Healing potential of stem cellsStem cells starting small... No matter how big a human becomes, it all began with 2 single cells: an ovum and a sperm cell. This means that cells exist, which have the potential to form a complete human being. After a few days in the womb, the blastocyst forms. If body tissue is dama-ged, stem cells hunt for the damaged area and advance the process of healing. Also, day-to-day processes in the human body rely on stem cells: our erythrocytes only live for about 120 days, by which time they have become too old, cannot transport enough oxygen and have to be replaced. This task is taken over by the hematopoietic stem cells, which can be found in the bone marrow.

1. Collection, Transportation, Processing, Storage, Claim and Discontinuation:

1.1 C4H will provide you a collection kit, transportation box and accompanying instructions.1.2 After notification by telephone, C4H will arrange for the pick-up and transportation of the sample by a specialized courier to be processed after delivery within 48 hours in our laboratory.1.3 C4H will isolate in her dedicated laboratory in the UK stem cells from the cord blood/ tissue as sent by undersigned.C4H will store those cells in case sufficient stem cells can be isolated for the period as has been agreed in 1.6.1.4 You explicitly agree that 5 milliliters of blood will be withdrawn from the mother just before or immediately after the delivery for testing of HIV, Syphilis, CMV, Hepatitis B and C. The results will be kept confidentially.1.5 The blood tests required are undertaken only for the purpose of assessing the suitability of the samples for storage.1.6 The stem cells will be stored in a professional way for twenty-five years.1.7 The stem cells remain the property of the child.1.8 C4H may retain the corresponding data in their electronic database. 1.9 C4H will otherwise retain the data strictly confidential. After first written request from the legal representative(s) or the child, older than 18 years, the data will be provided or be destroyed.1.10 Upon written request, signed by owner or legal representative(s), the data will be sent immediately to your treating physician together with all data in possession of C4H.1.11 Upon written request, signed by the owner or legal representative(s), the stem cells will be destroyed by C4H.1.12 At no times your sample will be used for other purposes.1.13 C4H will inform 6 months before termination of the agreed storage period, the undersigned, the legal representative(s) or child if older than 18 years, for possible extension of the contract. For an extension of the contract C4H will invoice you for the costs of storage applicable at that time.1.14 C4H may at their own discretion change the location of storage.1.15 C4H will provide you with a certificate of storage.

2. Costs

2.1 The amount of 2,000 AED is payable upon signing the agreement.2.2 The remaining amount of 8,000 AED is payable after successful storage. Optional HealthTissue: The additional amount of 2,800 AED is payable after successful storage.

2.3 In case you haven´t settled the outstanding balance after having received repeated sreminders, C4H will be authorized to destroy your sample.2.4 In case it would appear that your sample has not enough stem cells, the sample will not be stored. In that case you will not receive further invoices. However we cannot restitute the pre-payment of 2,000 AED.

3. Liability

3.1 C4H will not be liable in case cord blood could not be collected in a sufficient amount, or the cord blood would not contain a sufficient number of stem cells.3.2 C4H could be hold liable for a maximum of the amount paid to C4H.3.3 Liabilities of third parties selected by C4H are excluded, except of gross neglectance in selection of those third parties. In this case article 4.1 will be applicable for liability.3.4 C4H does not guarantee against any possible losses due to natural disasters or terrorist activity, which cause destruction or deterioration to the sample.3.5 C4H is not responsible for misuse of the collection set or any medical care until transportation of the sample.3.6 The hospital and its personnel will not be liable when due to unforeseen circumstances, the collection of cord blood is not possible, or the amount of cord blood taken proves to be insufficient.

In case the samples do not fulfill the HTA criteria, you will be notified so you can decide whether to continue with the storage or not.

4. Notice

4.1 This Agreement shall be governed by and construed in accordance with Swiss law.4.2 The undersigned legal representative(s)/ parent(s) will immediately notify C4H in writing of any changes in the information provided below. All notifications shall be sent to C4H Baarerstrasse 112,6302 Zug, Switzerland or info@cells4health.com.
