Referred as CHF or heart failure. A condition that reduces the ability
of the heart to pump blood effectively.

Fluid ManagementOutcome: Patient will weigh heim/her self daily in order to monitor for fluid retention * keep a daily log of fluid
intake * monitor for weight gain 3 lbs
in 1 week * monitor for feet and leg swelling * monitor for shortness of breath
Outcome: Patient will follow with prescribed medication regimen
* know the side effect of each medication
* take diuretic medication early in the morning, prevent interruption to sleep
* record daily pulse & blood pressure
DIET
EXERCISEOUTCOME: PATIENT WILL PERFORM DAILY EXERCISE AS TOLERATED* WALKING FOR 10-30 MINUTES OR TOLERATED* EXCESSIVE PHYSICAL ACTIVITY CAN FURTHER WEAKEN THE HEART
