Foreword

With the 15th anniversary of the Midwifery Re-

search and Education Unit I realised that I have

seen a dream come true the establishment of per-

manent midwifery research and education group

with collaborations at the local national and inter-

national level I am therefore pleased to offer you

the Research Review of the Midwifery Research

and Education Unit at the Hannover Medical

School which demonstrates our activities and the

people behind them

I have personally been interested in advancing research and continuing edu-

cation for midwives in Germany since at least 1989 when I attended an Inter-

national Congress of Midwives workshop on midwifery research in Tubingen

This experience compelled me to organise and chair annual research confer-

ences for German-speaking midwives and eventually to lead this research

unit dedicated to midwifery research The current participation in the Europe-

an Master of Science in Midwifery programme the supervision of doctoral

students as well as original research both supports young midwife scholars

and the profession while producing evidence to improve maternity care na-

tionally and internationally

Our research in midwifery and maternal health is diverse and our units

scholars have covered topics ranging from the influence of womenrsquos self-

diagnosis of labour progress to the current situation of midwifery research

and education in several regions I invite you to learn more about our work

and scholars in this report

The Hannover Midwifery Research and Education Unit is

settled within the Department of Obstetrics Gynaecology

and Reproductive Medicine at the Hannover Medical

School one of Germanys leading universities in medical

research patient care and teaching The Midwifery Re-

search Unit was formed in 2000 Since 2001 the unit has

published over 50 peer-reviewed papers and abstracts and

received funding from several sources including the German

Research Society (Deutsche Forschungsgemeinschaft) the

European Union and governmental institutions

Strategic Aims

The units main goals are to advance the profession of mid-

wifery and to improve the quality of midwifery care with a

combination of research and strategic action The unit seeks

to assess and create educational and professional opportu-

nities for midwives while developing a body of midwife-

focused research to inform the clinical needs of women and

their children

The Unitrsquos research interests span a variety of topics in ma-

ternal health including

The education and academic development of midwives The effect of medical intervention on the birth process Relating womenrsquos perceptions and expectations to birth

events and outcomes Scientific documentation of the process of birth with re-

spect to significant events over time The circumstances affecting the provision of midwife

care How to implement scientific research in midwifery care Midwivesrsquo perspectives on specific aspects of labour and

birth such as pain or caesarean birth Midwivesrsquo perceptions of research in their profession

Prof Dr Mechthild Gross

Midwife Director Midwifery Research and Education Unit

Prof Dr Gross was trained and has been practicing as a nurse midwife since 1983 She holds a Masters degree in Psychology from the University of Constance and a PhD from Bremen University She joined Hannover Medical School as a research fellow and consultant midwife in 2001 and has set up the Hannover Midwifery Research and Edu-cation Unit In 2010 her postdoctoral thesis (Habilitation) was accepted by the Hannover Medical School followed by becoming a professor in 2015

Dr Gross has a long-standing interest in the academic train-ing of midwives Following an International Congress of Midwives workshop on midwifery research in Tubingen Germany in 1989 she organized and has chaired annual research conferences for German speaking midwives These conferences are attended by midwives and midwifery students from German speaking countries many of whom have been actively involved in research

While she devotes half of her time to research Dr Gross continues to practice part-time as a midwife in the MHH Womens Clinic a tertiary hospital In addition she is in-volved in teaching medical students and has been setting up a programme for advanced midwifery education (MSc) Another focus is around midwife-led models of care

PhD Annette Bernloehr (2010-2015)

Midwife MSc PhD Lecturer

Dr Bernloehr is a trained midwife and has more than ten

years of clinical experience as a midwife She became a

lecturer in the European Master of Science in Midwifery

Programme at Hanover Medical School in March 2010 Her

scientific focus is on antenatal care in the European Union

and on the quality of guidelines Annette Bernloehr became

a professor at Bochum University of Health Sciences in

2015

Ms de Wall is a midwife and teacher of midwifery and holds

an MSc in life long learning She has more than twenty

years of teaching experience At MHH she works in the ad-

ministration of the European Master of Science in Midwifery

Canada She is very interested in obstetrics and hopes to

Susanne Grylka-Baeschlin (since 2013)

Midwife MSc PhD Student

Ms Grylka-Baeschlin is a trained midwife with more than 20

years of clinical experience In 2013 she finished the Euro-

pean Master of Science in Midwifery programme Since

September 2013 she works at the Midwifery Research and

Education Unit as a research assistant mainly for the Opti-

BIRTH study She still works as a midwife in a Swiss hospi-

tal and currently attends the PhD programme Epidemiology

at Hannover Biomedical Research School

Dr Antje Petersen (since 2008)

Dr Petersen was a doctoral student from

2008 until 2012 in the Midwifery Unit

collaboration in the Midwifery Unit by su-

pervising students for their bachelor and

Gabriela Schmitz (since 2010)

RN MA Nursing Pedagogy MSc Nursing Science

Ms Schmitz is a nursing scientist finishing her dissertation

focusing on urinary incontinence She worked as lecturer in

higher education for health professionals at the Chariteacute Ber-

lin and currently at the Protestantic Univerity of Applied Sci-

ence Berlin She is teaching modules focused on midwifery

education at the Hanover Medical School since 2010

Dr Christiane Schwarz (2006-2007 2013-2016)

Midwife MSc

Ms Schwarz is a midwife with a 25-year background in

homebirth and hospital birth as well as community midwife-

ry She practised in Germany UK and New Zealand At

MHH she is responsible for prendash and post-graduate mid-

wifery education She develops educational modules and is

part of the multiprofessional obstetric emergency simulation

training team

Kathrin Stoll (since 2011)

Lecturer PhD

Ms Stoll has been involved with MHH midwifery education and research unit for the past three years in the capacity of part-time lecturer and research collaborator Kathrin Stoll has degrees in psychology and sociology and recently com-pleted a doctorate at the University of British Columbia Dr Stoll teaches advanced research methods to midwifery graduate students and has been involved in academic and community based research for the past 10 years using a variety of qualitative and quantitative research methods She has expertise in rural maternity research pregnancy and birth research and quantitative methods including sur-vey and scale development regression modeling and analy-sis of rural perinatal outcomes using population data Her

primary postdoctoral research project is about perinatal outcomes of women who live in small rural communities

Lesley Woodward (since 2012)

Mrs Lesley Woodward is a practicing midwife and a part-

time lecturer on the European Master of Science in Midwife-

ry Programme She is currently undertaking a PhD with Bel-

fast University Ireland with a focus on obstetric and neona-

tal emergency skills training

Hanna Gehling (since 2015)

Midwife BSc MSc

Ms Gehling is a midwife who worked in antenatal delivery

and postnatal wards in Ireland Germany and Malawi since

graduating in 2006 After her experience as a volunteer mid-

wife instructor and practical supervisor in Africa she decided

to enrich her practical midwifery knowledge with an aca-

demic training She finished her postgradu-

ate studies at Hannover Medical School in

2015 Straight after graduating she started a

position as a research assistant at the mid-

wifery research and education unit as the

programme coordinator of the European

MSc Midwifery as well as leading four mod-

ules as a lecturer

Marina Weckend (since 2015)

Midwife Bsc MSc

Ms Weckend completed her midwifery training in Germany

in 2010 She then practiced as a midwife in both hospital

and community settings in various countries In 2011 she

commenced her MSc Midwifery studies and is expected to

finish these in September 2015 In August 2014 Marina

joined the Babies Born Better (B3) Project and works pre-

dominantly on the B3 User survey She is currently em-

ployed as a research assistant by the Hannover Medical

School where she continues her work on the B3 project as

well as on other studies

Anne Koumlhne (since 2016)

Midwife BSc MSc

Ms Koumlhne is a trained midwife who still works as a midwife

at a large maternity unit in Hannover In 2013 she started

working as a study midwife for the OptiBIRTH study for the

Midwifery Research and Education Unit Since 2016 is a

research assistant on-site She also finished her Masterrsquos

degree in Public Health in 2016

Research - current projects

OptiBIRTH Study

The Europe-wide study started in 2012 for a dura-tion of 4 years in Ireland Italy and Germany Five hospitals (Buumlrgerhospital Frankfurt Henriettenstif-tung Hannover Hannover Medical School Dritter Orden Muumlnchen and HE-LIOS Universitaumltsklinikum Wuppertal) in four federal

states (Bavaria Hesse Lower Saxony North Rhine-Westphalia) are participating in the German part of the Opti-BIRTH study The study examines the rising rates of Caesarean section throughout Europe focusing in particular on routine Caesare-an section following a previous Caesarean birth despite calls for increased vaginal births after Caesareans In comparison

to Caesarean sections vaginal births are associated with lower maternal mortality less overall morbidity for mothers and babies and are the preferred option for the majority of women The study aimed to improve the organisation of ma-ternal health service delivery and optimise childbirth by in-creasing vaginal birth after caesarean by about 15 through enhanced women-centred care Women-centred care entails along with the consideration of evidence-based insights the increased involvement of pregnant women in the planning and organisation of their impending delivery The project through meaningful women-centred care will influence EU health policymaking by increasing VBAC rates in at least three participating countries Ireland Italy and Ger-many httpwwwmh-hannoverde25025html

Antenatal care in the European Union

In 2004 a survey documented and compared the content of national guidelines on antenatal care This study found a marked difference between guidelines from states with a gross national product above average and those states with a more restricted national budged Differences were also observed between the lsquoolderrsquo member states and those which had just joined the Union Until today little is known about the methodological rigour of the national guidelines and how national guidelines develop their content further over time The survey aims at improving knowledge about antenatal

care in Europe and the evolution of national guidelines by detailing the current content of national guidelines on antena-tal care in the member states of the European Union (EU) analysing the changes in each national guideline over the past nine years identifying patterns of care according to dif-ferent geographical financial or other patterns of the mem-ber states and by appraising the development processes of the national guidelines httpwwwmh-hannoverde26973html

Stillbirth - Lancet

Stillbirth is a devastating event for parents families and health care staff Despite significant advances in medical science there has been little reduction in stillbirth rates in high-income countries for over two decades The Lancetrsquos Stillbirth Series which was published in April 2011 aimed to bring attention to stillbirth and create the change needed to reduce stillbirths However there is still a lot of work to do In this study parents care providers and community members from different countries were surveyed This knowledge will help to identify areas where actions can be taken to prevent stillbirths and to improve care for families who have had a stillborn baby Purpose of this study ishellip hellipto learn more about practices knowledge and views of stillbirth from parents who have a stillborn baby

Babies Born Better

A group of experts from the health sector is working on a large project that has the aim of identifying areas of best practice in maternity care in Europe and worldwide This pro-ject is called Babies Born Better It originally grew out of an EU- funded program (COST Act ion IS0907 iResearch4Birth) Now the group operates independently of the EU expanding the project to countries outside the EU In this survey we are trying to find out about womens views and experiences relating to maternity care in a number of different European countries This information will be used to identify areas of best practice in maternity care across Euro-pe The survey addressed to women who have given birth in the last 5 years takes the form of 20 questions which should

take less than 10 minutes to answer The survey is currently available in 19 languages and we have had to date 22928 response from women across Europe and beyond The survey is online and has been promote through soci-al media and via all our networks and contacts Please help us spread this further and reach new women who have not participated thus far The research group aims to disseminate the results of this survey as widely as possible httpwwwmh-hannoverde29963html

Fear of childbirth

In todays society pregnancy and childbirth are no longer considered natural stages of life but as a potential risk The next generation of expectant mothers grew up with an inter-vention-rich birth culture and equates them with the optimal standard treatment (Luyben 2001) Attitudes towards obs-tetric interventions such as epidurals or caesarean section and the fear of childbirth among young non-pregnant adults are shaped by representative experiences (reports of family members and friends or media representations) (Stoll et al 2014) In cooperation with international colleagues we are conduc-ting a survey to learn more about the attitudes of students

towards pregnancy and childbirth The survey is web-based The data are important to help us understand the attitudes and information needs of young adults from various countries towards pregnancy and childbirth Preferences in the area of prenatal care will also be recorded The questionnaire is al-ready being successfully used internationally (in Canada Australia the USA Iceland and England) This project will be carried out at the MHH in the fall of 2014 and is evaluated scientifically by Dr Kathrin Stoll (University of British Colum-bia Vancouver Canada) httpwwwmh-hannoverde30155html

Birthing with confidence

Self-efficacy (confidence) is an established psychological construct that is used in many areas of health care The view on well-being and salutogenesis may serve to not overstate risks Thereby it is possible to consider pregnancy and birth less as pathological but much more as normal physiological events This applies particularly in strengthening the compe-tence of (expectant) mothers For this instruments that adequately measure physiological settings are necessary The questionnaire for obstetric self-efficacy the Childbirth Self-Efficacy Inventory (CBSEI) may provide the impetus to deal specifically with birth and possible coping strategies

Womens confidence in their ability to cope with positive birth can be recorded and where appropriate strengthened More confident women give birth with less intervention and more often achieve spontaneous delivery It would be ideal if the CBSEI would be completed by every woman aspiring to vagi-nal delivery in the third trimester The most important aspect of the study however is the study of the association between mode of delivery and self-efficacy a so-called soft factor This has not previously been done httpwwwmh-hannoverde29957html

The Midwifery-Literature-Service (HeLiDi) is an abstract-based supplement in the field of midwifery and obstetric research Aiming to decrease misunderstandings based on language barriers English abstracts are translated into Ger-man to reduce language barriers regarding current interna-tional obstetric literature The data base contains approxi-mately 100 translated articles The supplement has been

published in four midwifery magazines in Germany Austria and Switzerland twice a year since 1992 For the 15th anniversary of the Midwifery-Literature-Service the Unit provided an index of contexts of all published issues on our homepage httpswwwmh-hannoverde13098htmlc37932

Research - completed projects

Pro Geb Study

In 2005 the German Research Council awarded the Han-nover Midwifery Unit a two-year research grant for the study ldquoProcess-centred care and well-being during low-risk-childbirthrdquo This study investigated the effects of obstetric interventions on the course of labour and on intrapartum maternal well-being in low-risk women We examined the hypothesis that the dynamics of labour are determined more by intrapartum variables than by pre-existent factors such as parity place of birth or predicted birth weight We used an instrument designed especially for this purpose to record data from 4438 women in our assessment All intrapartum variables were recorded with respect to timing during labour We collected data in differ-ent obstetric settings including hospitals birth centres and Because our survey instrument referenced each in relation to timing we were able to assess how the attendance of

midwives and doctors as well as intrapartum proce-dures and events affect the process of labour and the event of birth (ie birth out-come) The same data also allows us to analyse the amount of caregiver in-volvement during the process of birth

German Research Council Process-centred Care and Wellbeing during Low-risk-Childbirth

This project investigates the effects of obstetric interventions on the course of labour and on maternal wellbeing during labour in low-risk women With an instrument that is desig-ned especially for this purpose complemented with data from the women themselves we examine the hypothesis that the dynamics of labour are determined more by intrapartum variables than by pre-existent factors such as parity place of birth or infant birthweight The first phase of the project is devoted to the collection of accurately timed intrapartum

wifery education between 2002-2005 2 a guideline for their continuing education program

Childbirth Cultures Concerns and Consequences Creating an EU framework for optimal maternity care

Around 47 million European women experience childbirth

annually Optimal maternal and infant health is critical to

societal well-being Survival rates have improved but there

are now concerns about iatrogenic morbidity There are sig-

nificant cross-EU differences in maternity care cultures phi-

losophies organisation uptake and outcomes This Action

will advance scientific knowledge about the ways of improv-

ing maternity care provision and outcomes by examining

what works for whom in which circumstances and by iden-

tifying and learning from the best It will share synthesize

and extend the work of experts from a range of disciplines

using a common framework of salutogenesis and complexity

theory The work will include an examination of first tri-

mester prenatal diagnosis routine intrapartum interventions

for low risk women and care for migrant women It will focus

on the rapid dissemination of

new knowledge to improve

European competitiveness in

this area Meetings Short-Term

Scientific Missions Workshops

Training Schools and Conferences will catalyse effective

collaborations particularly for early stage and women re-

searchers Bringing all maternity care in Europe up to the

standard of the best is the ultimate aim of the European

Research Network that will be a result of the Action in the

long term


COST Action

In collaboration with universities in Academie Verloskunde MaastrichtZuyd University and Haute Eacutecole de la Santeacute Lau-

sanne HES-SO the MRE Unit currently offers a flexible study course toward an in-

ternationally accepted Master of Science in Midwifery Supported by the Erasmus

EU-Life-Long-Learning-Programme the MSc curriculum aims to provide European

midwives with a stimulating academic learning environment based on the values of

academic openness and critical appraisal The MSc programme is being developed

and implemented following the Declaration of Bologna International Confederation

of Midwives and World Health Organisation (Europe) educational standards

The target student group includes qualified midwives throughout Europe The ex-

pected output are midwives qualified at the Mastersrsquo level in their own discipline and

who challenge aspects of midwifery practice and policy within Europe They will re-

trieve organise synthesize and evaluate evidence as the basis for development and

delivery of midwifery throughout Europe

In the programme midwives will evaluate the role of the midwife in contributing to

womenrsquos health in their own countries and throughout Europe They will design man-

age and evaluate a research project relating to midwifery through the process of in-

dependent study They will also critically appraise the application of new knowledge

and perspectives in maximising an effective European midwifery service in line with

an evidence-based-practice approach to care This programme will qualify midwives

to work in leading positions conduct midwifery research midwife training and work

at the forefront in new emerging occupational fields within midwifery

Building Intrapartum Research Through Health - an interdisciplinary whole system approach to under-standing and contextualising physiological labour and birth (BIRTH)

Optimal maternal and infant health is critical to societal well-being Reducing childbirth mortality and severe morbidity is a primary concern for most governments However this focus on pathology has been associated with an over-extension of clinical interventions to low risk women with

unexpected adverse clinical consequences and rising health care costs Part of the problem has been a scientific focus on understanding pathologies of pregnancy and childbirth from simple clinical linear perspectives with a consequent lack of understanding of the range and limits of normal childbirth physiology in different populations individuals and contexts The proposed Action will advance scientific knowledge in this area from a whole-systems perspective using the realist research framework of what works for whom in what cir-cumstances httpwwwmh-hannoverde31507html

Foreword

With the 15th anniversary of the Midwifery Re-

search and Education Unit I realised that I have

seen a dream come true the establishment of per-

manent midwifery research and education group

with collaborations at the local national and inter-

national level I am therefore pleased to offer you

the Research Review of the Midwifery Research

and Education Unit at the Hannover Medical

School which demonstrates our activities and the

people behind them

I have personally been interested in advancing research and continuing edu-

cation for midwives in Germany since at least 1989 when I attended an Inter-

national Congress of Midwives workshop on midwifery research in Tubingen

This experience compelled me to organise and chair annual research confer-

ences for German-speaking midwives and eventually to lead this research

unit dedicated to midwifery research The current participation in the Europe-

an Master of Science in Midwifery programme the supervision of doctoral

students as well as original research both supports young midwife scholars

and the profession while producing evidence to improve maternity care na-

tionally and internationally

Our research in midwifery and maternal health is diverse and our units

scholars have covered topics ranging from the influence of womenrsquos self-

diagnosis of labour progress to the current situation of midwifery research

and education in several regions I invite you to learn more about our work

and scholars in this report
