MEET the TEAMMaintaining strong ties to our members and the community at large is essential to living the Passport Health Plan mission. Our community engagement team works side-by-side with you to reach out to groups and organizations, forging relationships, raising awareness, and educating Passport members about how to get the most from their plan benefits. Here are some of the services we offer:
• Trainings for you and your staff
• Presentations in the community for members, school, civic and community groups, about Passport Health Plan benefits, special programs and covered services.
• Member education sessions and materials
• Participation and/or collaboration on community events
• Addressing disparities in Health Care (The Equity, Diversity and Inclusion Program promotes and fosters Passport’s mission both internally and externally by identifying opportunities that improve the health of underserved groups, members with communication needs and the disabled.)
• Assistance with social and community resources
• Health education through our LIFE (Lifestyle Improvements for Everyone!) Program. We offer free presentations to any community group, church, organization, or school. Our health educators use interactive presentations and health manipulatives (props that provide a visual learning experience) to help educate our members and the public on a variety of health topics like condition management, nutrition, exercise, sleep, e-cigarette/vaping, tobacco cessation, QPR suicide gatekeeper training, life skills, bullying prevention, substance use, and behavioral health.
