Entrance to SchoolKindergarten
A child may enter kindergarten in September if s/he will be five years oldon or before December 1st of the school year. Kindergarten registration will beheld in March. Parents of pre-school-age children, who are on the school cen-sus, are notified of time and place of registration. All others should come to theCensus & Enrollment Office at the Howitt Middle School to prove residency andbe put on the school census.
First GradeA child who has had a full year of kindergarten in an approved school will
be admitted to the first grade. Students who are six years old and have notattended kindergarten will be entered initially into kindergarten and advancedaccording to ability.
New Residents and Transfer StudentsDuring the school year, new residents are admitted at the District
Census and Enrollment Office in the Howitt Middle School, which is located onVan Cott Avenue, Farmingdale, Monday through Friday, 8:00 am to 3:30 pmDuring the summer, enrollment for new residents and transfer students is heldMonday through Thursday, 8:00 am to 2:00 pm, and Friday from 8:00 am to11:00 am.
Proof of residence, original birth certificates, six state-mandated immu-nizations, health records and prior school report cards are needed for registra-tion. For further information, call 516-752-6555.
Special EducationSpecial education services are provided at all levels K-12 in the Farmingdale
School District. In addition, a pre-school student with a disability may have specialneeds that require appropriate services. All children are evaluated and recommend-ed for services by the Committee on Special Education (CSE) or the Committee onPre-School Education (CPSE). If parents or guardians suspect their school-age childhas a disability, they can refer their child to the Committee on Special Education bycalling 516-752-6591. For information about a pre-school child who may have a dis-ability, please call the Special Education office at 516-752-6515. Referrals in writingshould be sent to the Farmingdale Schools Special Education Department, 50 VanCott Avenue, Farmingdale, NY 11735.
ImmunizationsSTUDENTS WILL NOT BE ADMITTED TO SCHOOL IF IMMUNIZATION
REQUIREMENTS ARE NOT MET.New York State law requires written proof of the following immu-
nizations for school entry: 3 doses DPT, 3 doses Polio, 2 Measles, 1 Mumps,1 Rubella (or 2 MMR), 3 doses Hepatitis B, 1 Dose of Varicella (Chicken Pox).Students entering grade 6 need 1 dose of Varicella and 1 booster of Tdap.Acceptable proof must be from a doctor. Parental history is not acceptable.Students transferring from a parochial school to a public school are considerednew entrants.
Continuing Community EducationFarmingdale maintains one of the most extensive continuing community
education programs on Long Island. A publication describing the various courseofferings is mailed to district residents early in August and early in January of eachyear. Information about registration procedures and schedules is included in thecourse offerings publication. For additional information, please contact the Directorof Continuing Community Education at 516-752-6608 between the hours of 9:00 am- 1:00 pm and 6:00 pm - 9:00 pm while classes are in session.
Senior Citizens of Farmingdale, Inc.Senior Citizens of Farmingdale, Inc. is sponsored by Farmingdale Public
Schools Continuing Community Education and the Incorporated Village ofFarmingdale.
The program is open to school district residents over 55 years of age andhas regular meetings two afternoons per week (W&F) from 1 pm to 4:30 pm at theFarmingdale Public Library on Merritts Road (when school is in session). For furtherinformation, please call Adelaide Ketcham, Coordinator, at 516-753-0291.
Upon request, the district issues V.I.P. Courtesy Cards to residents who are62 years of age, which entitles the resident to free entrance to all school activities,plays, concerts and athletic events. In addition, Continuing Education courses willbe available without registration fee to all residents 62 years of age or older, on aspace-available basis. Exception: Registration fee for contractual service coursesmust be paid for in the usual manner. You may obtain a V.I.P. Courtesy Card by call-ing 516-752-6555.
Working PapersThe School District cooperates with the New York State Labor Department in
issuance of working papers required by law for students under the age of eighteen whoseek employment. All children who are residents of the district and who have the needfor such papers should complete an application form obtained from the High SchoolHealth Office (516-752-6613).
To secure working papers, a student must show proof of a physical examinationwithin the last year and submit proof of age in the form of a birth certificate, baptismal cer-tificate, hospital record or passport. For employment as a newspaper carrier, specialapplications and permits are available at the Howitt Middle School.
Emergency Closing/Delayed Opening of SchoolArrangements have been made to notify the general public whenever
schools are to be closed or if there is to be a delayed opening because of weatherconditions. Announcements will be broadcast intermittently over the following sta-tions. Free or Reduced Price LunchStudents whose families meet the financial criteria set by the government may
receive a free or reduced price lunch. Applications can be obtained at your child's schooland should be completed and returned as soon as possible. All applications will bereviewed for approval.
Pesticide NotificationEducation Law requires that all schools establish a pesticide notification procedure.
Part of that procedure includes establishing a registry of names and addresses of peoplethat wish to be notified prior to any application. Although the school district does not usepesticides, there may be an occasion where we have to. If you wish to be placed on thenotification registry, please contact the Office of Districtwide Operations (516-752-6545).For additional information, please contact the New York State Education Department web-site at www.emsc.nysed.gov/facplan.
Asbestos Abatement NotificationThe Asbestos Hazard Emergency Response Act (AHERA) requires that all schools
establish an asbestos notification plan. Part of that plan includes posting a 10 day notice atthe building where abatements are planned. For further information contact the DistrictwideOperations office at (516) 752-6545.
Student Information & the MilitaryThe Patriot Act obliges the district to provide student directory information to mili-
tary recruiters unless parents have signed an opt-out request and sent it to the guidanceoffice. Parents of juniors and seniors will receive a copy of the opt-out form in Septemberby mail. The dealine for return is September 29.
Non-Discrimination PolicyThe Board of Education of the Farmingdale Union Free School District does not
discriminate on the basis of race, color, national origin, creed, religion, marital status, gen-der, age, disability, military status, genetic predisposition, carrier status or sexual orientationin the educational programs and activities that it operates, or the employment therein.
Title IX / ADA 504Notice is hereby given that the Board of Education of the Farmingdale Union Free
School District, Farmingdale, New York does not discriminate on the basis of gender, raceor disability in educational programs and activities which it operates, that is required by TitleIX of the Federal Education Amendments of 1972, and the regulations implementing thesame not to discriminate in education programs and activities extends to employment there-in and the admission thereto and that inquiries, complaints or grievances concerning theapplication of Title IX harassment issues and ADA/504 Regulations must be referred in writ-ing to Barbara J. Horsley, Assistant Superintendent for Administration, 50 Van Cott Avenue,Farmingdale, NY 11735 or the Director of the Office of Civil Rights, U. S. Department of

Family Educational Rights & Privacy Act (FERPA)In compliance with PL-93-38 and PL-142 dealing with students’ parent(s) and eli-
gible students’ rights to examine student records, the Farmingdale Union Free SchoolDistrict has established:
The right of a student’s parent(s) or eligible student to inspect and review the stu-dent’s records within forty-five (45) days of the day that the school receives a request foraccess.
The right of a student’s parent(s) or eligible student to consent to disclosure of cer-tain information contained in the student’s education records, except to the extent thatFERPA authorizes disclosure without consent for: 1) information the District has receivedprior written consent of the student’s parent(s) or the eligible student to disclose, 2) directo-ry information unless a student’s parent(s) or eligible student has opted out, as defined inthis section of the calendar; or 3) under limited circumstances certain other information aspermitted by the Family Education Rights and Privacy Act (FERPA).
The right of a student’s parent(s) or eligible student to seek to amend parts of thestudent’s education record which s/he believes to be inaccurate, misleading or in violation ofthe student’s rights. The student’s parent(s) or eligible student should write the school prin-cipal (or appropriate official) clearly identifying the part of the record they want changed, andspecifying why it is inaccurate or misleading. If the school decides not to amend the recordas requested by the student’s parent(s) or eligible student, the school will notify the student’sparent(s) or eligible student of the decision and advise him/her of the right to a hearingregarding his/her request for amendment. Additional information regarding the hearing pro-cedures will be provided to the student’s parent(s) or eligible student when notified of theright to a hearing.
Unless objection to any of the specific items of information following is submittedin writing by parents or legal guardians or by those students themselves who are over theage of 18 years, the Farmingdale School District herewith gives notice of intention to: (1) pro-vide, release or publish in the District’s Newsletter, school or local newspapers, athletic pro-grams, musical or theatrical programs, the district website, news releases and/or all of thefollowing directory information pertaining to students as may be appropriate under the cir-cumstances: names of students, pictures, grade, major field of study, participation in recog-nized school activities, extra-curricular activities and sports programs, post-high schoolplans, academic honors, achievements, awards, scholarships, examples of student workand similar information, and (2) provide and release mailing addresses, which include stu-dent names and addresses, to Farmingdale Youth Council and its affiliated organizations -Farmingdale Public Library, Farmingdale PTA and Farmingdale CARE. Students mayappear in classroom video tapes that the State Education Department requires for perma-nent teacher certification. Under Title 45, Code, Part 99, Privacy Rights of Parents andStudents, parents who do not desire release of any of the above directory information mustmake specific requests in writing to the Superintendent of Schools. Failure to make suchrequest shall be deemed consent to release, provide, or publish the directory informationduring the school year.
The right of any person to file a complaint with the U.S. Department of Education,Family Compliance Office if the Farmingdale Union Free School District violates FERPA.The name and address of the Office that administers FERPA is: Family Policy ComplianceOffice, U.S. Department of Education, 400 Maryland Avenue, S.W., Washington, D.C.20202-4605.

Parents For Megan's Law - Sex Offender Email Alert ProgramParents For Megan's Law (PFML) will actively notify you via email alert when they arenotified of the presence of a registered sex offender by Suffolk or Nassau County PoliceDepartments, any participating Long Island Municipal Police Dept. or when they obtainofficial sex offender registration information from New York State for low, moderate orhigh risk sex offenders. Information on registered offenders is always available fromthe PFML website. School Bus - Accidental InjuryInsurance laws in New York State dictate that any injury sustainedwhile using a motor vehicle, including school buses, is to bereferred to the no-fault (Mandatory Personal Injury Protection) sec-tion of the family automobile insurance. It is required that a Noticeof Claim be filed with the family insurance within the first 30 daysafter the injury.
Transportation for Private/Parochial StudentsWhether you are a first-time or returning student going to a private or
parochial school, a transportation form must be submittedNO LATER THAN APRIL 1st OF EACH SCHOOL YEAR.
