Essential Elements Checklist
• Evidence of a written curriculum, tailored to meet the needs of the target population showing principles of AADE7® behaviors
• The curriculum is reviewed at least annually and updated as appropriate to reflect current evidence, practice guidelines and its cultural appropriateness
• Curriculum reflects maximum use of interactive training methods either in individual or group sessions

• The DSMES needs will be identified and led by the participant with assessment and support by one or more DSMES team members. Together, the participant and DSMES team member(s) will develop an individualized DSMES plan

Self-Assessment
• Completed by the individual prior to the first meeting
• Should be appropriate for the population the program serves
• Tailored also to meet the needs of any individual participant
Individualized Education Plan
• At least one complete, individualized education plan for each participant
• Include interventions and desired outcomes
• Develop plan COLLABORATIVELY with participant, caregivers, and providers
SMART Goals
• Action-oriented behavioral goals must be clearly documented in patient care plan
• Follow-up discussion of progress towards achieving goals and/or identified gaps in achieving goals should be clearly documented

On-going Support
• Document at least one individualized follow-up support plan
• Examples include telephone follow-up and use of other information technologies (e.g., Web-based, text-messaging, or automated phone calls), and may be used to augment face-to-face follow-up, progress assessments
