As per Minisrry letter No. S-1104513612012-CGHS (HEC) dt. 16-10-15 and subsequent revised guideline

for HCOs allor.ving benefit of continuous empanelment scheme vide order No. S.1 104513612012-CGHS(HEC) dt.

26-12-20lT,adverlisernent was given in the local paper as rvell website on dt. 13th March 2018. A Committee was

formed to securitize the papers submitted by the new HCOs for empanelment. On 28th March the committee,

comprising of Dr. Kiran Singh CMO-l/c (NFSG), CGHS WC DD1 D.Dun and Dr. AnilRana, SMO, CGHS WC

DD2 D.Dun, detebrated the papers submitted by the different HCOs. Only three hospitals, Kailash Hospital

Haridrvar Road Dehraclun, M M Naithani Eye Centre, 6l l4 Pritam Road, Opp Doon Inter National School

Dalanwala Dehradun & Vinod Ortho & Genesis IVF Clinic, 6-lnder Road, Dalanwala Dehradun were found

fulfilling the tencler conclition except that none of them was having QCI recommendation.

Subseque,tly M.M Natliani Eye Centre & Vindo Ortho Center & Genesis tVF Clinic submitted the necessary QCI

ceftificate. Hence A team was formed with following member:-

1. Dr. B S Kala, CMO -llc (SAG)' CGHS W.C DD2 D.Dun

2. Dr. Mrs l(iran Singh. CMO (NFSG) CCHS WC DDI D.Dun

3. Mr. Sanjeev Baloni, AD Office CGHS D"Dun

4. Mr. Dhananjay Singh, CGHS WC DDI D.Dun

The Conrrnittee visited the two HCO's M M Naithani Eye Centre,6'7 l4 Pritam Road, Opp Doon lnter National

School Dalanrvala Dehradun and Vinod Ortho & Genesis IVF Clinic, 6-Inder Road, Dalanwala Dehradun on 10'1'

September 20 l8 and inspected i authenticates the document submitted by the HCO's.

The list of qualified HCOs in accordance to category and purpose is attaclred as given below on the recommendation

of eCI & Hospital cornmittee for empanelnrent under continuous empanelment scheme upto Sept 2018 or next

empanelrnent rvhichever is earlier.

The Hospital who have qualified to be empanelled under CGHS Dehradun have submitted the Draft MoA along

with acceptance letter ancl perfonnance Bank Guarantee, are approved for empanelment under CGHS Dehradun

r,v.e.f the date of notificatiorr.

The empanel.ient shall be fbr a periocl upto 30'l'Sept 20 18 fbrm the clate of notification or till next whichever is

earlier.
