Does rhinitislead to asthma?

The GA2LEN campaignhighlights the importance of the

link between sneezing and wheezing:

• Patients with allergic rhinitis should beevaluated for the presence of asthma

• Patients with asthma should be evaluated for the presence of rhinitis

The brochures for Primary Care Physicians and for Patients are today available in

12 languages and are downloadable from the website www.ga2len.net. The campaign

is jointly conducted with EFA, EuropeanFederation of Allergy and Airway

The project includes the translationof “diary cards” of the DARE studyin all the languages of participatingcentres. The WP could pilot thediary cards in a small number ofpatients from all centres when theethical permission has beengranted.

Diary cards allow patients to reporttheir symptoms and the medicationthey took day after day. In theDARE study, they will be anessential tool to help describe whathappens during an asthmaexacerbation and compare withclinical definitions from both thepatient and physician perspectives. The observations in the diary cardsin combination with for instancepeak flow rates, bronchialinflammation measures, will be the

Allergy is often under-diagnosed inathletes and exercisers and nospecific allergy diagnostic protocolis available in sports medicine.

Within the activities of WP 2.8.2,two screening questionnaires havebeen developed by Centre INMM-CNR, Rome, for predicting allergyin elite athletes and in non-professional exercisers. The finalversion of AQUA (25 questions)

Under the Nutrition Survey we willuse a common instrument to assessfrequency of consumption of over200 foods in the last 12 months.Information on foods normallyconsumed in European countries(including staple foods) wascollected and put together in oneFood Frequency Questionnaire (FFQ)produced in English. The FFQ hasbeen translated into 5 Europeanlanguages: Finnish, German, Greek,Portuguese and Spanish, followingstandard translation procedure asrecommended by the WHO.Forward and back-translations weredone to ensure that the local FFQswere identical to the original version.

The piloting of the FFQ will assessthe validity of the questionnaireand its comparability acrosscountries. After this phase, a cross-sectional survey will be carried outin as many countries as possiblewithin Europe. All partner centresand research institutions currentlyparticipating in the GA2LENNetwork are being invited to jointhe survey. Otheracademic/research institutionsoutside the Network that areinterested in participating cancontact us for more details.

Avoidance/reduction of allergenexposure and treatment via specificimmunotherapy or oral desensitizationwere discussed in the workshops.

A previous 2005 GA2LEN workshopconcluded the need for an update oncurrent knowledge rather than a needfor designing new studies. Theworkshop naturally confirmed theinterest in the topic.

The “state of the art” workshop aimedto create a platform for future studiesin the field of allergy prevention. Threeareas were specifically addressed inmini-workshops: food allergy, inhalantallergy and specific immunotherapy. The groups' objective was to identifyareas in the allergy prevention field inwhich evidence was lacking as well asareas for future research. The workshop

writing abstracts and presenting data, interspersed with the scientific talks onallergy and asthma research, treatmentand prevention. The participants enjoyeda stay at the beautiful campus of St.Catherine's, and when not participatingin talks and workshops or eatingdelicious food in the dining hall, theywere introduced to the English sport of“Rounders” and treated to a field trip tonearby Blenheim Palace.

The Allergy School in Cluj Napoca, from5 - 10 September, featured severalGA2LEN speakers, including TorstenZuberbier from the Charite in Berlin,Jean Baptiste Watelet from GhentUniversity, and Peter Howarth fromSouthampton. Three of the days werejam-packed with talks on the treatmentof allergy and asthma. Non-scientifichighlights of the program included visitsto the World Heritage Site Sibiu anddinner in the Romanian village Albota.

The Allergy School in Bischoffsheim,21-24 September, featured the topic“Recombinant Allergens: FromFundamental Aspects To ClinicalApplications”. The conference consisted

of lectures by experts in the field, interactive workshops with presentationof clinical cases prepared in a pedagogicway by clinicians orchestrated by C.Metz-Favre (France), laboratory workorganized by Ines Swoboda (Vienna),and round-table discussions.Additionally, the participants visited awine cellar, ate traditional Alsatian foodand explored the historic centre ofStrasbourg.

The Allergy School in Crete took placefrom 30 June- 3 July. 71 participantsfrom 13 countries and 20 speakers tookpart in the program, whose theme was“The war against the allergic march”.The conference featured two hands-onworkshops in addition to the usual talks, and there were many fun socialactivities including beach sports, atraditional Greek meal with dancing, andvisits to archaeological sites. The schoolwas organised by Nikos Papadopoulos,GA2LEN Team Leader in Athens.

The Allergy School in Helsinki on“Prevention and Preventive Treatment ofAllergy and Asthma” took place from16- 18 August and was attended by105 participants. The three subtopics atthe conference were “Is there a rationalefor allergy prevention?”; “Preventativetreatment of asthma and wheezing”;and “Allergic symptoms are induced byallergens. How to avoid and treat toprevent?” Participants also enjoyed anevening cruise to Suomenlinna.

Gender

In your opinion, how canGA2LEN help more womenscientists to succeed?

The strong advantage of GA2LEN is tobring together people and researchersfrom all over Europe. The networkfacilitates meetings with high levelscientists. Contacts seem easier thanduring regular congresses, probablybecause researchers gather aroundthe shared objective to advanceresearch on a common theme. Theenvironment is also less competitivewithin GA2LEN.

I have the feeling that women andmen move together in the networkand that women are on a more equalfooting in the network than outside.

I don't know how GA2LEN could dobetter than this.

How many women are there inyour department? Is itimportant that women reachleadership level in research?

There is quite a high percentage ofwomen in my department, which ismaybe not the usual pattern. Ourdirector is a man but there is abalance between women and men andquite a lot of women team leaders.

Having a gender balance in the teammay create a more pleasant workingenvironment. However, I'm not surehow having more women leadersaffects the scientific work.

Why does gender imbalancehappen?

There is a gender imbalance inscientific research, as in many other

professions. In our environment, it isespecially true for higher positions, notably leadership positions, whichare mainly filled by men.

I think it is partly cultural. Men areprobably more inclined to acceptresponsibilities than women. A lot ofwomen do not go after suchpositions, especially if they have afamily life, children… In the end, they are not putting their candidatureforward whereas men look formanagement/leadership positions. There might also be some genderdiscrimination during recruitment. Finally, I think that the firstdiscrimination is done by womenthemselves.

What can be done to helpimprove the situation?

Scientific research, as other fields ofwork, is competitive and requires alot of personal commitment if youwant to reach a high level. Thisincludes professional commitment, time, but also regular trips related toyour responsibilities.

The inequality between women andmen is obvious. But I don't know howthis situation should be improved.

Imbalance is especially clear whenwomen have children. They are lessavailable for their work in the firstyears of life of their children. Asystem should maybe be put in placethat would take into account women'sfamily lives in their carriers. Forinstance, we may imagine “to adjust”the number of publications on thenumber of children they have. This isnot automatically done at the moment.

I believe women can combine familylife and careers. It is up to them to

make it possible but it requiressome multitasking. There are only24 hours in a day and it is difficultto do everything well. It is true thatthe investment of a woman withchildren is sometimes weaker: shewill work fewer hours per day.

Women have to choose how tomanage this situation: you cancombine family and career and doboth properly and satisfactorily ifyou do not have the same highexpectations for your career. Youhave to find a balance between theinvolvement you put in your familylife and your professional life. Itbecomes easier to develop yourcareer later, when children growand are independent.

Would you be in favour ofsetting targets? For example, for achieving 50% womenheads of GA2LEN workpackages by 2007?

Are targets a way to solve theproblem? I would consider targetsas an objective but not as a meansto reach gender balance.

I don’t think targets are justified if candidates have equalcompetencies. Setting targets is away to show a political will forgender balance, but by trying todecrease some inequality, you mayincrease or create new ones in othergroups.

For each open position, you canprobably find 2 or 3 fully qualified, competent people in a workinggroup. On the other hand, if there’sa consensus around two equalcandidates with equal competencesand qualifications, why not to

Lithuanian allergists are closely linkedto the community of Europeanallergists by teaching as well as byresearch. In 2005 already, Lithuaniahas organized the first Baltic AllergyCongress together with EAACI, andthe PAPRICA symposia were anopportunity to specifically addressprimary care physicians.

State-of-the-art lectures wereheld on five topics: “Howand when to diagnoseallergy in childhood”,"Respiratory allergy",“Atopic dermatitis”,“Food allergy”, and"Immunotherapy".The speakers werePeter Schmid-Grendelmeier, ChristianVirchow, Erkka Valovirta,Jonathan Hourihane andPhilippe Eigenmann. The topics werehighly welcomed by the participantswho had a large number of questionsregarding very practical aspects ofpatient care.

In total, close to 200 participantsattended the two Symposia. They

came from various regions ofLithuania, some delegates even camefrom neighbouring Latvia.

Allergy and clinical immunology has along standing history in Lithuania. However, as in other Europeancountries, the speciality has tostruggle to promote the specificity ofallergic diseases in the various age

groups. It was largely felt thatPAPRICA will help to further

promote allergy care inLithuania.

Nevertheless, similarlyto the previousPAPRICA sessions, these Symposia were a

unique opportunity todirectly address doctors in

their working area, as wellas to establish new and long

lasting friendship.

encourage women who may not putthemselves forward.

Women mostly need support and tobe more highly valued if they are togo for leadership positions. Targetshave the big disadvantage of makingyou the “quota” woman and thereforecreate the opposite effect.

A gender work package alreadyexists. Would you be in favourof setting up a gender taskforce?

A task force would be useful as it willprompt discussion on genderequality. Gender issues are notdiscussed enough. I don’t knowabout the political ways to improve

the situation but talking about theproblem will raise awareness ofwomen’s position in research. Yes,they are sometimes less availablethan men, but women have equalcompetences and should be takeninto account.

People should also be aware that ifwomen are less available during aperiod of their working life, it doesnot affect the quality of their work.

Men could take more responsibilitiesat home, in their family life. In thelong term, achieving equality at homemight help tipping the balance atwork. But I think women will alwaysbe less present when they haveinfants. People have to accept thisfact.

I feel quite serene in my position. The recruitment to the INSERM isdone via a competition, withoutgender discrimination during theprocess. It is probably quite aprotected environment in comparisonto other sectors.
