Content:

Understanding how to build mindful and committed leadership, capable of growing trust, credibility and engage-

ment to create organisational resilience

Exploring the fine balance between HRO systems complexity and simplification in ensuring safety systems and

processes suit risk profile of the organisation

Evaluating and creating understanding of operational excellence, operational discipline and engineering resilience

in practice

Course Objectives and Outcomes:

Candidates will discuss and learn how to create a resilient organisation through mindful leadership and eight dimen-

sion of trust

Group will specifically learn key leadership techniques typically distinguishing leadership from management and

creating mindful leadership, specifically how to motivate, inspire and influence others towards creating resilient

teams

Team will learn why there are such large benefits in creating a culture of ‘deference to expertise’ in decision mak-

ing, including the art of ‘mindful enquiries’

Candidates will review past issues with paper based safety management system and learn how SMS is applied in the

HRO setting, as well as how to apply practical operational strategies to bridge the gap between traditional opera-

tional excellence and HRO

Through sharing and mutual discussions, learning teams will create detailed understanding about how to create re-

silience in practice, including application of defences in depth and the concept of multiple redundancies. Teams will

develop HRO action plans outlining some of the newly learned strategies applicable to their own organisations
