INTRODUCTION

This report is intended to give the reader an overview of how ABP manage the

navigational safety of the Humber Estuary, Rivers Ouse, Trent and the Humber, in

relation to its obligations as a Harbour and Pilotage Authority. The Humber continues to

be the busiest estuary in the United Kingdom with some 13,182 inward shipping

movements in 2012, giving rise to 29,286 vessel movements in total. In addition, there

were 3871 recorded Oil Barge movements moving 1.15 million tonnes of product

between Immingham, Hull and upriver destinations. Latest available statistical figures

indicate that in excess of 81.3 million tonnes of cargo and over 1.030 million passengers

were handled.

Towards the end of 2010 Siemens and Associated British Ports announced that they

have signed a Memorandum of Understanding in connection with ABP's proposed

Green Port Hull development at Alexandra Dock, Hull. The parties have agreed to work

together to develop plans for the construction of a new Siemens offshore wind turbine

manufacturing and export facility at the port. Definitive agreements between the parties

are awaited prior to construction works commencing.

On the South bank, Able UK are awaiting a consent decision from the Secretary of State

to build a berth between South Killingholme Oil Jetty and Killingholme Haven to support

offshore wind farm manufacturing and exports.

Construction of Grimsby River Terminal (GRT) commenced in 2012, with commissioning

expected July 2013.

Depths in the River Trent were affected detrimentally through 2012 by prolonged dry

spells followed by “flash” periods of wet weather. Temporary draft restrictions were

applied and by year end depths were almost back to normal.

2012 saw a slight increase in river traffic overall with highest concentrations of vessel

traffic again centred on the Immingham area. The number of reported “Ship Defects”

accounts for the 8.5% rise in reported Navigational incidents; we continue to work

closely with the MCA by passing on all reported defects, however minor. This enables

the MCA to carry out targeted port state inspections.

Inward for Hull Alex Dk, Pilot reports vessel had to stop engines due to engine problem. Vessel proceeding to Hawke anchorage. 1838 Vessel advised to anchor on GY middle, unable to make Hawke anchorage. 1846 Pilot reports vessel brought up 0.4nm NW South Shoal. Cargo Timber. Problem appears to be water in the fuel, expect repairs to be completed by 0400. Pilot to remain on board. Duty manager advised. 12 POB, bunks 45.5T, Lub oil 1820Kg, cargo 2794 Cubes. 2149 Vessel testing engines. 2234 Engine trials complete. Pilot reports port engine operational, vessel still has problems with Stbd engine. 2318 Pilot reports both engines now operational. 0900 Vessel passing through lock at Hull Alex.

1713 Captain P (windfarm workboat) reports v/l has an electrical problem and needs to drop anchor. V/l anchors 3 cables SW of the 4B, clear of main channel. 1800 V/l able to power up jets and proceed to Grimsby. Due to electrical problem v/l is unable to heave anchor. Anchor position marked with "large pink Dan buoy" unlit. Advised Captain P to recover anchor soonest.

After speaking with Pilot and Agent it appears a seawater inlet pipe fractured earlier causing an ingress of water into the engine room, a temporary repair has been made. Nothing has been reported to VTS by the v/v/l's master. V/l was proceeding to lay by at

Top Gunness before proceeding to Albert Dock and drydock on Thursday. Vessel has no orders until PM on the 4th. Duty Manager advised.

Captain still declaring that one engine is not working but a PSC visit on his last voyage to Hull did take place. Expected to dry dock in January 2013, not returning to the Humber ports before this time.

Pilot reports to VTS, after disembarking the vessel, his concerns about the engine status after experiencing prolonged delays with engine commands during the act of pilotage on outward passage. VTS request, on VHF14, the Master declares his reasons for requesting DW anchorage and for confirmation that he will not be conducting any repairs whilst in the anchorage. Master confirms before anchoring that reason is only to await orders and no repairs to be carried out whilst at anchor.

Pride of Rotterdam starts to pass to the North of the Bar Protector just as she makes a large unrequired alteration of course to Port right in front of the POR. POR is forced to take an emergency turn to Port, out of the channel, passing North of the South Shoal Buoy.

Whilst singling up at IOT 2, the following vessel movements were underway: VLS Vogebulker - departing HIT 2 VLS Southern Crossdream - inward for HIT 2 VLS Centennial Harmony - inward for IBT. On going to stations, the Pilot contacted VTS with details. VTS advised that the vessel may depart between Vogebulker passing and

Southern Crossdream passing Upper Burcom. Once singled up, clearance was requested but denied owing to the location of Southern Crossdream. Unfortunately, the after springs and a forward spring had been let go (whilst the pilot was returning from within the wheelhouse to the

Master on the port bridge wing controls). With the flood tide, the pilot was unable to hold the vessel alongside so, advising VTS, all lines were let go and swung to port and astern, to pass the Southern Crossdream green to green, which occurred

10 Jan 12 RMS TWISTENDEN

Vessel departing Goole Ocean Lock for Sea has run aground on the Ness opposite the lock. Pilot reports no damage or pollution, no one injured. Vessel has 6 + 1 persons onboard, 36.9t Gas Oil, and 1000t steel products. Pilot is awaiting more water, Goole Docks and vessels in area warned. MCA advised. Goole Tide Gauge 4.71m HW 1951 UTC. 1857 Pilot reports vessel is afloat and is continuing on passage to sea. MCA advised. Next Port Sturzelberg.

Vessel departed Ocean Lock, Goole for outward passage. Master and Pilot were on the bridge at the time. Once safe to do so, hard to starboard helm and half ahead was given. The vessel swung into the tide and headed down river, with speed over the ground at 3 knots. Soon afterwards, the vessel started slowing down and turned to port, despite trying to keep former course. As the bow approached the east bank of the river, the main engine was stopped, and the vessel was set onto the ness off Ocean Lock middle pier. Vessel refloated at 1855 and continued outward passage.

12 Jan 12 GAASTBORG

Mid river off Grove wharf. Flix gauge 3.72m. 9 POB, 554.2T IFO, 18.3T GO, Cargo 2,118T Coke. C/G advised. 0749 refloated but bow pinned on N bank due to strong wind. 0810 Free and proceeding on passage

The vessel was boarded at Grove and after Pilot/Master exchange, the Flixborough gauge was requested from VTS. There wasn't enough water at that time and the Master was informed that the vessel would have to wait 20-30 minutes longer.

At 0730, when the gauge was 3.2m, the vessel left the berth on the back spring. Despite full thrust to starboard, slow ahead and 20 degrees starboard helm, the vessel canted across the river where the bow grounded on the bank, opposite the east end of Grove. The vessel refloated after a few minutes but it was difficult to get the bow through the current, owing to the wind on the superstructure. After 20 minutes manoeuvring, the ship got through the wind and current and proceeded on passage.

Vessel outward from Apex having aborted passage to Goole grounded upriver of 32C. Posn 53° 43.2'N 000° 37.9'W. Brough gauge 4.60m. 10 POB (inc 2 pilots). Cargo Steel wire, rod and bars 2052 tonnes, Bunkers 36 tonnes gas oil. HMCG advised. 2302 Refloated with assistance of Shovette & Lashette. HMCG updated.

1105, the vessel was inward for Goole at the East Walker Dyke. The Chief Officer was on the helm with the Master operating the engines. The vessel was informed by Goole that the tide was too much below prediction (50cm) and there was not enough height of water to continue. They also advised that the vessel could not use Blacktoft as a lay by berth, despite the fact that the theoretical predictions still allowed the vessel to berth on the next tide. The POM advised the pilot that the vessel agents

wished the vessel to return to the Bull Anchorage. With the WWD gauge showing 4.3m, the vessel commenced passage to the anchorage. At approximately 1129, the vessel ran aground 3.5 cables off the 32C buoy.

2230, the tugs 'Shovette' and 'Lashette' were in attendance. The vessel refloated at 2303.

06 Feb 12 NATALI

V/l grounds in a position 017 x 0.65m from the No 34 Buoy. WWD Gauge 4.0m. V/l refloats at 0440 and resumes passage to Goole. 9 POB. Bunkers 86t. Cargo Wind turbine parts.

The Master and Pilot on the bridge, with the Chief Officer acting as helmsman.The vessel was en-route to Goole and encountered poor visibility of 1-2 cables above the Humber Bridge. Having passed Brough, the visibility had reduced to less than 1 cable, but there was sufficient water, so the passage to Goole continued.

When approaching the number 34 buoy, the flood tide was setting the vessel to the west and onto the bank to starboard of the designated track. The vessel grounded at 0407 and was approximately 0.25 miles from the 34 buoy.

The two vessel astern passed down the port side of the Natali, following which the vessel refloated and continued on passage to Goole. The ship was underway again at 0440.The vessel berthed safely in Goole at 0705 without

further incident.

07 Feb 12 NAUTICA

0530 v/l' touched bottom' in the vicinity of flats light. inbound for grove. agents Whartons.

Vessel was rounding Trent Ness with Master and Pilot on the bridge. The pilot was hand steering around ness. Vessel on Dead slow - Master standing by to increase speed, at this time approximately 5 knots. Noticed fog bank coming across river - visibility up to that point had been about 1 mile.

Transit beacons could be seen until fog came in. Radar picture was good but showed vessel off track. The pilot asked for half ahead but the vessel grounded on bank, nearly abeam of Flats Light.

VTS were informed that the vessel entered the Trent with 4.33m on WWD. Ahead/ Astern movements were used with the bow thruster and vessel came off the bank. VTS reported 4.7m on WWD. Vessel was clear of the bank at 0540 and proceeded to berth with zero visibility.

07 Feb 12 TOMKE

0600 v/l 'touched bottom' in the vicinity of flats light. West Walker dykes gauge 5.08m. inbound for grove. agents Whartons.

Vessel had a draft of 4.4m entering the Trent with WWD indicating 4.92m. Visibility was 1-2 cables and utilising searchlight with shoreline sighted. Encountered dense fog on the swing to Cliff End leading lights. Vessel grounded to the west of Flats Light for approximately 3-4 minutes.

Prior to swinging vessel to starboard, the ship was stopped through the water when passing Grove LE 1. vessel ordered hard to starboard with Full Ahead. The vessel sheared to port so the engine was stopped to ensure the vessel swung clear to port of vessel alongside of Grove UM1.

26 Feb 12 PEAK OSLO

0800 Pilot reports inward bound v/l touched bottom in the vicinity of Flats light. West Walker Tide gauge 4.58m. V/L proceeding outbound before making a second attempt to enter the Trent.

0830 V/L re-enters the Trent, proceeding to Neap House. 0930 V/L berthed at NH1. Agents; Grove Wharf.

Draft on boarding 3.55m. Tides at Albert Dock and Humber Bridge cutting by 15cm and 40cm respectively. At 0615, when vessel in Skitter Channel, Master requested to deballast, to 3.35m aft.

Passed Brough at 0725 with Blacktoft showing 'on prediction'. Passed WWD at 0742 with 4.35m on gauge. Entered Trent in the correct manner, when turning to port to head up for Flats light, pilot could not distinguish any lights. It appears that the vessel had turned to port too much as the vessel grounded to 2-3 minutes. Following this, the vessel refloated

and proceeded back to WWD. Another approach was made without incident.

Vessel outward from Grove Dolphin aground as she left the berth. Pilot requires no assistance at this time and is awaiting more water. (Flix Gauge 5.05m). 0845 Vessel underway again. No damage or pollution.

27 Mar 12 NINA

Touched bottom in Goole reach near Earnshaw Clough. No reported damage. Diver to attend later as precautionary check

The Master, Pilot, 2nd Officer and one other person on the bridge. At the time of the incident, the tide was 44cm below prediction running late. Started to swing the vessel to starboard after passing Earnshaw Clough Light. Approximately half way through the swing, the vessel appeared to touch the bottom on the port quarter and the ship stopped as the stern landed on the bank.

The vessel was refloated by slowly moving with kicks ahead. Continued ahead until stern cleared, dropped anchor and swung vessel. Vessel then berthed without further incident. At the time of this report, it is not known if there was any damage to the vessel, the Master was requesting a diver to attend in the dock to check.

Pilot boarded vessel at 1630 while vessel was still closing hatches. Declared draft was 4.95m. The Mate checked the drafts and reported 5.2m aft and 5.0m fwd. The pilot informed the Master that the maximum draft for the tide was 5.3m and as long as the tide was on prediction, the vessel could let go at 1720. At Nypro, crossing to the west side of the river, she touched the bottom with the vessel on the line. She touched bottom again passing Meredyke and the pilot suspected that the reported draft was incorrect. Then mid way between Cliff End rear light and Cliff End 44, the vessel grounded, came to a stop and swung to port before stopping altogether. Attempts to refloat on same tide were unsuccessful. The Mate reported that there was still ballast water in some tanks and this was pumped out. At 0615 on the next tide, the vessel refloated and proceeded on

05 May 12 AGENA

Aground abeam Island House River Trent. BOS gauge 5.15m. Bunkers 40t, 10 Pob, Cargo Attapuleite. Adv Agent. V/l aground until next High water, 1 tug ordered to assist. 0325/6th Tug Shovette departs Alexandra dock Hull to assist refloating. 0630 Refloated 0645 Swung round proceeding to Grove.

On the bridge at the time of the incident were the Master, Chief Officer, Pilot and Trainee Pilot. On passing WWD at 1750, gauge read 5.8m. The vessel used new West Channel, transiting between 50-60 metres off shore.

Approaching Pilot House Corner, on Slow Ahead (approx 7 knots), vessel took large shear to port, positioning stern approximately 40m off Pilot House.

Attempts were made to refloat until the vessel settled with falling water on a SSE'ly heading, until the next ride where she refloated with the assistance of tug 'Shovette' and proceeded to berth.

08 May 12 AFON LLIGWY

On entering the lock outward, the vessel stopped. The tug informed pilot that the legs were not even. Barge lifted leg and vessel started moving.

On entering the lock outward, the vessel came to a stop. Engine power was increased to no effect. The tug informed the pilots that the legs on the barge were not even, one at 16m and one at 10m. The barge lifted the leg and vessel started to move.

The Dock Master was informed and divers ordered for investigation.The leg was level with the inner wheel, west side.

13 May 12 FAST SUS

After departing the berth at Flix. the vessel grounded in the vicinity of Manreval light for approx 15mins. No reported damage or pollution.

24 May 12 RMS RIGA

Vessel grounded approximately 1.8 cables SE of the Lower Whitton. (Brough Gauge 5.41m). Visibility 120/150m. 0850 Vessel afloat again and proceeding inward for Goole or Blacktoft. No damage, pollution or injury to report. Vessel too late for Goole, aborting to Blacktoft for this tide.

Grounded due to dense fog.

Departed grove TE1 and grounded in position 53 36.44N 000 42.47E. Flix gauge 4.08m POB = 9, Cargo 2,500T timber, Bunkers: 107,700Ltrs HFO & 6,100GO

C/G advised 2116 refloated. 2122 back alongside Grove wharf.

The Master, Second Officer and pilot on the bridge. At 2000, pilot informed VTS that the vessel was going to start making preparations for departure. VTS reported Flixborough

gauge was 3.6m. At 2010, vessel left the berth and proceeded downriver. At 2018, in a position just to the east of Grove LE1 and in the centre of

the channel, the vessel grounded. The speed of the vessel at the time was approximately 3 knots. Flixborough gauge at the time showed 4.0m. The chart showed that in that position, the depths should have been between 1.0m

and 1.5m. This would have indicted that total depth was expected to be in the region of 5.0m. After one hour, the vessel refloated and berthed safely at Grove LE1.

It should be noted that after the vessel had refloated (north of grounded position), upon moving south, the vessel again grounded briefly in the same position.

When crossing from Solitary House to Waddington Light, vessel grounded but refloated immediately. The vessel ahead also grounded 40-50 metres off Man Reval, then refloated, so own vessel passed Man Reval approximately 70 metres, but again grounded. Vessel remained grounded for approximately 15 minutes, then proceeded to berth.

Flixborough tide gauge at the time of grounding was 3.25m, and 3.35m when refloated. Tide was 0.05m above prediction. No reported damage.

15 Jul 12 AMADEUS

V/l grounded and stopped for a short period in a position 148 x 1.5 cables from Man Reval Light. Flix Gauge 3.16m. V/l refloated and resumed passage.

At approximately 1650, in a position 200m upriver of Man Reval Light, and about 30-40m off the bank, the vessel, proceeding at 7.5 knots, came to a complete stop. The vessel slowly gathered speed and continued, with the Flixborough gauge reading 3.10m. This incident indicates there is less water than the latest chart suggests.

15 Jul 12 BARBAROSSA

V/l grounded and stopped for a short period approx 1.5 cables to the EAST of the Upper Whitton. V/l was on the course line and Brough Gauge read 4.50m. V/l refloated and resumed passage. Ref H76 a least depth of 0.4m was expected upstream of the Upper Whitton.

VTS observed vessel's AIS stopped in Swinefleet Reach. No contact from the barge (who departed Goole at 0718) when it left Goole or stopped. Via a request to Goole ADM from VTS, information given: barge admits he is aground and "pumping ballast".

0757 AHM telephoned the barge and pointed out obligations to report. C/G advised. 0854 refloated and proceeding

16 Aug 12 BALDER

Pilot reports that the vessel is aground off Neap House Wharves. Flixborough gauge 4.16m. POB 8 (including pilot), ROB gas oil 56 t LO 1.5 t. Cargo: Shredded tyres.

Duty Manager and Coastguard advised. 0721 Refloated and proceeding to Grove LE1. C/G advised. V/l alongside the berth.

Vessel was down by the head 30cm. Tha pilot asked the Master if the vessel would steer well when down by the head and he replied that she would steer fine.When passing Neap House, the vessel touched the bottom and eventually grounded. Waited until High Water to refloat vessel and the Master then reported that the bow thruster wasn't working. The vessel stayed aground until the following tide and the vessel was then berthed at Grove LE1.

The vessel grounded at 1850 with HOT of 4.1m at Flixborough. The vessel refloated at 0720 with HOT of 4.99m at Flixborough.

The vessel left Grove Dolphin at 1925, with 4.3m on the tide gauge.

Master and Pilot on the bridge.

1930 - As the vessel approached Grove 2, the vessel took a turn to port and grounded. 1940 - Vessel refloated and swung head to tide, then proceeded outward. 1950 - Vessel refloated and swung head to tide with the aid of the starboard anchor.

1945 - Vessel grounded again in similar position to original grounding.

1955 - Anchor was reported as home and the decision was made with the Master to proceed to a safe berth (Grove TE1) As the vessel landed alongside, the starboard anchor fluke punctured the starboard bow causing a 30-5-cm hole. Upon investigation, it was found that the anchor was hanging 2-3 metres from being properly secure, even though the anchor had been

reported as home. 2010 - Vessel was moored alongside at Grove TE1 and VTS Humber informed.

Vessel grounded off Mere Dyke from 0317-0321 with Flixborough Tide Gauge 3.65m. Vessel arrived Flixborough HW +30 minutes, still some current. Max draft criteria needs revising - survey requires updating - latest available 03 September. From this survey, there should have been sufficient water.

06 Oct 12 SEA MITHRIL

Pilots reports vessel is all fast at Flixborough NE stbd side to in the River Trent. However when passing Man Revel Light with the Master in control the vessel's stbd quarter hit the bank damaging the stbd aquamaster.

The unit is currently out of service but the is no apparent damage to the hull. With no pollution and no one injured. The vessel dropped her port anchor prior to coming alongside to control her manoeuvre. The chain has now been slacked right down to the riverbed.

Once passed the Man Reval light, the Master was advised to bring the vessel to port towards Flixborough slowly. The vessels speed was 6 knots. The position of swinging was advised on the chart and on the radar. The master was concerned about the vessels speed, so he reduced it to 2 knots. This pushed the stern across to the starboard

bank. The vessel was brought alongside to starboard side to on the port aquamaster.

Master, Helmsman and Pilot on the bridge. Departed Blacktoft at 0830 for the River Trent, with visibility at 1.5 cables. VTS informed that there was a report from a survey

vessel in the Trent that neither side of the river was visible. When approaching the Boundary Light, visibility deteriorated to dense fog. Lookouts were already posted on the bow. Radar was set to 0.75m North up. Nearing the Apex, it could be seen on the radar that the vessel was too far to the North, so the course was altered to starboard. The Apex beacon was not seen in the clutter.

1725 V/l grounds on the East Bank opposite the lock at Goole. 1750 V/l refloats and resumes outward passage. Gauge 4.8m.Bunkers 35t. Lube 1t, Crew 10. No reported damage or pollution.

Vessel would not turn to starboard when leaving the lock at Goole, even with the bow thruster. From information obtained from the Master (very poor English), the engine management system does not allow a rapid increase in power.

Ship could not turn into strong flood tide and went striaght across to the opposite bank.

11 Dec 12 HUMBER PRINCESS

Touched bottom north of 32C. Brough gauge 3.44m. Cargo 460t Lube oil, 2 POB. 1428 underway and proceeding to Goole

Departed Hull, fog drifting in and out between 1 mile and 5c. At the bridge, visibility was 1 mile and at Brough reduced to 2.5c. Brough gauge not reading but as vessel passed West Cappers, the Bridge was reading 3.75m. Master expected to have 1m less on the Brough tide gauge which would give UKC of 0.8m. Vessel ran into a bank of fog on approaching 32C, so extra wide berth was given and caught the mud.

Vessel was there for a short period only and was then back underway. Vessel stopped at 1410 and was underway at 1425. All was reported to VTS Humber and Humber Charter stood by until vessel refloated.

0250 - Vessel sailed from Grove Wharf with Master and Chief Officer on the bridge. Hand steering engaged. 0305 - Vessel passed Flixborough. Once clear, ‘River Pilot’ deployed. 0340 - Vessel passed Burton Stather, at slow speed for inward traffic. No problems encountered with vessel. 0355 - When Trent Ness and Apex lights in transit, commenced turn to starboard, with ‘River Pilot’ ROT set for 40. Shortly

afterwards, the ROT was observed to be behaving erratically, 80 starboard, rudder observed to be hard to port (55). Vessel was swinging back to port and heading for west bank of the Trent, south of South Trent light. Vessel was put in hand steering, hard to starboard wheel. Engines were put to stop and then full astern, vessel swinging to starboard close to west bank of Trent.

0400- Vessel's port side touched bank south of South Trent light. Vessel sheared to starboard. Engines stopped and then put Dead Slow Ahead. Vessel was observed to be moving ahead and cleared Trent at 0404.

Tanks were sounded and no reported intake of water. At West Walker Dykes, the river pilot was engaged with no problems encountered until turning to port at Cappers West. At this point, the ‘River Pilot’ again behaved erratically, taking a shear to port, then observed with hard to starboard rudder. Hand steering was deployed until clear of the bridge. Again, at the Elbow buoy, the ‘River Pilot’ behaved erratically. Sea pilot deployed from this point onwards.

No reported damage to the vessel. 12 Dec 12 KAPITAN ZHIKHAREV

Touched bottom at the Apex. WWDK gauge 5.7m Whilst transiting the Skitter Channel inward, the visibility dropped to one mile then, at the Humber Bridge, thick fog was

encountered. This was to last all the way upriver. There was a vessel ahead and one mile separation was maintained. The Master went on the helm. The vessel ahead slowed right down and in order to maintain separation, the pilot reduced the speed of own vessel to match. It was not possible to overtake owing to draft of own vessel. As a result of the speed reduction, it became very difficult to steer the vessel and the pilot instructed the Master to keep the Apex Light fine on the port bow. The pilot had already requested Blacktoft via VTS, as the tide had cut by 20cm. This, together with the visibility and amount of outward vessels from the Trent, the decision was made to proceed to Blacktoft.

As the vessel passed the Apex, and abeam of Flaxfleet Ness, she touched the bottom and slowed down. The pilot ordered hard to starboard and Full Ahead and the vessel swung all the way around until she was heading back towards the Dykes. The vessel was then manoeuvred and proceeded to Blacktoft without further incident to await next tide.

some damage to the fendering. No reported damage to the vessel.

On approach to the lock from the Exxtor terminal, the Captain was in control of the vessel doing his own manoeuvring. Before turning to head into the lock, the vessel had gained two knots speed and the pilot advised the Captain to reduce his speed, reminding him of the westerly wind. By the time he had turned, the wind and his momentum had set the vessel to the east of the lock entrance. The Captain repositioned the vessel but as he approached, the vessel continued to be set to the east. The pilot advised to continue thrusting bodily to port but, despite this, the vessel landed on the East Inner knuckle with his starboard side, at which point two fenders broke away from the quayside. The captain had declared to the pilot that he was new onboard, but had been on the vessel before and was happy to do the manoeuvring.

No damage was done to the vessel.

14 Jan ROUBINI K Imm Dk Whilst entering the lock at Imm the vessel made contact with the lock side on the stb'd side causing damage to the bulwark stanchions. No reported damage to the lock.

On the bridge were the Master, helmsman, Pilot and trainee pilot.Whilst bending around the fender to enter Immingham Lock (West side-flood approach), a steel plate positioned on the shore side fender made contact with the underside of the floating bulwark, causing 3 angled bracketed stiffeners weld to be uplifted and slightly indented on the vessel.

30 Jan CITY OF PARIS GY Royal Pilot reports that whilst manoeuvring into the lock the vessel made contact with some coping stones, dislodging them, and damaging

the vessel's paintwork. No injuries or pollution reported. (Wind E'ly 3-4).

As bow entered the lock, the wind freshened slightly from the East, causing the starboard shoulder to rub along sheathing on west outer coping stone.The Master, Pilot, helmsman and OOW on the bridge.

There was scraped paintwork on the starboard bow and a slight indentation to coping stones sheathing.

12 Feb PELEUS Blacktoft Inward from Blacktoft to Goole. Pilot reports vessel has rope in propeller and aborts back to Blacktoft. Damage to vessel and jetty reported by Master. Pilot advises vessel has damage to paintwork only. No structural damage observed. Duty manager advised.

The vessel was boarded at Blacktoft at 0750, she was starboard side alongside with the tide flooding. The Pilot and Master were on the bridge. The pilot requested that the stern lines be let go as they were almost slack followed by the after springs - keeping engine minimum ahead and rudder hard to port to keep the stern alongside. The crew were heaving the lines when one is believed to have become stuck around the propeller. The engine was stopped and the pilot instructed a heaving line to be sent ashore, but this wasn't done. The stern started drifting away from the jetty and the stern swung across the river on the fwd springs. The vessel made a complete swing, head into tide and let go the remaining lines. The Master then confirmed that all lines were clear. The vessel was set mid river before swinging to port to head in the Goole direction. During the swing, the pilot again asked the Master if the bow was clear but the he was talking to the officer on the bow (in his own language). Full astern was ordered but the ship contacted with the west end of the jetty - shifting one top fender and the handrail. There were scratches to bow but the vessel was brought back alongside to investigate for further damage.

12 Feb SORMOVSKIY 3058 New H Pier Vessel departing New Holland Dock for sea lands on New Holland Pier. 2115 Tug Norman requested to assist.

2125 Pilot reports vessels bow now back in the dock. 2130 Norman stood down 2135 V/l fully in dock making fast. 2140 Pilot reports some damage to scaffolding on NHP no damage apparent to vessel - confirmed by chief mate Duty Manager Informed & Agents at NHP informed.

Vessel was boarded at 2000 and given clearance for departure at 2045. Albert gauge at that time was 38cm below prediction and Immingham was about 10 minutes to top. The vessel was manoeuvred out of the dock, but neither the stern or the bow could be lifted out of the tide - the vessel landing starboard side to on the scaffoldings on the walkway to New Holland Jetty. VTS and New Holland Jetty were informed. Once the tide had eased up, the vessel was manoeuvred back into the dock as, by this time, the ebb was away in Immingham. For the safety of the vessel, sailing was cancelled for the tide and a pilot reordered with a tug recommended for departure. There was damage observed to the scaffolding on New Holland Jetty but no apparent damage to the vessel.

13 Feb ARGOS KGD Contact made with shoreside crane during berthing. No damage to vessel, crane shifted 1m along quay.

Position 5 metres from end of dock, 10 shed KGD. The vessel approached the berth with the Master on controls and pilot advising. Once the vessel had cleared barge alongside 9 shed, the Master was advised to send a spring asap. However, this was delayed and in order to stop the vessel, astern propulsion was used. With the offshore wind and the vessel being left handed, she began turning to port. The Master was advised to make spring fast, put wheel hard to starboard and kick ahead. However, the slack couldn't be taken out and astern propulsion was needed to avoid contact forward. As the vessel was almost perpendicular to the quay, the decision was taken to let go forward lines and berth starboard side to quay. During the swing, the vessels starboard quarter began closing the knuckle by the steel terminal. The Master was advised to go hard to starboard and come ahead to clear. However, the vessel quickly picked up headway and contact was made with crane with the bow. The vessel then berthed starboard side to quay.

A small dent was reported to the vessels bow and the crane was moved approximately 2m along the rails.

17 Feb MARSTAN Imm Dk Bellmouth Pilot reports light contact with east side lock Minor damage to concrete. Master had the con.

The vessel was approaching Immingham Lock with the master on the controls. Lost the line of the lock on approach by approximately 10 degrees, the Pilot suggested that the master stop the vessel and realign. The Master continued towards the lock and attempted to lift the stern to starboard using engine and rudder. The vessel entered the lock about 10 degrees off line and vessel amidships touched the port side, contacting the east side entrance.

Lockmaster advised vessel that minor damage had been done to the cement on lock. No damage to vessel.

21 Feb UNION GEM GY Royal Pilot reports whilst manoeuvring to depart Grimsby Royal Lock for sea. (Pasajes next port). The vessel's bow has made contact with

the lock causing some damage to the ship. Ship's crew checking around, Pilot will report back shortly. ( Wind SW'ly 5-6). POM informed. POM reports having spoken to Pilot, vessel has a dent in the bow but no breach. No pollution and no injuries to crew. 1635 Captain confirms vessel is seaworthy. Ship proceeding with voyage.

Master and Pilot on the bridge, the pilot had the con. As the vessel approached the lock outward, the strong flood tide coming through the lock caught the starboard bow, pushing the ships bow off to port. Starboard helm, full thrust to starboard and, eventually, full astern were used. However, contact was made

between the bow and the west side of the lock. The vessel re-berthed to allow inspection of the bow and forepeak. No holes were found and the vessel continued on passage.

22 Feb LOMUR Goole Vessel hit the inner lock gate in Ocean lock Goole, as the engines would not operate astern. Goole ADM reports approx 3 to 4 m of

top decking damaged. Damage to ship unknown.

22 Feb HERM Imm Dk

Master and Pilot on the bridge. Vessel was making approach to Immingham Lock on ebb tide with the tug Guardsman in attendance. The tug was fast on centre lead

aft. Once vessel was in position to start turning to port, the following orders were given: Tug - Starboard Quarter minimum weight. Rudder - Hard to port. Engine - Slow Ahead. Bow Thruster - 50% to port. As rate of turn was slow, tug was instructed to increase weight to 25%. After a while, it was noticed that rate of turn was still slow but headway was almost stopped. When looking astern, it was found that the tug was pulling right astern instead of starboard quarter, as requested. Tug was ordered to move towards the starboard side, but the tug master reported difficulties in getting across the tide. After a while, the tug got into position and vessel turned to line up for the lock. The 'All Easy' order was then given to the tug and after a short while, the tug requested to come around and give a check on the port quarter. The vessel was still turning to port and, on looking astern, it was found that the tug was still on starboard quarter instead of the requested port quarter.

At this time, the berthing staff reported the distance closing between the vessel and west knuckle. The order to the tug was repeated and all means attempted to avoid contact with the lock side. Despite this, the vessel landed on the west knuckle fendering at a speed of approximately 0.8 knots.

The report from the berthing staff stated that two rubber plates of west knuckle broke off once the vessel landed on it.

Vessel approaching the berth, approximately 5m off, parallel to quay. Forward spring and stern line were sent ashore. Vessel moved ahead so order given to slacken spring. Vessel manoeuvred ahead when spring made fast, causing bow to shear to starboard, making contact with shore gangway, bending a foot plate.

26 Feb AFRICAN WILDCAT Imm Dk Whilst berthing at 3 Quay, Imm Dock, vessel made contact with shore side crane resulting in minor scratching damage. Pilot reports insufficient fendering at the berth.

Vessel moving alongside No 3 Quay extension port side to, Yokohama fenders were placed alongside flat panel fenders on Quay in wrong positions, such that when vessel came alongside, Yokohama fenders moved in between panel fenders, thus vessels port side awning stanchion contacted crane on quayside. On the vessel, the bridge wing awning stanchions were damaged and the man overboard flare ignited. The shore crane suffered scratching only.

01 Mar ANGLIA SEAWAYS Imm Dk Vessel inward to Immingham Dock from Rotterdam. Pilot reports vessel made contact with lock edge near to the outer gates whilst

manoeuvring, dislodging a section of concrete into the lock. No structural damage to vessel, no pollution and no injuries to personnel.

The Master was doing a PEC for Immingham Dock. The vessel landed heavily, but belive damage may have been caused by a cross like structure 'Heysham Cross', the vertical member would have caught the brickwork, ie hull bounced not slid.

A coping stone was reported damaged

08 Mar AUTOPRIDE GY Royal Inward for GY Royal Dk, contact made with roller wheel at lock entrance. Slight damage to wheel, no damage to v/l.

Whilst entering Grimsby Lock, vessel made contact with East Knuckle roller fender. No damage to vessel and no visual damage to fender. ADM reported that engineers needed to assess damage. Vessel proceeded to Alex Dock with no further incident.

The Master, Pilot and Chief Officer on the bridge. Speed at time of incident less than 1 knot.

22 Mar CLARITY Trent Whilst berthing at Neap House No 1 vessel made contact with the berth causing damage to the vessel in way of indentation of the hull in the vicinity of the port quarter. Damage was sustained to No 2 Jetty in way of one wooden pile.

On approaching Neap House, vessel was slowed down to approximately 2 knots, ready for turning in a position the Pilot believed would be safe and clear. The Master complying in all respects let go the port anchor. The vessel swung to starboard but got set in by the tide. The result was that the port quarter made contact with No 2 jetty. When the vessel settled in the river, the anchor was heaved and berthing completed. The pilot believes the anchor was let go a fraction too early and the vessel should have been more to starboard before dropping. There was no damage to the initial contact point, but the third pile from the west side was damaged. Damage to own vessel was an indentation to the port quarter.

31 Mar BOUZAS Imm Dk Whilst entering Imm Lock vessel made contact with the Outer gate causing damage to the hand rail.

Vessel entering Immingham Dock at LW slack, with Master, Pilot and Chief Officer on the bridge. The starboard flare of the bow contacted with vertical stanchions on the West Outer gate. The stanchions had not been lowered. There was superficial damage only to the vessel, with two bent vertical stanchions on the west side outer gate.

04 Apr BROVIG CIERZO E/J Vessel berthing at east jetty extension Immingham, lands heavily on approach splitting some timberwork. Wind NE'ly 30kts. Ebb tide.

The vessel was approaching Immingham East Jetty in gale force wind on ebb approach. The vessel was dropping down nicely onto the berth, but at the last minute, the bow swung to starboard and port quarter landed heavily on the eastern end of the East Jetty Extension. At the time, there was full thrust to port and helm was hard to port with engines being kicked ahead. There was a slight indentation to the steelwork on the vessel, on the port side accommodation below the main deck. The fendering and steel plate was distorted and split ashore.

10 Apr FAST JULIA Gunness Vessel transiting from Flixborough South End to Top Gunness Fina in the River Trent. Pilot reports vessel landed heavily alongside the jetty at Fina causing a small spilt in the ships plating. No injuries occurred and there was no pollution. The split is above the waterline and about 10m from forward on the stbd side, approximately 4 inches in length.

The Pilot and Master were on the bridge, the pilot giving instructions with the Master on controls. The vessel sailed from Flixborough and swung at Keadby without incident. Vessel backed down to a position where the vessels stern was in line with the south end of the jetty, approximately 50m abeam. Vessel then crabbed in slowly whilst also drifting back for position. A spring was sent ashore forward, once the bow appeared to the clearing dolphin upstream of TG Fina. Vessel crabbed in further at this point and the pilot asked the master to confirm that the vessel was clearing the jetty and by how much. The deck crew told the master that the vessel had to come further astern. From experience, the Pilot knew that this was incorrect as the vessel looked almost in position. Nevertheless, this raised doubt and the engine was put astern. This effected a turn to starboard (even thought the thruster was going to port) and once angle created on tide, vessel increased turn to starboard. Helm was put hard to port with port thrust, and engine run ahead. This slowly reduced angle of vessel with quay but not enough to prevent starboard shoulder landing moderately on corner stone. To summarise, the pilot states he should have ignored the misleading information given by the crew. There was a stone mark and possible 4' horizontal tear in the hull, starboard side - approximately 10m from forward and 1.5m above the waterline.

13 Apr EIDER GY Royal Contact made with east side outer wheel fender whilst departing Grimsby on the level.

Vessel departing Grimsby on the level. Starboard quarter was closing on wheel so Master was advised to apply starboard 10 helm. Port wheel was applied so quarter landed on east side outer wheel fender. The wooden sacrificial fender was hit ashore and the starboard aft railing bent slightly on vessel

13 Apr MAERSK RAPIER SKJ On departing the berth at SKJ made contact and caused damage to some timber fendering, scrapes on ship's paint only

Lifting off with tugs in Push/Pull mode. Underestimated lift of aft tug (Guardsman) building from 50-100% pull. Power of forward tug overcame aft lift causing port quarter to graze jetty timber face/fendering. There were scratches to the paintwork on the vessel. Ashore, there was slight damage to timber face and possible damage to gratings.

16 Apr CHRISTINA Goole Contact made with quay side when entering Goole Docks.

A quarter line was run from the port quarter to assist the turn to port after exiting the lock (normal practice). Weight came onto the line as the turn was commenced and the line parted. At the last distance given off of 10m, the turn was continued, but the vessel just failed to turn clear and contacted the quayside with the starboard aft corner of the vessel. No personnel were injured. The parted mooring was checked, and although not new, appeared in fair condition. The Master was manoeuvring, under guidance from the pilot and trainee pilot observing.

23 Apr LINNAU Goole Made contact with the quay in Goole at Caldaire Terminal causing 3 or 4 large dents in the ships plating on vessel's port side (hull

not pierced) . Some damage to the quay's concrete.

The Master, Pilot and pilot trainee on the bridge. Master was on the controls with trainee pilot conning with authorised pilot overseeing. On the approach to the berth, the vessel sheared to starboard, possible due to the vessels' draft and shallow water, or a build up of silt/mud.

The bow thruster was immediately put full to port, the helm put hard to port and the engine run ahead, to stop the vessel from colliding with the corner. At the time, it seemed these actions had worked as there was no impact felt on the ship. However, when the

vessel was moored, a visual inspection showed that the plating was dented over a 1.5m section aft of amidships, port side.

02 May COASTALWATER IGT Contact made with vertical ladder next to fender on berthing.

Vessel was being berthed on the ebb tide, with a strong NE'ly wind in ballast. The Master had the con. The Master was advised of the current direction and the strength of the wind, as the vessel was in ballast. On berthing, the port

shoulder made contact with the vertical ladder next to the fender. The top railing of the vertical ladder was bent along a length of approximately 2 feet.

05 May CAROLINE THERESA Imm Dk Vessel made contact with Immingham inner lock gates.

The vessel was making her final approach to Immingham lock with a speed of 2 knots. When the ship passed the outer lock gates, the pilot advised the Master to give 'Slow Astern' to reduce the speed for the lock mooring. The Chief Officer reported the ships speed at 2.2 knots and increasing. On checking the pitch indicator, it showed 20% ahead. The pilot immediately advised the Master to reverse the pitch, 'Full Astern', but on doing so, the pitch still showed 20% ahead. The pilot instructed the moorings to be put on the bollards to minimise the impact to the inner lock gates, however, all lines parted owing to the vessel speed. The pilot advised that the bow thruster was worked full to starboard to bring the bow to the lock wall. However, contrary to this advice, ordered that the port anchor be dropped and emergency stopped the main engine. However, the pitch still showed 20% ahead. Via Immingham tower, two tugs were ordered and made fast by 1630. The vessel then berthed at 3 Quay extension at 1715. There was a slight dent to the starboard part of the bulbous bow.

10 May CALLISTO Imm Dk Whilst berthing at no 9B quay in Imm. Dock vessel made contact with the quay causing 'slight indentation ' on the stb'd side for'd of the bridge.

On bridge at time of incident: Pilot, Master, OOW and helmsman. When coming alongside No 9b mineral quay (un-fendered), the vessel landed heavily causing scuffed paintwork and slight indentation to shell plating, stbd side in way of No 4 hatch. No fenders were provided by the vessel.

15 May FORTH SENTINEL GY Royal On entering GY lock, "nudged" the lock gate dislodging 1 piece of timber

Whilst entering the lock, the barge being pushed brushed the West Outer lock gate, dislodging timber. No known damage to vessel.

31 May CLIPPER POINT Imm Dk On the approach to the lock, the starboard bow rested on the East side whilst the vessel was entering the lock. The flare of the bow

came into contact with the stanchions which were upright at the time, resulting in 7 of them being slightly bent.

The Master and pilot were on the bridge, with the Master on the controls. Vessel made a normal approach to the lock. During the lock entry, the vessel sheared off to starboard and the stbd fwd part of the

vessel made contact with the lock wall. There was a small dent to the starboard shoulder. Some concrete crumbled to top of capping on north wall of lock, in the vicinity of the inner corner of outer lock gate recess.

03 Jun CHRISTINA GY Royal

On departure from Grimsby, the vessel stern made contact with outer gate.

On departure from Grimsby, the vessels stern made contact with the east side of the outer gate. Some damage reported to catwalk reported by the dock master. No visible damage observed from the ship.

03 Jun IVAN GY Royal

On departure Grimsby with tugs, the v/l took a shear causing the tugs wire to catch the wind sock and a number of railings on the West side.

On exiting Grimsby Lock, a strong wind on the starboard beam caused vessel to shear quickly to port. Bow thrust, tugs and rudder used to full extent. The aft tugs port line contacted wind sock mast and some fencing. The after tug then slipped both his lines.

07 Jun HEENVLIET Trent

Vessel contacted with bank swinging off Flixborough on passage to Grove. No damage, no pollution.

Vessel departed Flixborough at 2145 and slow steamed into current. When inward vessel 'ANKIE' had passed, the vessel was swung to port. Even with the thruster full to port, the vessel would not swing and the bow touched the bank opposite Flixborough, even though full astern was applied. At approximately 2200, the vessel was brought off the bank and headed into the current. The forepeak was inspected by the ships crew with no damage reported. The vessel then completed passage to Neap House with no further incident.

No immediate damage observed, but fender later noted to be misaligned. 1748 - First Line Ashore (Fwd spring). 1752 - Moderate/heavy landing on No 2 fender (APT berthing Masters words) No obvious damage, no ship damage, but fender displaced to maximum and returned to normal position on berthing. Maintenance to

inspect over low water period. 1840 - All fast.

10 Jun DONGEBORG Imm Dk Contacted with fendering on 3 Quay, Imm when berthing causing damage to fender. No ship damage reported.

Master, Chief Officer and Pilot on the bridge. Master was on the controls with pilot advising from the bridge wing. Vessel had full cargo of wind farm blades on aft half of deck. Wind from the East with vessel berthing port side alongside. Forward spring was sent but parted and vessel contacted fender. At this point, the wind was catching the blades on deck and vessel pivoting off. The crew were very slow to get the stern line out. Eventually, called for tug assistance via Immingham. On 3 quay extension, 1 chain was broken on fender and ladder bent on dock, marks on the concrete. On vessel, the bulbous bow was covered in concrete dust in way of an old dent.

13 Jun FURORE G Imm Dk Coping stone displaced as vessel rising in Immingham Dock

Vessel was rising in Immingham Lock. A fender from the vessel was caught and displaced the coping stone. No reported damage to the vessel.

15 Jun ABIS BRESKENS Goole Whilst manoeuvring in Goole Docks, vessel lands heavily on Pilot House corner. Vessel returns to berth. A reported indent to the

bulbous bow has caused a breach of the hull.

Pilot and Master on the bridge. Captain doing own manoeuvring with pilot advice. Tug 'Shuva' ordered to assist turning into Ocean Lock. The vessel was backing down with Master at central controls with limited vision. He declined to use the wing controls due to the weather (Rain). Vessel was left handed. Bow approaching position to clear Pilot House corner, pilot advised to continue moving astern and thrust to starboard. Instead, found Captain has thruster to starboard but gone ahead on the engine. The forward momentum and starboard thrust combined to cause the bow to land heavily on Pilot House corner. Initially, the tug 'Shuva' was used on port side to counteract Easterly wind, then moved to starboard quarter ready for assisting the turn into Ocean Lock.

Damage to vessel was indent to bulbous bow, causing breach of hull.

15 Jun CLIPPER POINT Imm Dk Damage sustained to rubbing band whilst entering Immingham lock.

Vessel making flood approach to lock, under Masters control with pilot advice. Master and pilot positioned on port bridge wing, Chief Officer and trainee pilot on starboard bridge wing. The forward visibility of bow was poor, so the pilot moved to the centre to observe position of bow on approach to lock. The Master seemed to have difficulty lifting the stern up into tide and strong wind. Bow entered the lock under control with starboard side close to the west side lock fendering. The stern was lifting up but bow was not moving to port as required to clear fendering. The forward edge of the starboard rubbing, which presents a blunt profile, caught lock side walling, between fendering and outer gate. The leading edge of the rubbing band was crushed and minor deformation to ships side plating was reported. The damage was reported to dock and VTS and vessel proceeded to 11 quay without further incident. The rubbing bands start about 30m or more from bow and are about 1ft square box section and not faired to ships side. Damage was done to about 1m of the rubbing band.

19 Jun HMS TYNE KGD Dock master reports that whilst the vessel was passing through the West middle gate, it made contact resulting in one damaged

stanchion. No reported damage to vessel.

The Master, XO, Navigating Officer and numerous other personnel were on the bridge. The Master was manoeuvring the vessel, assisted by the bridge team. After the vessel berthed, the dock master advised the pilot that the vessel had broken a guard rail stanchion while moving along the west wall of the lock.

After positioning the vessel in Immingham bell mouth, the Master took over the manoeuvring, as had been agreed in the Pilot/Master exchange. After the vessel was secured in the lock, the Berthing Master indicated that the running band on the vessels port quarter had damaged a coping stone on the east side, by way of the knuckle. The Captain disputed this, but would take it up with his agent whilst in port. Owing to the design of the bridge controls, the pilot was unable to see down the ships side.

24 Jul RMS VOERDE Trent Reports making contact with quay whilst berthing Neap House 1

After completing the swing for Neap House 1, the vessel was proceeding to the berth. The vessel was veering to starboard which was corrected with kicks of ahead propulsion and hard port rudder. Pilot did not fully appreciate the speed and momentum of the vessel and on approach, the linesman noted the speed on VHF 17. The pilot checked and saw the speed to be 3 knots. The order for midships and slow astern was given and the vessel swung to starboard. Full thrust to port, stop engine and hard to port was ordered, then dead slow ahead. Despite this, the bow made glancing but heavy contact with the downstream end of the quay. The vessel was quickly brought under control and moored alongside. At no point did the Master bring the speed of the vessel to the attention of the pilot. A wooden fender at the top end of the quay was knocked loose and an upright fender on pile split.

Damage to own vessel were minor scratches to paintwork on the starboard bow.

01 Aug AUTOPRIDE GY Royal See attached pilot report.

On entering Grimsby Lock outward, the Master had the con on the starboard bridge wing. Vessel entered slowly to avoid east inner where the tyre is missing. Vessel landed moderately on west inner tyre, splitting sacrificial timber.

04 Aug GAS EVOLUZIONE IGT See pilot report attached.

Approaching IGT on the ebb tide. The initial plan was to use tug to press forward into fenders, then use ships engine and rudder to fetch stern in. This was owing to no

spring being taken from IGT because of fouling on the berth. First part went to plan, then Captain ordered astern, allowing stern to close quicker than pilot would have liked. Hence, the protruding pipe that protected the fairlead rollers became damaged. Pilot states that this ship can be awkward to manoeuvre at slow speed due to stop/start engine, and tug positioning either right forward or aft.

Pilot also states that he was distracted during the manoeuvre from data centre telephone call to confirm double act job.

04 Aug LONGSTONE HST Whilst berthing at the HST berth 2 vessel made contact with the quay causing 'minor' damage to the last fender. No reported damage

to the vessel.

Port quarter clipped. Up river last fender catching facing rubber. Vessels speed just about stopped 0.2kt.Master berthing vessel. No damage to vessel. Minor damage to fender.

16 Aug PACHUCA Imm Dk Damage reported to east outer knuckle when entering bell mouth. No damage to vessel.

On entering the lock at Immingham, with the Master on the con, the vessels port side made contact with the east side of the lock. Initial reports showed a weakening of concrete to the lock side. However, it was also reported at the time of the occurrence, by

the Master and shore personnel, that this area had previous damage.

05 Sep SAMSKIP COURIER KGD Report from ADM KG, vessel experienced a blackout whilst maneovering for departure off berth, during which time the vessel came

in contact with the quay. This contact resulted in a 4 foot long section of damage approx 2 feet wide into the quay. The damage was caused by the vessels rudder stock. Tug Tradesman assisted vessel alongside where damage is being assessed.

07 Sep LEMONIA IOT2 Aft spring got trapped on wooden fendering causing large piece of timber to be dislodged on the main face downstream

14 Sep WIND SOLUTION GY Royal Whilst departing GY Royal vessel made contact with the no 6 bollard causing damage. Also when entering the lock the vessel made contact causing damage to the timberwork. No reported damage to the vessel. Wind W'ly 35-40 kts.

On boarding, the Master informed the pilot that a second tug had been ordered to assist into the lock, as the wind was gusting to 35 knots on the beam. He said the tug wasn't required for un berthing as the bow thruster was adequate.

The Master took the con on the starboard wing controls, pilot assisting with tug orders and a deck officer on the port wing giving distances.

The tug 'Inge' was ordered 50% pull as soon as moorings were slipped. Before the vessel moved off the berth, boatmen reported damage to a bollard.

Once clear from the vessel moored on 5,4 and 3 quays, Inge came around to pick up the starboard leg of the stern bridle. Once fast, she was put on 20% astern. Este had come on the starboard side amidships and was pushing as the vessel approached the lock. Due to her design, the Wind Solution cannot roll along the wheels in the lock and with the wind, contact was unavoidable. Informed by the ADM after departure that the vessel had damaged a bollard on 6 quay and the timber on the west and east sides of the lock.

21 Sep TONGAN Imm Dk See Pilot report

On approaching Immingham Lock entrance, the starboard side rubbing band of the ship touched the west side knuckle at the lock entrance. This dislodged two rubber face plates, fitted to steel backing plates on the turn of the knuckle. These two rubber plates, approximately 3ft square, were recovered by the mooring boat before the ship berthed.

25 Sep WESTEWIND Apex Vessel outbound from Grove wharf. Pilot reports vessel coming into contact with the Apex training wall, in vicinity of North Trent Bn. Contact made around port quarter of vessel.POB 7 including pilot. In ballast. Gas oil ROB 29.4 m3. Lub oil ROB 3400 litres. 0623 Anchored in Hawke anchorage to await inspection. HMCG Bridlington informed.

Master, Chief Officer and pilot on the bridge. Vessel outward from Grove, in ballast. Vessel made contact with the training wall between South Trent Beacon and North Trent Beacon with her port quarter.

30 Sep MUSCA GY Royal Vessel inward for Grimsby Royal Dock from sea. Pilot reports vessel grazed the dolphin on approach to the lock. Contact made with

vessel's port side but no damage evident to ship or structure. No pollution or injury to report.

When approaching Grimsby basin, the vessels speed was reduced to approximately 3.5 knots, with the wind and tide setting vessel across the lock. As the vessel was turned to port, the swing began to accelerate due to the wind on the port quarter. The engines were put to full astern by the Master to reduce all headway and the vessel came to a stop, heading south about 5 metres off the dolphin on the west side. As the vessel gathered minimum sternway, it began a slow port swing, again due to wind on the quarter, and the bow grazed the corned of the dolphin, causing minor (superficial) damage to the cement. There was no damage to the ship or pollution. The vessel continued to berth without incident.

30 Sep ABIS BRESKENS KGD Vessel inward from sea to KGD. Whilst manoeuvring into the lock the vessel made heavy contact with the west side of the stonework.

( Wind SW'ly 20kts ) There is however no apparent damage to either the lock or vessel. No one was injured and there is no pollution.

The initial plan was to approach on the eastern side and spring the vessel in, but the lock was not ready, and Pride of Rotterdam was about to sail from RT1.

The vessel proceeded upriver and swung around and, once the ferry was clear, an approach to the eastern bullnose was attempted. However, the cargo (wind turbine blades) caught the wind and the vessel landed heavily on the wet side timber framing.

Once alongside, the Master made a check and reported no damage. KGD advised that an investigation will be carried out.

01 Oct CHARLIE ROCK Alex Dk Barge Charlie Rock dislodges coping stones when entering the lock. ADM reports unable to close inner gates and can only work levels.

02 Oct GALICIA Imm Dk See attached pilot report.

On returning to Port House at 2245, the pilot was informed that on entering Immingham Lock, the vessel knocked off one teflon fender on the west knuckle. When the vessel entered the lock, it did touch the west end but no damage was ever reported, despite there being one berthing man on the west side.

04 Oct EAGLE KANGAR IOT At 0812, commenced letting go of the forward long springs, as is the usual case on this size of vessel. They got caught between the

ships hull and the fender face. Tugs 1 and 2 eased down to help retrieval but to no avail. The decision was made to let go the remaining short springs, and to heave on the caught long springs (0824).

Once short springs were on board, the vessel started to leave the berth, with the forward crew heaving on the trapped springs. Unfortunately, the springs had dropped in behind the fender face, and as they were heaved, ran over the top edge of the fender. It caught and pulled off an A4 size piece of rubber (most probably loosened by previous vessels).The damage was reported by the berthing Master and confirmed by the mate forward.

19 Oct VIGILANT Albert Dk Vessel inward for Albert Dock. On entering the lock, vessel makes contact with the lock wall, causing damage to stanchions.

As the vessel was lining up into the lock, the steering was switched to the port bridge wing tiller. The wheel was put hard to starboard to stop the stern swinging to starboard. The rudder actually went hard to port and the starboard quarter landed on the lock wall and the damage occurred. The problem with the tiller was noticed and rectified by re-switching it to the port wing. The vessel went on to berth without further incident. There was a small dent to the starboard propeller guard on the vessel.

A report from the Lock Master indicated that two stanchions were slightly damaged on the northern outer lock gate.

23 Oct SOAVE Albert Dk Vessel inward to Albert Dock from sea. Backing into the lock the pilot reports making contact with one of the knuckles causing a small

dent in the stbd quarter of the vessel. No pollution or injury reported.

Vessel backing down to Albert Lock, with tug 'Lashette' fast fwd and tug 'Beamer' fast aft. Due to the lack of water at the entrance of Albert, the vessel had to be backed close to the knuckle, before backing into the lock. Whilst backing down, the starboard quarter made contact with the knuckle, causing a small dent. There was no water to the North for the tug to lift the quarter.

25 Oct ALEXANDER KUPRIN Goole Made contact with Goole lock, eastern side of the entrance. Damage to some concrete and ship's bow indented to approx 20 x 20 cms. Small hole made in bow, above waterline.

The Master, Chief Officer, Pilot and trainee pilot on the bridge. The initial plan was to run the ock, but even after HW Goole, the ebb didn't start. Decided to go port side to with the conventional approach. The forward spring was passed and pilot asked for this to be made fast. Engines were on minimum ahead, bow thruster full to starboard and helm hard to starboard. Was called by tendering and informed that the bow was approaching quickly into the lock. Half astern was ordered but no movement was noticed. Full astern was ordered but again no movement. Contact was made and damage recorded only the south side of the entrance to Ocean Lock. Once in the lock, the Chief Officer reported that there was a dent on the bow, approximately 20 cm diameter.

Vessel then proceeded to berth port side to in West Dock, SE berth.

25 Oct MISTRAL IGT Vessel departs IGT for Terneuzen. Pilot reports vessel's handrails damaged two strips of rubber fendering on the downstream end of

the jetty whilst departing the berth. No damage to ship.

The Master, Chief Officer, Third Officer, Helmsman and Pilot on the bridge. Vessel port side to berth, tug 'Serviceman' made fast starboard side, Push / Pull. Flood tide. As the stern was lifting off the jetty, a slightly protruding hand rail on the poop deck caught on the down river fender. As the vessel stern lifted off, it pulled 2 strips of rubber fendering away, approximately 1.5m x 20cm.

30 Oct UKD ORCA Grimsby Royal ADM at Grimsby reports the UKD Orca has suffered an electrical failure whilst dredging in the entrance to the dock resulting in the

vessel contacting the new river terminal causing some damage to both structure and vessel. ADM confirms no injuries onboard the vessel.

1733 UKD Orca reports the vessel has damaged her bulbous bow, but they are checking around the vessel. They believe the vessel sound and seaworthy. No pollution reported. (10 POB, Bunkers 119t Gas Oil). 1741 Duty Manager informed. Vessel intends to transit to Imm Dock to have inspection conducted by divers. Awaiting news on the damage to the terminal. DM on route to assess. 1809 ADM reports some concrete damage to the terminal. A LW inspection to be conducted. From DM vessel will now transit to Alex Dock Hull. Duty Manager informed. 1814 Vessel confirms again that all tanks and hull are sound. 2049 Vessel safely moored in Alex Dock alongside the Euro Terminal.

01 Nov AGATE GY Royal Vessel inward from sea, manoeuvring for lock entry damaged some rails on her port side. No injury or pollution reported. ADM to

advise condition of fender wheel. Vessel also reportedly touched the bottom on approach to the lock.

13 Nov MARC ANDRE Goole Vessel en route to Goole South Dock Terminal landed heavily on South Dock Bridge causing slight damage to railings. No reported damage to vessel.

The Master had the con of the vessel and was manoeuvring to South Dock Bridge Hole. Bow was entering the bridge hole at minimum speed. Forward spring was ready on the port side, but ineffective when vessel sheared to starboard. The bow thruster was full to port and wheel hard to port as stern cleared Steel terminal pillar. However, the vessel sheared to starboard and the flare of the bow made contact with the handrails. When the vessel was finally berthed at South Dock terminal, was unable to get vessel alongside and was approximately 0.5m off the berth.

The Master, Pilot and trainee pilot were on the bridge at the time of the incident.

Three stanchions were bent on South Dock Bridge - no reported damage to vessel.

20 Nov BERGE ARCTIC IBT Whilst berthing at IBT vessel made contact with a fender on the quay.

The vessel was in the final part of the approach with tugs No 1 and No 4 out on wires ready to check. The headway was taken off using Astern on the ships ME and No 4 tug checking the transverse thrust. The bow swing rate to port increased and No 2 was stopped and No 1 was used to check the bow. The bow landed first on No 8 fender and the vessel landed flat shortly afterwards. The vessel was stopped F&A at this time. Nothing was said from the shore until the pilot was leaving the vessel, when it was stated that some damage had been caused to No 8 fender. On inspection, the pilot noted a small piece of concrete (slither) found on No 8 fender top. The fender itself seemed to be undamaged. Concrete looked to have detached itself from fender back itself.

No reported damage to the vessel.

21 Nov ARCO BECK. Alex Dk Pilot reports making contact with lock side and causing damage to vessels paintwork and a slight indentation on the port quarter.

The Master had the con as the vessel departed the lock (Alex Dock, Hull). The vessel was too close to the east side of the lock and scraped the port quarter momentarily. There was no obvious damage until the Chief Engineer reported a slight indentation to frames on the port quarter and some scuffed paintwork.

29 Nov RIX PHOENIX IOT 1507 - Rix Phoenix departs Albert Dock, Hull, bound for IOT Finger Pier with 25 Gas Oil. 1724 - Vessel manoeuvring off IOT Finger Pier. 1731 - Vessel informs VTS that the vessel will no longer be loading at the Finger Pier and is now bound for Immingham Dock -

awaiting pen in. 1747 - Rix Phoenix declares change of destination, now returning to Albert Dock, Hull. 1905 - Rix Phoenix arrives at Albert Dock, Hull. Report was received retrospectively (30/11/12 - 0900) from Immingham ADM, that whilst manoeuvring off the IOT Finger Pier, the

vessel made contact with the jetty and sustained damage. No report was made to VTS Humber by the vessel at time of incident.

01 Dec MAIN HIGHWAY GY Royal PEC No 1159. Vessel lands heavily on the Flood Gate whilst entering Grimsby Lock, no reported damage to the gate but the vessel

has punctured the hull approx 2.5m above the waterline.Hole measures approx 10cm by 10cm. Reported to VTS via Grimsby ADM, nothing reported from vessels master. Advised MCA who will refer to MAIB.

08 Dec ANGON Alex Dk Whilst manoeuvring in Alex dock vessel made contact with the quay on the stb'd quarter, resulting in a small indentation and minor

paint damage. No damage reported to the quay.

After entering Alexandra Dock extension, the vessel commenced swinging to starboard. The Master had the con and was following Pilots advice. The vessel started to build momentum after a command of Slow Astern was given, while the thruster was running to starboard. The vessel gathered speed and closed on the quay. Despite an order of hard starboard rudder and Half Ahead, the starboard quarter made contact with the quay. There was a minor buckling of the shell plate and scratched paintwork on the starboard quarter.

09 Dec WILSON SKY Imm Dk Whilst berthing at 4 quay vessel made contact with the dock wall. No visible damage although the bulbous bow was below the

waterline.

Vessel was in the lock at Immingham with the Pilot, Master and helmsman on the bridge. The vessels intended berth was No 4, with a vessel on No 5 and bunker barge on No 3 quay extension. Pilot had a discussion with the master about going SST, and running a fwd spring ashore asap, to run into due to wind conditions and vessel being fully loaded. With standard rudder and under powered bow thruster (220kw), Master agreed that SST would be the best approach. Vessel departed lock and made approach to No 4 quay. Shallow approach made to berth, with the bow 10m-15m off quay. Vessel crew delay on sending fwd spring. Vessel making 1.5kts (SOG), half astern given to slow vessel and bow to starboard to avoid bunker barge (wind setting vessel onto barge). At a speed of less than 0.5kts (SOG), vessel's bow made contact with No 4 quay. Fwd spring made fast, rudder hard to port and good kick ahead given. Bow still run to starboard to keep clear of the bunker barge. Stern line ashore and vessel heaved alongside with aid of engine. Vessel made fast at 0330 and VTS informed of making light contact.

There was slight paint damage to vessels starboard bow. No other visual damage done but due to vessels draft, slight plate damage to bulbous bow could have occurred.

19 Dec KIRSTEN GY Royal Vessel inward from Sea to GY Royal Dock. Pilot reports vessel made contact with the west knuckle on approach to the lock causing a

dent in the bow. No one was injured and there was no pollution. Pilot informed ADM. Wind SE'ly 7/8.

At Pilot/Master exchange, discussed wind, tide and speed at Dead Slow. Master insisted that he had sufficient power to keep control for the approach speed, entering and berthing. As the bow approached the lock entrance, the stern set down slightly and despite helm and increased power, the alignment was not regained. Headway was stopped and the bow landed on the west knuckle. Spring was put out, and worked vessel into the lock, through on the level and SST to No 8 quay, without further problems. Master stated that he was unconcerned and that he was happy with the action taken. There was a slight dent and scrape on the starboard bow.

27 Dec CENTRICA PRIDE Imm Dk Pilot reports vessel made contact with the East Inner Gate at Immingham dock, suspected damage to the gate which will be

inspected shortly, no reported damage to the vessel. 2000 Immingham report minor damage only to the walkway.

Master, Chief Officer and Pilot on the bridge. The Master was conning the vessel from the Control position. Whilst entering the lock at slow speed, the stern drifted to starboard, causing the starboard quarter to make light contact with the East Inner Lock. Superficial damage to fender - possible 2 bolts on uprights popped.

30 Dec PACHUCA Imm Dk Pilot reports possible damage to quayside safety ladder. To be confirmed on daylight inspection.

Vessel came out of Immingham Lock into the square and swung to port, to back onto 8 quay. The containers on deck caught the wind and the vessel was pushed onto Henderson Quay. The landing was onto the fenders except the focsle. The Master was on the controls and manoeuvring the vessel. Vessel then made fast at Henderson Quay. The Master, Chief Officer and pilot were on the bridge.

There was possible damage to the safety ladder at Henderson Quay.

15 Feb KHOLMOGORY KGD

Whilst manoeuvring in KGD prior to departure, the vessel makes contact with tug 'Tradesman' and 11 Quay, before returning to the berth.

The vessel was boarded at 1045 and the pilot advised that a tug be ordered, owing to the wind and small draft. The Captain refused to order a tug. The Master was doing his own manoeuvring and informed the pilot that the bow thruster was inoperative. The tug 'Roman' was requested to assist whilst the Master was operating engines on the bridge wing (from Full Ahead to Full Astern). The vessel drifted into 11 Quay and made contact with the tug 'Tradesman', potentially causing damage to the vessel own rudder. Two more tugs were ordered via the Dock Master. The Yeoman was made fast on the stern and Tradesman pushing on the bow. The vessel berthed at 15 shed KGD. Damage was observed to KGD 11 Quay and dented superstructure on the tradesman.

24 Feb STAR ARUBA Alex Dk

On moving in the dock prior to sailing, the vessel lost all engine control and struck the barge Humber Energy causing its ropes to part and drift in the dock. Incident managed and reports to be made by ADM

The Master and Pilot were on the bridge with the Master conducting his own manoeuvring. The vessel sprang off on a port forward spring, cleared the berth without indicent and proceeded slowly towards the gutway. Approximately 70- 100m from the gutway, very loud banging and vibration was heard aft; because of this, the pilot suggested that the vessel berth inside the extension and make a check. The Master then indicated that he had lost control of the ME and steering, but still had control of the bow thruster. The Pilot then asked if the ME could be stopped but, with many alarms sounding, the Master indicated that he thought it was stopped. There was still considerable wash coming from aft, indicating that the vessel was still being propelled ahead and turning to port. The engineer then came to the bridge and the pilot suggested stopping the engine locally, which the engineer went to do. At this point, the bow entered the gutway and the noise aft ceased. The vessel struck a glanding blow with the port bow on the south wall of the gutway. After striking the wall, the vessel sheared a little and slowed slightly, but the bow then continued to port and contacted the port quarter of the barge 'Humber Energy', whose moorings parted; she then drifted across and berthed herself on East Quay Main Dock. The Star Aruba then continued to swing to port and landed bodily alongside the 'Grovedale' causing no damage, before continuing on towards the south dock wall. The Pilot asked the Master to drop an anchor to slow the approach to the south wall which he did. The vessel stopped bow first on the south wall, with no further damage done. At this point, the vessel landed bodily alongside the 'Humber Energy' with no further damage. Linesmen arrived and both vessel were secured.

17 Apr OLZA Goole

Vessel transiting Goole Docks, contacted with moored vessel. Minor damage to railings on stbd side Fast Sus (moored v/l), no damage to Olza. Master had con.

The Master, Chief Officer, Pilot and Trainee Pilot on the bridge. Vessel exited Ocean Lock with Captain manoeuvring and pilot on the wing, giving advice about distance off. Pilot returned to the wheelhouse to advise to start turning. Pilot noted that the speed had become excessive. The vessel was left handed and manoeuvres to turn were not as effective as expected. The bow made contact with side of 'Fast Sus' on Gantry berth. There were bent handrails over about 2m on the Fast Sus, no reported damage to own vessel.

10 May AQUARIUS Trent

On turning for Flixborough S, hit the Fast Sus (berthed on Flixborough North) with his stern. Fast Sus reports ballast water leaking from a crack in the fore peak tank (above the waterline). Damage to Aquarius on port quarter sheer strake set in approx 6cms for a length of 25cms. H Master advised. Both vessels advised to contact their classification societies.

The Master and Pilot were on the bridge with the crew at stations. Routine swing to starboard off berth with the bow near the opposite bank. Engine was put astern to assist the swing. The Master was on the controls with the pilot observing the port quarter. The pilot gave an ahead order but this was delayed as the Master had left the controls to pass something to a crewmember who had arrived on the bridge. A situation was developing, the order amidships and 'Full Ahead' was given to clear the berthed vessel. Unfortunately, contact was made with the 'Fast Sus'. Once the vessel had berthed, the pilot and Master inspected the damage. There was a slight indentation on the port quarter of Aquarius, main deck level, 20cm long and set in 5cm. The port shoulder of the Aquarius was punctured above the waterline reporting in leakage from the water ballast tank.

23 May PRUDENCE Keadby

Vessel outward from Top Gunness Fina in the River Trent for Immingham Dock, takes a shear, and has made light contact with crane barge Collingham moored at Keadby on the River Trent. Pilot is stopping the vessel to assess for any damage.

1952 Pilot reports no visible damage to Collingham or his own vessel at this time. Collingham is manned and crew are checking around. 1958 Pilot confirms that Collingham states she is undamaged. Prudence has 5 POB, and 30,000 litres gas oil. MCA informed. Coastguard informed. 2014 Collingham confirms with VTS on CH12 that they have no known damage at this time. 2049 Duty Manager advised. 0909/24th Collingham reports that during the incident a steel upright forming part of the repairs to the jetty was bent.

Vessel departed Gunness Fina at 1945 with HOT 1.9m at Keadby. When passing Keadby, vessel took a sudden shear to port and made contact with the crane barge 'Collingham', which was being made fast to Keadby berth. The ship was brought under control and both vessels visually inspected, with no apparent damage. A report was made to VTS and the vessel continued to Immingham without further incident.

20 Jun WILSON MALM Imm Dk

When exiting the lock, proceeding into Immingham Dock,

Captain, Pilot and helmsman on the bridge. Going out of the lock into Immingham Dock with a tug fast astern, the vessel was pushed west, close to a barge. With no thruster, the vessel was not able to swing to port against the wind. The vessel made contact with the starboard bow of the 'MAI LEHMAN' moored at Number 5 Quay.

Vessel was boarded at 1330, vessel ready to sail. The tug 'Guardsman' called and asked for instructions. Pilot asked him to pass a tugs line to centre lead aft so he could tow the vessel to the centre of the square for an approach to the lock. During this time, the UKD Bluefin let go, moved across to No 4 berth alongside / close to Wilson Hook, to commence dredging. At this time, the master of the Alexander Tvardovskiy instructed the pilot not to make the tug fast, just leave him free on the port side to standby. It was his first trip and he wanted to do the manoeuvring to the lock. The Mate was on the wheel with the Master on the Controls. The vessel let go, the wind pushed her off the quay and the Master set dead slow astern. At 50 metres from the UKD Bluefin, the vessel was picking up speed and the pilot requested the Master to stop engines. He spoke in Russian to the Mate and ignored the pilot. The pilot ordered Half Ahead, followed by Full Ahead. The Captain was watching over his shoulder and then went full ahead. The Master on the UKD Bluefin asked if the vessel was going ahead, to which the pilot replied that he was now on full ahead with the wheel amidships. However, the vessel didn’t seem to slow down. On impact, the vessel still pushed astern and seemed to push the UKD Bluefin sideways into the Wilson Hawk. Once the sternway had ceased, the engines were stopped. Then. With dead slow ahead, the vessel made her way back to No 2 berth. Once alongside, the vessel was checked for damage. There were no injuries or pollution. The Master informed his owners who said they would organise a P&I club inspection. On watching a video replay, at 50m away when they engines were put full ahead, the vessel seemed to accelerate astern, so there was possibly a gearbox problem. This would explain why she didn't stop when the engines were put ahead.

Pilot reports that when mooring inward vessel on Grove TE1 contact was made with bow of berthed vessel 'Steinau', moored on Grove LM1. Slight damage, to paintwork reported.

Master, Chief Officer and Pilot on the bridge. 2125 - Pilot on board at grove LE1, draft 2.9m F, 2.7m A. Noted with Master that the vessel Steinau was berthed 10m astern of Celtic venture on Grove LM1. The pilot informed Master of the procedure for singling up and chartlet shown of shoal water off Grove Wharf (05/07/12). 2200 - VTS were informed that the vessel was singled up and letting go (Flix 3.95m). Lines were let go, bow thruster to port, engines DS Ahead, wheel stbd 10. Vessel lifted bodily off the berth and moved ahead. When vessel was parallel to the berth, 40m off, the engines were put to DS Astern, with the bow thruster remaining full to port. Vessel moved astern with the bow swinging rapidly to starboard towards the berth and across the current. The pilot noticed that the vessel was drifting quickly towards Steinau. The engines were put Slow Ahead and the stbd bow put alongside Grove LE1. Vessel would not move up the berth for re-berthing and stern closing on Steinau. Wheel was put hard to stbd and engines reduced to DS Ahead, stern lifting clear of Steinau. Bow thruster remained full to port to prevent the bow moving towards the other vessel. Vessel was angled approximately 60 degrees to berth, engines put half astern with rudder amidships. Vessel commenced moving astern and bow lifting off the berth with current on the port side. It was again noticed that the vessel was drifting towards Steinau, at a slower rate. 2215 - Vessel made contact with port bow of Steinau, scratching paintwork above the vessels name and container support on the starboard side of Celtic Venture. Vessel remained moving astern, lifting off Steinau with thruster still full to port. Vessel cleared Steinau and no further incidents reported. The Celtic Venture crew reported that they could see no damage to either vessel except for scratched paintwork. No crew sighted onboard Steinau. Master, Chief Officer and Pilot inspected Steinau from the quay, but there was no visible damage and the vessel was secure alongside.

Svitzer Susan attempted to recover buoys but was unable to do so due to high freeboard. Contacted IOT who offered the services of the Bull Sand. Bull Sand recovers the buoys with a long length of rope and anchor attached.

06 Jun VTS 14 Vessel makes contact with 4m baulk of timber approximately 1.5' SSW of No2 Haile Sand buoy. No damage to vessel, broadcast made.

06 Jun PILOT Pilot reports large baulk of timber vicinity of Clay Huts. Immingham dock contacted and they have tasked the ABP Progress to investigate. 0915 ABP Progress recovers timber from the river.

07 Jun COASTGUARD Informed by HMCG Bridlington that a member of the public has reported a large object in the vicinity of the Humber Bridge. Warning to shipping made.

05 Jul VTS 15 Call from Humberside police advising of the sighting of an object between KGD and Alexandra Dock (possibly a body). UKD Seahorse working Alex entrance advised. 1725 H.Rescue launched and on route. 1800 H.Rescue advises object is not a body and standing down.

07 Jul VTS 15 Pilot reports debris in the river north of the Pyewipe Outfall. 2m x 1m orange object, possibly some fendering. All ships warned by broadcast.

0809 Pride of York reports debris now near to HPI. VTS continue to warn vessels individually. 0842 Sighted near Upper Burcom. Appears to be 2 orange drums.

13 Dec VTS 15 Vessel reports small container 1m in diameter in the river between the Sand End L/F and the 19A Buoy. All ships broadcast made and vessel Precision 1 in the area warned directly. H Mercury sent to search the area but nothing found Cherry Sand recovered a large upright freezer in the vicinity of the IBT

Imm dock staff informs VTS that they have a small fire in the pump house of the lock. One of the pumps has caught fire. Vessel Hafnia Seaways requested to wait and stem outside off the W/J until situation resolved. 1556 Imm report situation is resolved and normal operations resume.

Humberside fire and rescue inform VTS via phone that there was fire onboard vessel Cricklade. Initial emergency call at 1134, Fire Brigade in attendance at 1140. ADM KGD, agents and owners informed. 1233 ADM KGD calls to report fire was in the engine room and is now extinguished.

2043: FV Calisha (PD235) (MMSI 235003876), contacts VTS to ask for assistance. Captain reports two Latvian crewmembers onboard the vessel are drunk, and since sailing have continued to drink. Now starting to behave in a threatening manner. Captain refuses to put to sea with these crew members on board and wishes to involve the police. Unable to pen back into Gy Fish due to state of tide, so request to send to Immingham. ADM requires vessel to contact them directly to organise a lock in.

2052: Contact Duty Manager. 2100 Imm confirm they will take vessel at approx 2300. MIO requested to contact the police and request that attend under the railways

and transport safety act of 2003. 2110 Humberside Police confirm they will attend (Incident No 694). IMM informed. Coastguard also informed. (All COMMS on CH13). 1315 FV Calisha all fast in Immingham lock.

Fast Sam reports what appears to be traces of oil from Thorngumbald to SEJ. 0740 H. Charter reports patches of what appears to be a coal dust type deposit mixed with debris . No sheen sighted . Area checked and

similar deposits sighted, it does not appear to be oil but maybe debris washed off the shore or deck washings.

Call from pilot on the Yasa Neslihan berthing HIT 1, reporting an Hydraulic Oil spill from a burst pipe on the HIT, less than 500 ltr spilt but mainly contained on the jetty, some spray entered the river causing a slight sheen.

Jetty staff dealing with the situation and MCA advised. 1330 Rcvd a call from Imm Duty Manager advising all the oil was contained on the jetty and none entered the river. MCA advised.

Request from Humber CG to check the precautionary area to the east of SLF for oil pollution . HMCG Bridlington received a report from satellite surveillance indicating a slick in the area. Nothing found launch returning to station.

02 Oct EAGLE KANGAR IOT1

Vessel moored at IOT1, whilst swinging the gangway on with a crane a hydraulic hose parted spraying the vessel with hydraulic oil, mainly contained on the vessel with a minimal amount entering the river. A shore side mooring adviser slipped on the oil and banged his head but injury not thought to be serious. H. Master and MCA advised. Polrep Completed.

20 Oct MASTER / CAPTAIN SPURN HAVEN 2 TMB

Spurn Haven 2 reports 100 litres of oily water discharged into the river. Spurn Haven 2 & Monogirl using wash from propellers to disperse. They are monitoring the situation. POLREP sent to HCG.

1523 Spurn Haven 2 update now pumping water into system to prevent any further egress of oil. Continuing to use Monogirl 2 to breakup the oil and confirm no dispersants used.

1700 Spurn Haven 2 reports sheen has now mainly dispersed and v/ls standing down.

21 Nov CAPTAIN RIX PHOENIX IOT

Sheen of oil reported covering an area between the shore and IOT Main Jetties. IOT Pier Master informed and making visual checks. IMM ADM informed also making visual checks. Origin of sheen may have emanated from the shore due rainfall run off- to be confirmed. 1215 IOT advise from visual inspection source coming from the land in the vicinity of the east jetty Simon storage area. 1245 IMM ADM - area checked and pollutant appears to be a chemical. Source suspected to be from the dock estate probably from a

drain. 1315 IMM ADM reports discharge into the river ceased source appeared to come from a drain running alongside Simon Storage. IMM H & S continuing investigations. No sheen now observable in the river POLREP Completed and sent to HCG.

Black oil, approx 20 m.cu sighted near Imm W/J. Several vessels in the area contacted and no further sightings. 0852 C/G and Po Ho advised. IOT report small amount of coal dust in the area. 0900 update from Imm, it appears to be breaking itself up and could be a light oil. Total Entries: 20

Conservancy Activities

December 2011 to December 2012 During this twelve-month period, the Survey Section of Humber Estuary Services undertook 648 individual hydrographic surveys, including 19 contract multibeam surveys for outside parties, 90 for UK Dredging and 76 Ouse leading line surveys. The greater part of the data obtained was passed to the United Kingdom Hydrographic Office to form the basis of Admiralty charts for the Humber Area. All information gained from these surveys was processed and charted, resulting in the production and distribution of over, 10,000 chart copies, mainly to be used in Pilotage and general navigation, a number of the local charts also being offered for general sale. The survey work resulted in the planning and carrying out of some 59 movements of floating navigation marks, in order to mark the fluctuations in the alignment of the navigable channel, mainly in the Upper Humber. In addition, out of a total of 138 local Notices to Mariners issued during the period, based on survey results, 84 contained warnings of shoal water.

Humber Charter The monitoring and maintenance of navigational aids dealt with 8 reports of deficiencies in floating marks and 6 in shore marks all being satisfactorily dealt with. The “Skitter” channel continued to be effectively the main channel for traffic bound to and from the rivers Trent and Ouse due to the continued shoaling in the Hessle Sands area.

Multibeam survey contracts have increased dramatically with 19 outside contract surveys. The Royal Navy conducted a multibeam survey from South Killingholme to Spurn Light Float in the later half of the year. Monthly multibeam surveys of the Centrica pipeline at Stallingborough commenced in May. It was in response to erosion that had created free spanning areas of pipeline.

The entrance to the River Trent saw significant shoaling with the channel having to be moved temporarily to the West side. The Sunk Dredged Channel (SDC) remained at 9.5m below chart datum or deeper for most of the year.

Visitors to Spurn Control Tower

There were 48 organised visits to Spurn Tower over the year involving the entertaining of 139 guests.

These ranged from visits of prospective PEC candidates; familiarisation visits by new pilots and staff through to larger groups from marine orientated clubs and other similar organisations, including the ‘Hull Cat Zero Project’ which is working to get unemployed young people back into mainstream employment or education and is supported by ABP.
