Behandeling
• Inductie therapie:
• Start Prednisolon 40-60 mg/dag.
• Indien oogheelkundige betrokkenheid: opname voor 3 maal 1000 mg Methylprednisolon (Solu-medrol) i.v.
• Afbouwschema:
• Indien klinische en biochemische remissie na 3-4 weken bereikt is, start afbouwschema:
• met 10 mg/2 weken tot 20 mg/dag
• daarna met 2,5 mg/4 weken (mits remissie is bereikt) tot 10 mg/dag
• daarna met 1 mg/4 weken (mits remissie is bereikt) tot 0 (zie voorbeeld PMR).
Co-mediactie
• Omeprazol 20mg
• Alendroninezuur 70 mg/week
• Calcichew D3 500mg/ Vit2 dd
• Zo nodig paracetamol 3 dd 1 gram
• Naproxen max 2 dd 500 mg
• Carbasalaat calcium = ter discussie
Monitoren
• Ziekte activiteit GCA; symptomen systemische dan wel craniele GCA + een verhoogde BSE dan wel crp
• Schade GCA; Screen in hoog risico groep op AA dmv X-Thorax AP en Lateraal (TTE op indicatie) en echo abdomen ieder 2 jaar
• Bijwerkingen; Maag, BDM en Vit D

Background
• PMR is the most common inflammatory musculoskeletal
disease in adults >50 years old
• Glucocorticoids are the mainstay of treatment
• Unmet need for alternative/adjuvant treatments
– 50% of patients relapse or have difficulties to reduce glucocorticoids
– Glucocorticoids have short-term and long-term side-effects/toxicity
• Low dose Methotrexate low-moderate GC-sparing effect
– 2 blinded RCTs and 4 open label studies
Why leflunomide
• Leflunomide has shown effectiveness in 2 case
series
Dasgupta et al. 2012, Diamantopoulos et al 2013
• Effective, inexpensive and safe DMARD
• Inhibits
– the dendritic cell maturation and their T cell stimulatory
capacity
– production of IL-17, TNF-α, IL-2, IL-6,IL-12
A randomized, double-blind, placebo controlled study
� Prednisolone tapering:
week 1-4 prednisolone 15 mg once daily
week 5-8 prednisolone 12.5 mg once daily
week 9-12 prednisolone 10 mg once daily

Consensus criteria for PMR
Response will be assessed by
(1) > 70% patient global improvement,
(2) > 70% reduction in morning stiffness
(3) Normal CRP (<5) and / or ESR (< 30)
Relapse
Signs of active PMR (aching and stiffness at shoulder, hip girdle, or both)
Increased ESR (>30 mm/h) and/or CRP (>5 mg/L).
Recurrence if it was observed after steroid withdrawal.
Criteria for remission
No signs of active PMR (aching and stiffness at shoulder, hip girdle, or
both) and a normal ESR and/or CRP

Polymyalgia Rheumatica en Arteriitis Temporalis
• VERWIJZING OF CONSULTATIE
• Verwijs bij:
• acute visusdaling, (geheel of gedeeltelijk) gezichtsveldverlies of dubbelzien met spoed naar de oogarts;
• vermoeden van arteriitis temporalis naar een reumatoloog/internist binnen 24 uur;
• ernstige bijwerkingen van of contra-indicaties voor langdurig gebruik van glucocorticoïden naar de reumatoloog/internist voor een eventuele behandeling met methotrexaat.
Acute zorg bij uitval
• De diagnostiek dient zo georganiseerd te zijn dat er zo min mogelijk tijdverlies plaatsvindt. Het dient adequaat en tijdefficiënt plaats te vinden. Met ander woorden bij voorkeur georganiseerd op 1 dagdeel
• Echo/doppler en of Pet/CT• Arteria Temporalis biopt• Bij visus klachten spoedconsult OHK• Bij Amaurosis fugax spoedconsult neuroloog• Bij aortitis ook infecties zoal lues uitsluiten en
bloedkweken doen

• OHK betrokkenheid 30%, kan nog ontstaan na start steroiden
• Biopt is nog steeds gouden standaard, maar kan negatief zijn
