Nurse-Family Partnership (NFP)
An evidence-based community
health program that:
Partners registered nurses with first-time mothers;
Changes the future for
vulnerable babies born into
poverty, and their families;
Addresses social determinants
of health.

The conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.
