DescriptionICHTHOPASTE is a bandage made of openwove cloth impregnated with a pastecontaining 6.32% Zinc Oxide BP and 2%Ichthammol BP rolled onto a plastic core. Italso contains Purified water BP, GlycerineBP, Gelatin BP, Emulsifying wax BP,Phenoxyethanol BP. Each bandage ispresented in a wrapper and sealed in apolythene bag contained in a cardboardcarton.

Mode of actionICHTHOPASTE provides topical treatment. Itprovides a moist wound healingenvironment over a venous leg ulcer, helpsto reduce skin irritation and soothes andprotects surrounding skin. It will not dry outin use.

IndicationsICHTHOPASTE is indicated to assist themanagement of leg ulcers. Where venousinsufficiency exists, the paste bandageshould be used under graduatedcompression bandaging. ICHTHOPASTE isalso suitable for use in the management ofchronic eczema/dermatitis, whereocclusion is indicated.

Contra-indicationsDo not use in known cases of sensitivity orallergy to any of the ingredients.

PrecautionsWhere venous insufficiency exists,ICHTHOPASTE should be used undergraduated compression bandaging, afterfirst assessing the patient to excludearterial disease. The use of Dopplerultrasound is recommended for thispurpose. Compression bandaging shouldnot be used on patients who have arterialdisease (ABPI <0.8) and is notrecommended for use on diabetic patientswith advanced small vessel disease.Failure to detect reduced arterial flowcan result in pressure necrosis,amputation, or even death. The risk of apatient having arterial as well as venousdisease rises with age. The skin of legulcer patients is easily sensitised to topicalmedicaments, including preservatives.Sensitisation should be suspected inpatients particularly where there isdeterioration of the surrounding skin. Suchpatients should be referred for specialistdiagnosis, including patch testing. One ofthe functions of occlusive bandages is toincrease absorption. Care should be taken,therefore, if it is decided to apply topicalsteroid, anaesthetic or retinoid preparationsunder the bandage, as their absorptionmay be significantly increased. This maylead to a shorter duration of effect of atopical anaesthetic product in addition toincreasing the risk of sensitisation to thesepreparations. For topical use only.ICHTHOPASTE is a single use only product.If used on more than one patient, cross-contamination or infection may result. Oncethe packaging has been opened, unusedmaterial should not be retained forapplication at a later date.

Instructions for useThis product is ready for use upon removalfrom the sealed pouch and wrapper. Thereare two ways that the bandage can beapplied:1. Beginning at the base of the toes, thebandage should be loosely wrappedaround the foot, heel and around the leg ina spiral fashion, to just below the knee.Once applied, the bandage should then be smoothed and moulded around the leg.2.Beginning at the base of the toes, thebandage should be loosely wrappedaround the foot and heel and then, whilstwrapping from the ankle, with every turn,the bandage should be folded back onitself in a pleat, at the front of the leg. Thisshould be repeated up the leg until justbelow the knee. Compression bandagingmay follow. Once ICHTHOPASTE has beenapplied, the leg should be covered by abandage or dressing to prevent soiling toclothes.

Recommended duration of use adultsand childrenChange bandage as required, can be wornup to 7 days.

Additional informationIchthammol (of itself, not related to theocclusive nature of the product) mayincrease the absorption of corticosteroids,heparin and chloramphenicol through theskin. Ichthammol has also been shown tosynergise the activity of oxytetracycline andantagonise the activity of ampicillin whenpresent together in various ointment bases.

VirkemådeICHTHOPASTE anvendes til topiskbehandling. Den giver et fugtigtsårhelingsmiljø over et venøst bensår, ermed til at reducere hudirritation og lindrerog beskytter den omgivende hud.Bandagen tørrer ikke ud under brug.

IndikationerICHTHOPASTE er indiceret til anvendelse ibehandlingen af bensår. I tilfælde af venøsinsufficiens skal pastabandagen anvendesunder gradueret bandagering medkompression. ICHTHOPASTE er også egnettil anvendelse i behandlingen af kroniskeksem/dermatitis, hvor okklusion erindiceret.

KontraindikationerMå ikke anvendes i tilfælde af kendtoverfølsomhed eller allergi over for et ellerflere af indholdsstofferne.

ForsigtighedsreglerI tilfælde af venøs insufficiens skalICHTHOPASTE anvendes under gradueretbandagering med kompression, efter atpatienten er undersøgt for at udelukkearteriel sygdom. IndicationsICHTHOPASTE est indiqué dans la prise encharge des ulcères de jambe. En casd’insuffisance veineuse, la bande imprégnéesera accompagnée d’une bande decontention progressive. Précautions d’emploiEn cas d’insuffisance veineuse, utiliserICHTHOPASTE sous une bande de contentionprogressive après avoir écarté touteartériopathie à l’examen du patient. A cettefin, l’examen Doppler est recommandé. Labande de contention ne devra pas êtreutilisée chez le patient atteint d’artériopathie(IPS <0,8). Elle n’est pas non plusrecommandée chez le patient diabétiqueatteint de microangiopathie au stade avancé.L’absence de détection d’un débit artérielfaible peut avoir des conséquencessérieuses : nécroses de compression,amputation, voire décès. Le risqued’atteinte mixte (à la fois veineuse etartérielle) augmente avec l’âge. La peaudes patients souffrant d’ulcère de jamberéagit facilement aux médicaments topiques,y compris aux agents de conservation. Onsoupçonnera tout particulièrement unesensibilisation chez le patient présentant unedégradation des tissus cutanés environnants.Dans ce cas, le patient devra être orienté versun service adapté pour y subir un examenspécialisé, notamment un test épicutané. Lespansements occlusifs ont entre autres pourfonction d’augmenter l’absorption. Uneattention particulière devra donc être portéeen cas d’application topique de préparationsà base de stéroïdes, d’anesthésiques ou derétinoïdes sous la bande, car il est possibleque leur absorption en soit considérablementaccrue. L’effet du produit anesthésique pourraalors être écourté et le risque desensibilisation à de telles préparations accru.À usage topique uniquement. ICHTHOPASTEest un produit à usage unique. L’utilisationchez plus d’un patient peut entraîner unecontamination croisée ou une infection. Aprèsouverture du sachet, tout pansement nonutilisé ne doit pas être gardé pour uneapplication ultérieure.

Mode d’emploiUne fois retiré de son sachet hermétique etde son enveloppe protectrice, ce produit estprêt à l’emploi. Deux méthodes d’applicationsont possibles :1. En partant de la base des orteils, appliquerla bande en spirale (sans serrer) autour dupied, du talon et de la jambe jusqu’au-dessous du genou. Une fois posée, lisser lasurface de la bande pour qu’elle mouleparfaitement la jambe.2. En partant de la base des orteils, appliquerla bande (sans serrer) autour du pied et dutalon, puis à partir de la cheville, replier labande sur elle-même sur le devant de lajambe à chaque spire. Répéter ce gestejusqu’à atteindre le dessous du genou. Fairesuivre d’une bande de contention sinécessaire. Une fois l’applicationd’ICHTHOPASTE terminée, recouvrir la jambed’une bande ou d’un pansement afin de nepas tacher les vêtements.

Durée d’utilisation recommandée chez lesadultes et les enfantsChanger le bandage si nécessaire, peutrester en place jusqu’à 7 jours au maximum.

Autres informationsL’ichthyolammonium (en soi, sans égard à lanature occlusive du produit) est susceptibled’accroître l’absorption cutanée descorticostéroïdes, de l’héparine et duchloramphénicol. On a également montré quel’ichthyolammonium exerce un effetsynergique sur l’activité de l’oxytétracyclineainsi qu’un effet antagoniste sur l’activité del’ampicilline lorsque ces deux composantssont tous deux utilisés en tant qu’excipients.

Meccanismo d’azioneICHTHOPASTE costituisce un trattamentolocale. Fornisce un ambiente umido per lacicatrizzazione della ferita in corrispondenzadell’ulcera venosa dell’arto inferiore, è utilenella riduzione delle manifestazioni irritative,decongestiona e protegge la cuteperilesionale. Non si secca durante l’uso.

IndicazioniICHTHOPASTE è indicato come coadiuvantenel trattamento delle ulcere degli artiinferiori. In presenza di insufficienza venosa,la benda con pasta dovrebbe essereimpiegata effettuando un bendaggio acompressione graduale. ICHTHOPASTE èadatto anche nella gestione di eczemi odermatiti croniche, nei casi in cui sia indicatal’occlusione.

ControindicazioniNon utilizzare nei casi accertati di sensibilitào allergia a uno qualsiasi dei componenti.

PrecauzioniIn presenza di insufficienza venosa,applicare ICHTHOPASTE effettuando unbendaggio a compressione graduale, dopoavere esaminato il paziente per escludereun’arteriopatia. A tale scopo si raccomandal’impiego di ultrasonografia Doppler. Noneffettuare il bendaggio compressivo inpazienti con arteriopatia (ABPI < 0,8); se nesconsiglia l’uso nei pazienti diabetici conmicrovasculopatia in stadio avanzato. Lamancata diagnosi di un ridotto flussoarterioso può portare alla necrosi dapressione, all’amputazione e persino allamorte. Il rischio di un’arteriopatia inassociazione alla patologia venosaaumenta con l’età del paziente. La cutedei pazienti affetti da ulcere degli artiinferiori è particolarmente sensibile aitrattamenti topici, compresi i conservanti. Sideve sospettare la sensibilizzazionesoprattutto nei pazienti che presentano undeterioramento della cute perilesionale. Talipazienti devono essere inviati allospecialista per l’esecuzione di patch test afini diagnostici. Una delle funzioni delbendaggio occlusivo è aumentarel’assorbimento. Si deve dunque fareattenzione, qualora si decida di applicaresotto il bendaggio preparazioni topiche abase di steroidi, anestetici o retinoidi, dalmomento che il loro assorbimento puòaumentare notevolmente. Ciò può ridurre ladurata dell’effetto degli anestetici locali, oltreche aumentare il rischio di sensibilizzazionea tali preparazioni. Solo per uso topico.ICHTHOPASTE è un prodotto esclusivamentemonouso. Se utilizzato su più di unpaziente, potrebbero verificarsicontaminazioni crociate o infezioni. Unavolta aperto il confezionamento, il materialenon utilizzato non deve essere conservatoper essere applicato in un secondomomento.

Istruzioni per l’usoUna volta estratto dal sacchetto sigillato edall’involucro, il prodotto è pronto per l’uso. Ilbendaggio si può applicare in due modi:1. Partendo dalla base delle dita del piede,avvolgere senza stringere la benda attornoal piede, al tallone e alla gamba procedendoa spirale, fin sotto il ginocchio. Una voltaapplicata, fare aderire e modellare la bendaattorno alla gamba.2. Partendo dalla base delle dita del piede,avvolgere senza stringere la benda attornoal piede e al tallone e quindi, mentre siprocede dalla caviglia, a ogni giro ripiegarela benda su se stessa sul lato anterioredella gamba. Ripetere per tutta la gamba finsotto il ginocchioo. In seguito si puòapplicare un bendaggio compressivo. Unavolta applicato ICHTHOPASTE, coprire lagamba con una benda o una medicazione perevitare di sporcare gli indumenti.

Durata d'impiego raccomandata peradulti e bambiniIl bendaggio va cambiato secondo lenecessità, può rimanere in sede fino a7 giorni.

Ulteriori informazioniL’ittiolo (di per sé e non in relazione allanatura occlusiva del prodotto) puòaumentare l’assorbimento transcutaneo deicorticosteroidi, dell’eparina e delcloramfenicolo. È stata dimostrata ancheun’attività sinergica tra ittiolo eossitetraciclina e un antagonismo tra ittioloe ampicillina quando queste sostanze sonoentrambe presenti nelle basi per unguenti.
