Guidelines - Electronic enrollment For students enrolling to 2 nd , 3 rd , 4 th , 5 th and 6 th year of studies Enrollment consists of few steps: Electronic o Validation request o Registration of eligible and compulsory subjects In the study department o Submission of registration list and index in the FSAD office It is important that you follow (and put in practice) all of the following guidelines one after another without jumping ahead. 1. Make sure that you have fulfilled all of your study obligations and that both your SIS and your index are completed Summer clerkship is also considered a compulsory subject!! State exams are excluded from this – you may request validation of your results regardless whether or not you have passed your state exams in that particular academic year. 2. Request a validation of your study results. You can do that in your SIS in the SUMMARY OF EXAM RESULTS, menu VALIDATIONS. Electronic notification will be sent to study department which will confirm or decline your validation in the SIS as well as via email.

Guidelines - Electronic enrollment

For students enrolling to 2nd, 3rd, 4th, 5th and 6th year of studies Enrollment consists of few steps:

• In the study department o Submission of registration list and index in the FSAD office

It is important that you follow (and put in practice) all of the following guidelines one after another without jumping ahead.

3. Wait for an email from study department. If your validation is confirmed you will receive an email asking you to pick the group you want to be in in your next year of studies. In case of validation decline, you will be informed why. Should your validation be confirmed continue to the next steps, if not, resolve before continuing!

4. Pick a study group 2nd to 4th year students will email the decision to Michaela [email protected] , 5th and 6th year students to Jitka [email protected] By this time you should be able to see the validation as confirmed in the Examination Results section in your SIS

(to be able to see it you will need to set the system setting to academic year 2016/17 through the SETTING button in the top green task bar of the SIS main screen)

5. Make sure your personal details in the SIS are correct and up to date. Do that in the Personal Data section in your SIS (through the data change icon in the top green task bar)

6. Print out the register list (registration form). Also to be found in the Personal Data section. Please make sure to fill in and update all your personal details before printing the register list. Keep it safe until the day of your enrollment in the FSAD office.

7. Enroll for the elective subjects for the upcoming year. Enrollment of elective subjects for winter semester 2016/17 is open from 14.7.2016 for students who fulfilled

all study obligations of the academic year 2015/2016 (summer semester eligible subject enrollment will be open from 1.2.2017 till 1.3.2017).

Copy the subjects you have enrolled for from your SIS into your index. In your index, start on brand new page – filling in the left-hand-side of it with name of the study subject and its

guarantor per line, leaving the right-hand-side of the index book empty for your classification and teacher’s signatures!!

10. Plan your enrollment in Foreign Students Affairs Department. Either come to your regular enrollment day

2nd year - 26.9.2016 3rd year - 27.9.2016 4th year - 29.9.2016 5th year GM - 30.9.2016 5th year Dentistry - 15.9.2016 6th year - 22.9.2016
