    Q1. List 3 essential features of a Permit to Work System.

    A formal written system used to control certain types of work which areidentified as potentially hazardous.

    It is also a means of communication between site managers, supervisorsand those who carry out the work.

    It aims to ensure that proper planning and consideration is given to the

    risks of a particular job.

    The permit is a written document which authorises certain people to carryout specific work, at a certain time and place.

    It sets out the main precautions needed to complete the job safely.

    Q2. Give two examples of when a Permit to Work would be applicable.

    When carrying out work which may adversely affect the safety ofpersonnel, the environment or the plant.

    Normally considered to be more appropriate to non-routine activities whichmay require some form of JSA prior to the work commencing.

    Whenever there is a need to co-ordinate activities to ensure that the workis carried out safely.

    PEC also specifies activities that are subject to PTW systems.

    Q3. List 4 responsibilities of the Issuing Authority.

    Understand the nature of the work.

    Has identified ALL the hazards associated with the job.

    Has ensured that ALL the necessary precautions have been implemented,including isolations, BEFORE the work begins.

    Ensures that ALL people who may be affected by the work are informed

    before the work begins, when the work is suspended and when the work iscomplete.Cross references Permits for tasks that interact.

    Ensures that Effective arrangements are made for the work site to beexamined before the work begins, on completion of work and asappropriate when work is suspended.

    Ensures that sufficient time is available on shift handover to discuss allongoing or suspended permits with oncoming Permit Issuer.

    Q4. List 4 responsibilities of the Performing Authority.

    They have received training in the PTW system as applied in that particularlocation.

    Their work group has received adequate instruction in the system.

    They discuss the job fully with the person issuing the permit.

    The permit is posted at the work site.

    The work group is briefed FULLY on the details of the permit including anypotential hazards, and on ALL the precautions taken or to be taken.

    The precautions and conditions of the permit are maintained throughoutthe work activity.

    Work group are aware and understand that if circumstances change, workmust be stopped and advice sought.

    The work group stays within the limitations set on the permit. (Physicalboundaries, types of work and validity time)

    On completion or suspension of the work, the site is left in a safe conditionand the permit issuer is informed.

    Q5. List three responsibilities of the Permit to Work Coordinator.

    Assist with control and communication of all permits.

    Maintains a permit register.

    Formal handover PTW to Performing Authority (P.A.) after the specifiedinspection of site by P.A. and I.A.

    Receive all PTW on expiry date.

    Maintain file of all PTW, associated certificates and documents.

    Q6. Gas testing, as part of a PTW preparation, should be conductedwhen?

    As close as possible to the commencement of work.

    Q7. Who gives the authority to apply and remove and isolations?

    Q8. Permits to Work should be displayed where?

    At the work site. Where this is not practicable (e.g. at an exposed location)the person in charge of the job should retain a copy on his person, havingensured that the work party members are familiar with its content.

    At the main control/coordination room, where they should be displayed ina systematic arrangement.

    Where the permit issuer is remote from the control/coordination room, heshould have a copy of the permit.

    Q9. Working WITHOUT a valid Permit to Work constitutes a what?

    A serious NEAR MISS.

    Q10. On completion of work, what action must the Issuing Authorityand Performing Authority complete prior to signing off the PTW.

    An inspection of the worksite to ensure that it has been left in a safecondition.
