MESSAGE FROM THE CHAIR

It has been a pleasure to serve as volunteer chair of the Pacific Region Board of Directors for the past two years. The years have flown by, with many fond memories and outstanding accomplishments to share. I met so many adults and children with arthritis—the very important “Faces of Arthritis” we serve. Wonderful people like 12-year-old Rhiannon Carter and her mom, Katy, who initiated and starred in a groundbreaking juvenile arthritis awareness piece on the national TV program, “The Doctors.” I am touched by their story and inspired by their courage. Multiply their story by millions, and you can truly understand the urgency of our cause. We need to stop the pain and find a cure NOW!

I also had the honor of working with volunteer leaders and national staff from across the country and quickly understood the nationwide scope and importance of our work. I saw first hand that the Arthritis Foundation continues to make an impact on the lives of people with arthritis of all ages. I learned how much people depend on the Arthritis Foundation for information and resources to improve their lives, as well as the vital role it plays in connecting people to the broader arthritis community. The community continues to grow, as many talented and dedicated volunteers are joining us in the fight for a cure. Designation of the Arthritis Foundation, Pacific Region, as a Charity Navigator 4-star charity was a personal highlight for me. The 4-star rating is the highest rating awarded to a nonprofit organization. You can be confident that the Foundation carefully stewards your generous donations to advance our mission. Momentum was built on many fronts during the past two years, and we are well poised to achieve even greater success in 2015 and beyond. Your valued support will continue to bring HOPE to each and every adult and child with arthritis as we move towards a cure. With thanks and deep appreciation, Peter MainstainChair, Pacific Region Board of Directors

MESSAGE FROM THE CEO

As we approach the end of the year, our thoughts turn to the holidays and the joy and rewards of giving. We are truly grateful for your generous support, which has positively changed the lives of millions of adults and children with arthritis. Other people within corporations are advancing our mission right alongside with you during the year. Our sponsors’ support of our mission is a reflection of their corporate culture and humanitarian values. Here is a wonderful example from the California Coast Classic Bicycle Tour, our biggest fundraising event of the year. The Northern California Division of Safeway, Inc. led by Karl Schroeder, provided food and other vital support for the tour. Notably, Team Safeway led by example. Karl and

his team, including Public and Government Affairs Director, Keith Turner, rode the entire 520-mile course, fueled by stories of courageous youth honorees that they met along the way. The story of sixteen-year-old Caitlin Ryan touched their hearts. Caitlin had both hips replaced by the age of twelve due to her arthritis. For years, Caitlin dreamed of cycling with her dad and brothers, but was unable to ride a traditional bike. During the Bike Tour finish line festivities, Caitlin tested the outdoor elliptical bike (ElliptiGO) used by several riders with arthritis. She fell in love with it! Her joy was apparent to members of ElliptiGO and Team Safeway, who surprised her with her own ElliptiGO bike, the gift of her dreams! Due to the extraordinary generosity and kindness of ElliptiGO and Safeway, Inc., Caitlin hopes to someday accompany her dad on the California Coast Classic Bicycle Tour. Stories like these inspire us and fill our hearts with joy and gratitude all year long. Thanks to each one of you for helping adults and children like Caitlin realize their dreams for a better life as we march together towards a world free of arthritis pain. I wish you and yours a warm and wonderful Thanksgiving.

Manuel Loya CEO, Pacific Region

RESEARCH FOR A CURE

THE FACE OF ARTHRITIS RESEARCH:GARY S. FIRESTEIN, MD

Congratulations to Gary S. Firestein, MD, one of this year’s National Innovative Research Grant recipients from the Pacific Region. Dr. Firestein is a recognized international authority in the field of rheuma-toid arthritis research. This distinguished investigator, author and teacher currently serves as director of the Clinical and Translational Research Institute, dean and associate vice chancellor of Translational Medicine, and professor of medicine at the UC San Diego School of Medicine. Dr. Firestein joined the faculty as an assistant professor of medicine in 1988, and led the Division of Rheumatology, Allergy and Immunology at UC San

Diego from 1998 until 2010. He has served in leadership roles for governmental agencies (Food and Drug Administration and the National Institute of Arthritis and Musculoskeletal and Skin Diseases) as well as professional organizations such as the American College of Rheumatology. In the 1990s, Dr. Firestein’s groundbreaking research on cytokines contributed to the development of biologic response modifiers to treat rheumatoid arthritis. His current research explores how certain enzymes within cells control genes involved in inflammation. The research will expand our understanding of the fundamental mechanisms of disease in RA, leading to the development of new therapies and identification of diagnostic biomarkers.

Dr. Firestein’s many achievements have been recognized by the Arthritis Foundation. In 2006, he received the National Arthritis Foundation Lee C. Howley Sr. Prize for Arthritis Research. Dr. Firestein also received the Arthritis Foundation, Pacific Region’s prestigious Jane Wyman Humanitarian Award in 2010 for his many contributions to rheumatology. In addition to pursuing his research this year, Dr. Firestein has graciously volunteered to serve as Research Honoree for the Jingle Bell Run/Walk for Arthritis in San Diego. When not working in the laboratory, Dr. Firestein is an avid surfer and publishes a cooking blog with his wife, Linda Firestein (cookingwithlarue.blogspot.com).

Thank you, Dr. Firestein, for advancing our mission through innovative research that is moving us ever closer to a cure!

NATIONAL NEWS:SCIENCE DEPARTMENT POISED FOR BREAKTHROUGHS IN 2015

Since 1948, the Arthritis Foundation has initiated and supported scientific discoveries that have improved health outcomes for people with arthritis.

Under the leadership of National Scientific Director, Amanda Niskar, DrPH, MPH, BSN, the Arthritis Foundation’s Science Department is pro-viding scientific direction to improve everyday life for people with arthritis and related diseases while finding a cure. Dr. Niskar’s goal? “I want to take the science off the shelf and translate it into high-impact results that make a positive difference for people with arthritis.”

Here are key strategies that will drive the scientific direction for the Foundation in 2015 and beyond:

• Identify and prioritize scientific challenge areas where proactive effort can remove barriers and/or catalyze the development of new or improved approaches for prevention, diagnosis, treatment, and cure

• Validate scientific methods and content for position statements, guidelines, and other products • Accelerate the process of scientific discovery and translation all the way to the point of care

Visit arthritis.org/research/ to learn more about the Arthritis Foundation’s scientific contributions and exciting new research developments.

ADVOCACY IN ACTION

RAISING OUR VOICES AT THE 2015 NATIONAL ADVOCACY SUMMIT The 2015 National Advocacy Summit will be held on March 23-24 in Washington, DC. Volunteers from across the United States will meet with legislators and staff on Capitol Hill to educate them about the unacceptable pain, disability and economic impact of arthritis.

For more information about the Advocacy Summit, advocacy priorities, and to register to become an e-advocate, visit arthritis.org/advocacy or contact Krystin Herr at [email protected].

MAKING OUR VOICES HEARD WITH STATE LAWMAKERS IN 2015 More than 13,000 advocates in the Pacific Region will rally together in 2015 to address the following issues with state lawmakers on behalf of millions adults and children with arthritis:

Specialty Tiers: Caps on Drug Co-Payments Create Barrier to Medication Access People with arthritis need legislation to cap the out-of-pocket monthly cost for needed “specialty tier” medications (mostly biologics) which require that patients pay a percentage of the drug cost rather than a fixed co-payment. Passage of legislation will reduce the financial burden on individuals and families with arthritis and prevent negative health outcomes resulting from lack of access to needed medications.

Access to BiosimilarsBiologic medications have been extremely successful at helping people with arthritis and other chronic conditions. Legislation will be required to allow a pathway for substitution of biologic medications with biosimilars, a new class of medications that are nearly identical derivatives of original biologics at a lower cost. Uniform Prior AuthorizationHealth insurance plans require that physicians complete a prior authorization form when the provider prescribes a medicine or treat-ment not covered by the plan or insurer’s formulary. Legislation would mandate that a standardized authorization form be used for all insurers and that a response be available to the provider within a 48 hour period.

THE POWER OF PARTNERSHIPS This year, the Arthritis Foundation enhanced the quality of life of hundreds of thousands of people with arthritis through collaborative partnerships with governmental agencies, health care organizations and corporations.

The Arthritis Foundation, Pacific Region partnered with five University of California campuses to offer the Walk With Ease program to almost 300 campus faculty and employees. A National Arthritis Foundation grant funded the pilot program, and the California Arthritis Partnership Program (CAPP) provided technical support to facilitate its implementation. Overall, more than 90% of participants reported that the program increased their daily physical activity and were satisfied with the program.

Thank you to the University of California and many other partners who have worked to expand the reach of Arthritis Foundation programs this year:

YOUTH PROGRAMS

SUMMER CAMPS CHANGE LIVES

This summer, nearly 300 children and teens with arthritis or related diseases from Hawaii to Arizona celebrated an unforgettable outdoors experience at six summer camps. Each camp offered a safe, nurturing and medically-supervised environment where campers participated in FUN and exciting outdoors activities while building self-esteem, independence, friendships, and hope for a lifetime.

For more information or to donate to Pacific Region youth programs, please contact Allie Avishai ([email protected]) or Jennifer Ziegler ([email protected]).

JA SYMPOSIUM: A FLAVORFUL CONNECTION FOR JA FAMILIES

Parents and children with juvenile arthritis enjoyed the region’s inaugural Juvenile Arthritis Symposium in August at Point Loma Nazarene University in San Diego. The one-day symposium provided exciting educational and networking opportunities for JA families. Parents learned about topics ranging from JA 101 to managing their child’s transition to young adult-hood. One parent said, “At the JA Symposium, I learned that being positive will help you face whatever challenge may be in front of you.”

During the day, children with juvenile arthritis and their siblings enjoyed a separate track of fun activities, including learning how to make healthy snacks. After an inspirational keynote address from Monet Diamante, attendees gathered for a flavorful ice cream social.

Many thanks to the volunteer symposium committee for making the first JA Symposium a fun and life-changing event for all.

YOUNG ADULT PROGRAMS

YOUNG ADULTS WITH ARTHRITIS THRIVE AT “WELLNESS REINVENTED: LIVING BETTER WITH ARTHRITIS” RETREAT Two-thirds of people with arthritis are younger than 65, yet very little programming targets young adults living with arthritis and other rheumatic diseases such as lupus, ankylosing spondylitis and fibromyalgia. To address the unique needs of young adults with rheumatic diseases, the Arthritis Foundation sponsored “Wellness Reinvented: Living Better with Arthritis”, a special one-day retreat on World Arthritis Day in Burbank, CA.

Attendees learned about the mind, body and spirit connection of living well with arthritis in tailored educational sessions led by health care and subject matter experts. Topics ranged from “Nutrition Ammunition” and “Mindfulness Tips” to “Yoga on the Lawn”. Participants also enjoyed connecting with others during social networking opportunities offered before and throughout the retreat.

Many thanks to the dedicated Arthritis Foundation volunteers and partner organizations (Arthritis Introspective, Spondylitis Association of America, Arthritis National Research Foundation and the USC Division of Occupational Science and Occupational Therapy) for making “Wellness Reinvented” a model of excellence for young adults with rheumatic diseases.

LIVING BETTER WITH ARTHRITIS

page 10

ASK THE EXPERT: E. ROBERT HARRIS, MD

RHEUMATOID ARTHRITIS What are the symptoms of the beginning stages of Rheumatoid Arthritis (RA)?Early signs of RA include early morning stiffness and swelling in the small joints of the hands and feet, and occasionally in the wrists and ankles. Early on, the pain and stiffness may migrate from joints on one side of the body to the other; however, after 6-8 weeks or so, the stiffness and swelling can affect both hands and wrists. Concurrently, there may be occasional fever, rash and fatigue, with an overall feeling of weakness. When should a patient seek medical care? A patient should consult with a physician if early morning joint stiffness and swelling of the small joints

of the hands and feet persist for a period of 2-3 weeks, and then continue intermittently without signs of improvement over an 8-10 week period from the onset of symptoms. What are typical treatments for RA? The faster the condition is treated, the better the outcome. Within a period of 3-6 months, irreversible joint damage may occur, resulting in disability and absence from work. Typically, nonsteroidal anti-inflammatory drugs, or NSAIDS (ibuprofen, naproxen or aspirin) are used to treat the early symptoms of inflammatory arthritis. If pain, swelling and stiffness do not decrease after 10-12 weeks from the onset of symptoms, with maximum doses of the NSAIDs, patients should visit a rheumatologist to develop a personalized treatment plan. Typically, disease-modifying anti-rheumatic drugs (DMARDs) are prescribed as the next step to treat rheumatoid arthritis pain, swelling and stiffness. The DMARDs contribute to slowing of the disease progression. Use of these more advanced forms of medications may result in serious adverse events. Consequently, a rheumatologist should be involved in patient care.

After use of DMARDs, a rheumatologist may consider prescribing one of the different classes of biologics currently on the market. Biologics work to control inflammation by stopping a person’s immune system from attacking his/her joints and other tissues. Insurance authorization is a key factor in a patient’s ability to access these specialized medications.

Are there any experimental treatments currently in development? Experimental medication is exactly that—not proven in the market, not approved by the FDA and not available for use by physicians and patients without access to government or laboratory authorization. There is a pipe-line of various new and promising potential treatments under investigation at this time. What are ways that RA patients can enhance their quality of life?

Early diagnosis and treatment followed by controlling disease progression and symptoms will help patients experience less fatigue and pain, and enjoy a gainful living as well as a better quality of life with family and loved ones. The Arthritis Foundation furnishes educational information in hard copy brochures and on their websites, arthritis.org and arthritistoday.org. The Foundation also provides patient information in physicians’ offices, and promotes effective communication with physicians, physician’s assistants, physical therapists or occupational therapists to optimize treatment in addition to the use of medications.

The Foundation provides tools to encourage daily physical activity as well as guidance in diet and nutrition for well-being. It also helps patients gain access to needed medications through various insurance plans, and advocates for greater access to care at both state and national levels.

In all ways, the Arthritis Foundation works to help alleviate RA patients’ fear or anxiety about securing appropriate care and achieving a better quality of life with arthritis.

He serves as Co-Medical Director for the Acute Rehabilitation Unit at Presbyterian Intercommunity Hospital and maintains a private practice of Rheumatology and Related Diseases in Whittier, CA. A Fellow of the American College of Rheumatology and member of the California Rheumatology Alliance, Dr. Harris also serves as CEO of the Southern California Collaborative of Rheumatologists, a clinical research group of investigators.

People with any form of arthritis typically ask, “Is there an arthritis diet?” Or more to the point, “What can I eat to help my joints?” Here is an anti-inflammatory diet, also known as the Mediterranean diet, which is low in processed foods and saturated fat and rich in fruits, vegetables, fish, nuts and beans. Studies confirm that eating these foods lowers blood pressure and protects against chronic conditions ranging from cancer to stroke. The diet helps arthritis by curbing inflammation – which benefits your joints as well as your heart. Another bonus is weight loss, which makes a huge difference in managing joint pain.Whether you call it a Mediterranean diet, an anti-inflammatory diet or simply an arthritis diet, here is a look at key foods for joint health.

FISH (3-4+ ounces, twice a week) Some types of fish are good sources of inflammation-fighting omega-3 fatty acids. Increasing the ratio of omega-3 to omega-6 fatty acids—eating more fish and less fast food—helps mitigate chronic diseases, including RA. Best Sources: Salmon, tuna, sardines, herring, anchovies, scallops and other cold-water fish. Hate fish? Take a supplement. Studies show that taking 600 to 1,000 mg of fish oil daily eases joint stiffness, tenderness, pain and swelling.

NUTS & SEEDS (1.5 oz. daily (1 oz. is about one handful)) Nuts are jam-packed with inflammation-fighting monounsaturated fat. In addition to taste and texture, nuts boast protein and fiber. Even though they are relatively high in fat and calories, studies show that noshing on nuts promotes weight loss, because their protein, fiber and monounsaturated fats are satiating. Best Sources: Walnuts, pine nuts, pistachios and almonds.

FRUITS & VEGGIES (Aim for 9+ servings daily) Fruits and vegetables are loaded with antioxidants. These potent chemicals act as the body’s natural defense system, helping to neutralize unstable molecules called free radicals that can damage cells.Best Sources: Colorful fruits and veggies like blueberries, blackberries, cherries, strawberries, spinach, kale, broccoli, eggplant and bell peppers.

OLIVE OIL (2-3 tablespoons daily) Olive oil is made up largely of healthful, monounsaturated fat. It’s anti-inflammatory, heart-healthy and tasty, too. Best Source: Extra virgin olive oil. It goes through less refining and processing, so it retains more nutrients than standard varieties.

BEANS (About 1 cup, twice a week) Beans are loaded with fiber, a nutrient that helps lower CRP, an indicator of inflammation found in the blood. They are also are full of antioxidant and anti-inflammatory compounds. Beans are a great (and inexpensive) source of protein; protein is important for overall health, especially for muscle health. Stronger muscles make it easier to keep joints moving.

Best Sources: Red beans, small red kidney beans and pinto beans. Other beans you may want to add to your rotation are black beans, garbanzo beans and black-eyed peas.
