LET’S GET STARTED
GO TO: https://gopatrickfl.com/Click on the ID picture on the right side of screen – This will bring you to the MILITARY PERSONNEL FLIGHT page.
Step 1. Scroll down and find “Military/Retiree/Civilian Dependent ID Card Renewal/Replacement” and click the “Retiree & Dependent ID Card Renewal Application”
Once complete, save the application to your computer.
Please fill this form completely.
This is the “Retiree & Dependent ID Card Renewal Application”
Step 2. Click on the DMDC website link to certify the DD Form 1172-2 or go to: https://www.dmdc.osd.mil/self_service
Click on “Sign In”
Click on “OK”
If you have a DS Logon or CAC, please log in.
If you do not have a CAC or DS Logon, please create a DS Logon. Select the drop down option “More DS Logon Options” and follow instructions to create a logon.
NOTE:If you experiences problems creating a DS Logon, please call DMDC at 1-800-538-9552 for assistance
After your DS Logon is created, return to the logon screen, enter your Username and Password, click “Login”
Under Family Members’ Contact Information & ID CardsClick on “Replace ID Card” for the dependent that needs the ID card renewed
Note: If the dependent ID card has expired you will not be allowed to use this site to obtain a DD Form 1172-2. Consult with a 45 FSS Customer Support Representative (321-494-6144) to complete the form.
Click on “Proceed”
Click on “Proceed”
1. Type in DS Logon User Name and Password2. Then click on “Reauthenticate”
Click on “Continue”
You will receive the following message “Your DD Form 1172-2 has been submitted”. You can save a copy by clicking on display form.
Before you proceed, please gather the following documents for submittal:
• Retiree & Dependent ID Card Renewal Application (Completed in step 1)• Photo of Two Forms of Identification (check next page for accepted IDs)• Acceptable Photo of member renewing ID card• Medicare card (only if turning 65)
Any two forms of ID from the list provided below will be acceptable.
Unacceptable & Acceptable photos
Photo must be from the shoulders up, with a white/off-white background. Please ensure the photo is not blurry and there is no glare on the member’s face. Member should also not have glasses on, hair in the face, or wearing a hat.
Step 3. Click on the link for “AMRDEC SAFE” or go to https://safe.amrdec.army.mil/safe

1. Individuals Using a CAC Card will select “CAC Users”2. Individuals without a CAC Card will use “Non-CAC Users”
1. Enter Your Name2. Enter e-mail address3. Confirm e-mail address4. Select “Browse and Upload” the
following documents from your computer and/or Smart Device:• ID Card Renewal Application
(Completed in step 1)• Photo of Two Forms of
Identification • Acceptable Photo of
applicant• Medicare card (only if
turning 65)5. In the Description of File(s): state
what items are being attached
1. Enter [email protected] in the e-mail address box2. Click “Add”
Verify [email protected] e-mail address listed in the Recipients List:
1. Select Notify me when file(s) downloads are STARTED2. Select Notify me when file(s) downloads are COMPLETED3. Click on “None”4. Click on “Upload”
Your items have been submitted to the 45th FSS Customer Support Section for processing. You will receive email notification from ARMDEC when your files are downloaded. The 45 FSS Customer Support Team will notify you via email if there is an error with your package or when your ID card is ready for pick up.
