BMS EXTRA-CURRICULAR PROGRAM OPPORTUNITIES
Fall 2018
Please complete and return the attached Registration/Consent form if your child wishes to
participate in an extracurricular program. Forms must be completed in full with payment (check
or money order-no cash) for all programs. Cost of all programs are listed in the description. Fee
is nonrefundable. Please write one check for each program registration; it makes the process
easier if a program is cancelled and checks need to be voided and returned.
Most programs require enrollment of at least 10 students; unless otherwise stated. Programs
may be cancelled if less than 10 students enroll. If registrations exceed program limit, a lottery
system will be used.
Parents must pick up all students at the conclusion of their extra-curricular program. This is a
school policy and assures that students are being picked up by their parent/guardian or
authorized adult. If a student is to be picked up by someone other than a parent/guardian, the
student must present a note to the office at the beginning day in which program is scheduled.
All persons picking up students must have a photo ID. Thank you in advance for your
cooperation.
