Placement F1 – General Medicine (Clarke)The department Relatively small department run by two Consultants. There are

facilities for doing echocardiograms including trans-oesophageal and stress echoes. There is a cardioversion list each week.

Consultant ward rounds every day to see high acuity patients. General house officer jobs such as liaising with relatives and multidisciplinary team members. The team are very good at teaching on the ward round and organising other teaching sessions if you are interested.

Main duties of the placement

Ensure the ward round runs smoothly and the required information is available. Organise investigations, communicate with other members of the MDT, and assess unwell patients. We have lots of medical students, both 3rd and final years, so some time each day is spent teaching them and helping them see patients.

Main duties of the placement Day to day job involves ward work including documentation of ward rounds, prescribing medications and fluids, ordering tests and imaging, arranging other speciality involvement where required, reviewing tests and imaging. Also reviewing any sick patients and implementing appropriate treatment.

In addition to this the Endocrine job has Wednesday morning endocrine testing clinic where you do tests such as the short synacthen, insulin tolerance, glucose tolerance etc testing with the specialist endocrine nurse. There are opportunities to go to general medical, endocrine or diabetic clinic.

On-call role includes acute admissions where you do any jobs required for newly admitted patients. There are some opportunities to clerk patients. Ward cover is also part of this being called to review patients on the wards, prescribing, actioning and reviewing tests ordered by the usual day team and implement appropriate action in light of the results. You also hold the crash bleep and will be called to any cardiac arrests.

Placement F1 - Elderly Care CliftonThe department Clifton is a rehabilitation ward with patients that often have

complex needs, multi-morbidities and recent acute medical or surgical issues. The patient group is predominantly older patients although some are younger with specific rehabilitation goals. The ward is managed by two consultant geriatricians.

Daily multi-disciplinary meetings (MDTs), daily review of rehabilitation referrals for Clifton, ward rounds and ward jobs.Learning opportunities through consultant ward rounds, alternate weekly radiology lunchtime meetings, Elderly Care teaching and Grand Round. There are also opportunities to

medicine ward. There are two consultants who look after half the ward each. The job also involves cover for the elderly care patient outliers on McGill, EMAU on a rotational basis with full senior support.

Main duties of the placement

Typical working pattern in this post involves consultant ward rounds Monday/ Wednesday/ Friday mornings and SpR/CMT ward rounds Tuesday/ Thursday mornings. The ward round includes the admissions unit for new patients and outlying patients under the gastro team on other wards including surgical wards, the paediatric ward and ICU.

Afternoons as a HO involve actioning jobs from the mornings ward round including ordering investigations, chasing results, making referrals, discharge summaries, procedures and liaising with other members of the MDT. In addition to this any patients that become unwell on the ward are reviewed and discussions with relatives for collateral histories/to update on management plans are common. Procedures include chest drains, ascitic drains/taps and central lines. New patients on the admissions unit also need to be reviewed in the afternoon. On Tuesday and Thursday afternoons patients attend for venesection to the discharge lounge and these patients must be seen in addition to the ward patients.

The type of work to expect and learning opportunities

Ward based work. Daily ward round with SHO or on your own if SHO is away. Looking after the patients medically pre and post op. Trauma meetings every morning where the F1’s present the new patient’s x-rays and there are opportunities for seniors to teach. There are many opportunities to go to theatre and clinics if you so wish.

The type of work to expect and learning opportunities

The work is mainly ward based. It mainly involves a ward round followed by ward jobs e.g. blood taking, writing TTOs etc. You are a solo house-officer so are required to assess patients and problems as they arise on the ward. There is plenty opportunity for attending theatre, and practicing urological skills e.g. 3-way catheter insertion. Urology is a fairly busy placement but there plenty of learning and medical student teaching opportunity and the team are supportive and approachable.

Placement F1 - Respiratory MedicineThe department The respiratory medicine department is led by three respiratory

consultants supported by specialist respiratory nurses and pulmonary function technicians.

Aside from daily inpatient care, there are regular bronchoscopy and thoracoscopy lists and outpatient clinics at both RHCH and Andover Hospital. Patients attend as day case for procedures including venesection and lung biopsy. The team is working to build links with the community, in particular with a community based integrated respiratory serviceThe team consists of 3 consultants, 2 Specialist registrars ( 1 training in respiratory medicine , 1 training in acute medicine), 2 core medical trainees and 3 FY1s.

Main duties of the placement

Daily consultant ward rounds ( Monday to Friday)Daily job lists will be created on the rounds, and jobs allocated to each team member and prioritized. The rest of the day consists in ensuring each task is complete and outstanding tasks or results are fully followed through before the next consultant ward round.

Jobs may include, clerking elective admissions, perfoming practical procedures eg blood gases, discharge summaries, speaking to relatives, liaising with other teams, arranging investigations and checking blood tests and results. House officers are also in charge of creating the patient list before each ward round every morning.

Standard day: 0900-18.00

On call: weekdays 9.00-21.30

Weekends: 9:00-21.30 either ward cover or Medical
