Claim1. Claim intimation in the prescribed format with required documents as per nature of death should reach Bajaj Insurance Operations Team/branch.2. Bajaj Insurance Operations Team/branch will check the documents, correct them and send them forward to HDFC Life Claim team at the below address: Group Claims Department 11th Floor, Lodha Excellus, Apollo Mills, N M Joshi Marg, Mahalaxmi, Mumbai - 400 011. Maharashtra. 022-666827793. HDFC Life reserves the right to call for additional documents or raise further requirements if required.4. Claim amount is settled as per member authorisation and claim form received. The claim amount will be transferred via fund transfer/NEFT to the bank account.
