Access the Googledocs homepage, create a Google account, and login

Go to the “New” button and click “Spreadsheet”

Click the “Share” button and save spreadsheet

Click the “Form” button

Click the “Create a Form” link

EXERCISECreate your own form based on the information below!

You want to collect the following school summary data from theelementary schools in your locality:

• School Name• Submitter Name (Full Name of the Person Submitting this Data)• Age Group Being Reported (possible ages 10-12)• Aerobic Capacity # Tested – Boys• Aerobic Capacity # met wz – Boys• BMI Average – Boys (Average for age group)

Once you have created and saved your form (use your last name to title your save), go to the form link under preview and enter in some

data. Go back to the spreadsheet to see howit looks!
