Our Department’s many subspecialtyprograms continue to grow, and the WeillCornell/NewYoYoY rk-Presbyterian Center fofof r the Perfrfr ofof rmingArtist has helped a large number of perfrfr ofof rming artists inNew YoYoY rk City. We have exciting new programs andleadership opportunities fofof r our faculty, which arefeatured inside. In particular, I am pleased that aftftf er anational search, Dr. Sam Selesnick was named the nextEditor-in-Chief of The Laryngoscope journal, whichmeans that our specialty’s premier journal will stay in theDepartment.

We are doing many surgical cases at the Hospital’sstate-of-f-f the-art ambulatory center, the David H KochCenter. The technology and the patient experience aresecond to none, and this new Center has been a greataddition fofof r our patients, faculty and residents.

Our residency and fellowship programs are also doingextremely well, and we continue to benefit from ourunique collaborative relationship with the Department ofOtolaryngology-Head & Neck Surgery at ColumbiaUniversity’s College of Physicians and Surgeons, whichis also growing under the leadership of ChairmanDr. Larry Lustig.

Global Reach

“We exist within a medical school and ahospital that have extensive globalreach,” says Dr. Michael Stewart,Chairman of the Department. “Manyinternational patients come to New Yorkto be cared for in our hospital, and ourDepartment has also hosted severalinternational Otolaryngologists fromcountries such as France, the CzechRepublic, Greece, the Canary Islands,Mexico, Uzbekistan, Ukraine andRussia.”

One of the reasons Weill Cornell enjoys aworldwide reputation is the outreach workthat its clinicians are doing in othercountries. “We have collaborations on sixcontinents,” Dr. Stewart explains. WeillCornell has founded successful healthcare initiatives in developed countries likeQatar, Brazil, Taiwan and South Korea,and also in resource-poor countries likeHaiti and Tanzania.

In addition to Weill Cornell Medicine-Qatar — the first American university tooffer an M.D. degree overseas — theDepartment has also collaborated withthe ENT Department at Hamad MedicalCorporation in Qatar, with visitingprofessors giving lectures, offeringpatient consultations and providingassistance with surgery. Hamad MedicalCorporation achieved ACGMEInternational accreditation for theirotolaryngology residency, making themonly one of a handful of ACGME-Iaccredited program in the world.

For many years, the Department hashosted two annual seminars in Austria

(one in temporal bone surgery, and asecond in otolaryngology) as part of theSalzburg Weill Cornell Seminars, whichfocus on clinicians from former EasternBloc nations. Sponsored by the AmericanAustrian Foundation, the two SalzburgWeill Cornell Seminars are havingsignificant real-world impacts: “A doctorfrom Mongolia learned about cochlearimplants and went back to Mongolia tostart the first cochlear implant program inMongolia,” Dr. Stewart explains. “She gotthe idea when she was at the Salzburgseminar.”

The Department also has close ties tothe city of Paris, where Dr. SamuelSelesnick, Vice Chairman of theDepartment, is the Senior US MedicalCounselor at the American Hospital ofParis and is improving academiccoordination between the two institutions.And at the city’s Institute Arthur Vernes, aresident exchange program allowsresidents from that hospital to do electivetime with Weill Cornell, whose residentscan also visit the Paris hospital.Additionally, the Department is activelyexploring possible collaborations withotolaryngology departments in China.

Individual faculty members are alsoextending the global influence of theDepartment. “Many of our faculty areknown internationally and get invited tobe visiting professors around the world,”says Dr. Stewart. Faculty members havemade presentations in Guangzhou andHong Kong, China; Tokyo, Japan;Zagreb, Croatia; Cartagena, Colombia;Stockholm, Sweden; Mexico City,

Among the faculty members active ininternational research and education, Dr.Selesnick is also the General Secretary ofthe Politzer Society, the internationalsociety fofof r Otology. In that role, he isresponsible fofof r helping to organize theirinternational meetings.

Dr. Maria Suurna, Assistant Professor inthe Department of Otolaryngology, hasbeen named the Regional Advisor fofof rEurope of the American Academy ofOtolaryngology-Head and Neck Surgeryand Dr. Joseph Montano, Chief ofAudiology and Speech LanguagePathology, has been named to the WorldHealth Organization’s Working Group onthe Rehabilitation CompetenciesFramework as part of “Rehabilitation2030: A Call fofof r Action.”

These worldwide initiatives enhance WeillCornell’s expanding global reach. “We arefofof rtunate to be in an institution that givesus great international opportunities, and tohave faculty who are internationallyknown” says Dr. Stewart.

Sentinel Node Mapping Using Fluorescent Targeted SilicaNanoparticles

A new intraoperative procedure formapping sentinel lymph nodes during headand neck cancer surgery is the focus of aclinical investigation now being conductedby Weill Cornell Medicine in collaborationwith Memorial Sloan-Kettering CancerCenter and NewYork-PresbyterianHospital.

The use of technetium-99m sulfur colloidwith or without blue dye in sentinel nodemapping is well established, but there areradiation safety protocols and otherlimitations to its use. Nonetheless, “it’s veryimportant from a prognostic standpoint anda treatment standpoint to know whichlymph nodes need to be treated,” explainsDr. David Kutler, Associate Professor ofOtolaryngology and primary investigator inthe new research.

Dr. Kutler’s new research initiative involvesa different type of sentinel node mappingusing injections of targeted silicananoparticles known as cRGDY-PEG-Cy5.5-C dye-labeled particles (dots). Thisimaging agent was developed by Dr.Michelle Bradbury, Director ofIntraoperative Imaging and Dr. SnehalPatel at Memorial Sloan-Kettering CancerCenter. The fluorescent nanoparticles canbe injected intraoperatively and are takenup by the lymphatics in about one half-hour.

For the Phase II clinical investigation,patients will be injected with technetium-99m sulfur colloid prior to surgery as partof their standard of care, and those imageswill be acquired about two hours later usinga gamma probe. Additionally, each patient

will receive injections of cRGDY-PEG-Cy5.5-C dots in four quadrants aroundtheir tumor about 30 minutes prior to theirsurgical operation. Imaging of this agentusing a hand-held camera system andvideo monitoring will continue throughoutthe biopsy procedure.

The potential advantages to using thesetargeted silica nanoparticles are numerous.Because they are fluorescent, real-timeimaging with a hand-held fluorescenceimaging camera can be used duringsurgery.

Additionally, the use of the targeted silicananoparticles obviates the need for theprecautions typically associated withtechnetium-99m sulfur colloid or otherradioactive tracers, so their use “might besafer because you don’t have to exposethe staff or the patient to radioactivity,” Dr.Kutler adds. “It’s also potentially moresensitive in finding the lymph nodes.”

The clinical investigation is currentlyrecruiting up to 60 patients withmelanoma and oral cavity squamous cellcarcinoma and is expected to last abouttwo years. Due to the novelty of usingtargeted silica nanoparticles fofof r oralcavity cancers, “it’s not yet approved bythe FDA fofof r this particular purpose. It’sbeen used fofof r other things — mostly fofof r

melanoma — but not fofof r head and neckcancers,” Dr. Kutler explains. “It’scompletely new. It’s not being doneanywhere else.”

Craniofacial Program Since 2012, when the Department ofPediatric Otolaryngology assumedresponsibility for the craniofacial programat Weill Cornell Medicine, the Cleft andCraniofacial Team has experiencedimpressive growth. “We’ve built it into amultidisciplinary program and receivedaccreditation from the American CleftPalate-Craniofacial Association (ACPA),”says Dr. Vikash Modi, Co-Director of theCleft and Craniofacial Team.

“Before 2012, the team saw less than 30patients a year and performed fewer than10 craniofacial surgeries annually. Now wesee over 100 patients annually, and we’reperforming over 70 craniofacial surgeriesannually. We are now a regional referralcenter and are attracting patients fromother institutions as well as overseas.”

The Cleft and Craniofacial Team’s growthhas been matched by an excellent surgicalsuccess record and use of innovation.“Palatal fistulas can be a concern in cleftpalate surgery, but our fistula rate is lessthan five percent, one of the lowest fistularates in the country,” Dr. Modi says. “OurCleft and Craniofacial Center is alsounique in that we are the first institution toimplement polysomnography-guidedmandibular distraction, eliminating theguess work, and introducing precision withdistraction.”

Helping families through a trying periodcan begin before birth by providingprenatal consultations. “Whenever anultrasound is suggestive of a cleft lip or acleft palate, we meet the parentsprenatally to provide support and preparethem for the road ahead ” says Dr. Modi.

Much of the team’s success can beattributed to the comprehensive array ofspecialists now housed under one roof atthe Pediatric Cleft and Craniofacial Centeron East 72nd Street in Manhattan. “Wehave a fantastic team which includes facialplastic surgeons, pediatric sleep medicinespecialists, oral surgeons, orthodontists,speech and swallow therapists,neurodevelopmental pediatricians,neurosurgeons, prosthodontists, pediatricdentists and otolaryngologists — it’s a biggroup of people who all work together for acommon goal,” says Dr. Modi.

Additionally, an estimated 20 percent ofchildren who have cleft palate surgery willneed specialized speech therapy toaddress velopharyngeal insufficiency(VPI). “Our Pediatric Cleft and CraniofacialCenter is unique in that we have a VPIcenter within our team that provides carefor kids with VPI speech problems,” Dr.Modi adds.

A team of dedicated professionals needs acoordinator who can keep everyonemoving ahead. Amanda Neilan, NP, who isalso a pediatric nurse practitioner, is theclinical coordinator for the Cleft andCraniofacial Team and is responsible forproviding continuity of care among thedifferent specialties and helping patientsand their families understand whichappointments are upcoming and how toprepare for them. For families andchildren facing complex medicalprocedures, “its often overwhelming,” saysDr. Modi. “It can be challenging to navigateall the different caregivers and specialists.A clinical coordinator provides support forthe families and creates a comprehensiveroadmap for their care.”

The program achievedtremendous growth afterthe team receivedaccreditation.

Sean Parker Fellowship The Sean Parker Fellowship in Laryngology, now in itssecond year, is a unique two-year training program that was established to satisfy an unmet need inlaryngological research.

“As laryngology matures as a subspecialty,clinicians trained to a high level in clinicalinvestigation become more and moreimportant,” explains Dr. Lucian Sulica, theSean Parker Professor of Otolaryngologyand Director of the Sean Parker Institutefofof r the VoVoV ice. “The poorly-received 2009Clinical Practice Guideline fofof r Hoarsenessdid servrvr e to highlight the lack of highpowered clinical research to infofof rm patientcare in our field.” TaTaT king on the task oftraining individuals to carry out such workwas a natural fit fofof r The Sean ParkerInstitute, combining its missions fofof rresearch and education. “In the long term,of course, it also servrvr es our primarymission, which is patient care,” observrvr esDr. Sulica.

The Sean Parker Fellowship grantsparticipants a Master of Science degree inClinical and Translational Investigation, inaddition to the usual clinical training. “TheSean Parker Fellowship is an endowedfellowship unlike any other in the country,”says Dr. Sulica. “It’s conceived fofof r peoplewho want to be future thought leaders inour field. We don’t expect it to appeal toeveryone - only a subset of the candidatepool in any given year gets excited by theopportunity, but this is fofof r them.”

A commitment to that research work drivesDr. Keith Chadwick, the first holder of theSean Parker Fellowship. His Masters’projojo ect examines KTP laser and itssuccess rate in treating benign vocal fofof ldlesions in awake patients in the offfff ice,when compared to traditionalmicrolaryngoscopy surgery. Regarding theKTP laser, Dr. Chadwick says, “It’s becomewidely used, driven by convenience andthe ability to avoid general anesthesia,despite the lack of data to show that resultsare truly equivalent to microsurgery. Even

small difffff erences may be of consequenceto some patients, like the many perfrfr ofof rmerstreated at the Sean Parker Institute fofof r theVoVoV ice.”

Indeed, KTP surgery has not been thesubjbjb ect of prospective, comparativeevaluation. Dr. Chadwick is enrollingpatients fofof r such a study. “We’re fofof llowingthem over a certain amount of time toassess the appearance of the surgical site,their voice quality, and how their voice isfunctioning fofof r them and then comparingthose outcomes,” Dr. Chadwick explains.“It’s a fundamental kind of study, but ithasn’t yet been done.”

As a fofof rmer actor and singer himself, Dr.Chadwick brings an insider’s perspective tohis work with perfrfr ofof rming artists. “I did a lotof perfrfr ofof rming when I was younger, then Itook a year offfff befofof re I started med school,and during that time I did someprofessional musical theater perfrfr ofof rmance,”he explains. “I think it gives me someunique insight with our perfrfr ofof rming voicepatients. And I feel like I can connect withthem and their concerns a little bit betterbecause of that.”

National Leadership Update

Michael Stewart, M.D., M.P.H., has been appointed the Executive Vice President ofthe American Rhinologic Society (ARS), where he will serve as the organization’s mainoperating officer. Dr. Stewart was appointed by the board of the ARS — the principalprofessional association in the United States for rhinologists — for a renewable term ofthree years. Additionally, he is a Past President of the American Board of Otolaryngology -Head and Neck Surgery, where he was the Director of the Board for several years.

Samuel Selesnick, M.D., has been selected to be the Editor-in-Chief of the journal TheLaryngoscope following a year-long search. First published in 1896, The Laryngoscope isthe journal of The Triological Society and remains the leading source for information onnew developments in the diagnosis and treatment of head and neck disorders. Dr.Selesnick is also a Past President of both the American Otological Society and theAmerican Neurotology Society. Dr. Selesnick succeeds Michael Stewart, M.D., M.P.H. whoserved as the publication’s Editor-in-Chief for 8 years.

The physicians in the Department of Otolaryngology atWeill Cornell Medicine are recognized internationally asleaders in their field. Many serve in prominent positionswithin professional medical associations, where theycontinue to set the standards for research, educationand clinical care.

Anaïs Rameau, M.D., has been awarded a CTSC KL2 Career DevelopmentAward for the proposal, “Bedside aspiration screening using artificial neuralnetworks in the recognition of the ‘wet’ voice,” for the award period July 1, 2018,through June 30, 2020.

Our New Residency Alumni AssociationThe Departments of Otolaryngology – Head & Neck Surgery at Weill Cornell Medicine andColumbia University worked together to create a new Residency Alumni Association in 2017,which includes graduates of all Otolaryngology residency programs that are or wereassociated with Weill Cornell Medical College, Columbia University, and Manhattan Eye, Ear,and Throat Hospital.

Our Residency Alumni Association hosts an annual dinner during the yearly, two-daycomprehensive otolaryngology update course held in New YoYoY rk City. During this dinner, alumniand faculty, as well as current residents and fellows, are able to connect and network, fofof rmingand re-establishing lasting and valuable professional relationships that span generations.
