Administrative Approval and Financial Sanction (AA&FS) is hereby accorded for an amount of

Rs. 2,25,00,000.00 (Two Core Twenty Five Lakh) only in favour of different District Health &

Family Welfare Samities (DH&FWSs) for execution of works under the activity heads of

Comprehensive Primary Health Care [i.e.,Infrastructure Strengthening of UPHC to H&WC NUHM

(Tally Code P.04.03.02.B) and IEC activities H&WCNUHM (Tally Code P.04.06a)].

2. Accordingly, Rs. 2,25,00,000.00 (Two Core Twenty Five Lakh) only is hereby released in

favour of respective District Health & Family Welfare Samities (DH&FWSs) as per district wise

breakup of allocation given below for up gradation of existing infrastructure of the U-PHC,

procurement of equipment of U-PHCs and branding & IEC related activities for implementation

of Comprehensive Primary Health Care under NUHM.

3. The detailed ULB& activity wise breakup of allocation is enclosed in Annexure-A

4. An indicative guideline for utilization of the allotted fund is enclosed in Annexure-B.

5. All concerned District Health & Family Welfare Samitis (DH&FWSs) will give necessary

support and assistance to their respective ULBs to utilize this fund as per indicative guideline.

This fund may not be devolved to ULBs.

6. Sr. Accounts Officer, NHMwill act as Drawing & Disbursing Officer for the purpose.

7. The expenditure will be incurred under different activity heads as per norms of NUHM and to

be booked under respective Tally Codes (i.e., P.04.03.02.B & P.04.06a).

8. This order has been issued with the approval of Mission Director, NHM & Secretary, Health &

Family Welfare, Government of West Bengal.

Indicative guideline for utilization of funds for Comprehensive Primary Health CareServices under NUHM

);> Infrastructure Strengthening ofUPHC to H&WC (Tally Code P.04.03.02.B)- Fund allotted

under this activity head is divided into two major categories which are noted below. This

allocation has been sanctioned as one time grant for the selected Suswasthya Kendras (Health &

Wellness Centers)

i) Up-gradation ofU-PHCs as Suswasthya Kendras (Health &Wellness Centers)

./ Minor renovation work if necessary

./ Ramp for the severe patients, pregnant women physically challenged and elderly

person

Decoration for patients waiting lounge with still or iron chair and other amenities

like supply of drinking water

Rack and Almirah for the storage of medicines, registers and other essential

consumables of the U-PHC

./ Foot steps

./ Screen for privacy of the patients and pregnant mothers

./ Child friendly immunization room

./ Setting up of NCDcorner where NCDcases address separately

./ Wheel Chair

./ Office Chair

./ Doctors chairs

./ Mattress for bed

./ Notice board

ii) Equipment for U-PHCs- This fund will be utilized for the procurement of following

essential equipments of Suswasthya Kendras (Health &Wellness Centers) as furnished in

Table No. I & Table No. II in accordance with the Memo No. HFW-27011/163/2019-

NHMSEC -Dept. ofH&W /2992, dated: 05/09/2019.

In addition, this fund may be utilized to meet up the gaps in the existing laboratory

equipment of the U-PHCs as per modalities provided by the State in accordance with

the Memo No. HFW/NUHM/01/2016/13, dated: 04/01/2016.
